'This. refers to.the routine safety inspection conducted by Messrs. W. Baunack,
'
E. ' Collins,. D. Lew, 'and J. Wechselberger, July 31 to September 10, 1988, at_the
.0yster Creek Nuclear Generating Station.
The inspection consisted of document re-
views, interviews and observations of activities as documented in the enclosed in-
spection report. The findings were discussed with you and other members'of your
staff at the conclusion of the inspection.
Duringlthis period,' a special inspection was conducted to review the events re-
ported by you on September 2,1988 concerning the operation of the plant with the
"A" isolation condenser vent line isolated.
This event is considered significant in that the isolated vent path on the "A"
.-(
isolation condenser steam line prevented the necessary purging of noncondensible
gases from this line and had the potential to make the isolation condenser,inoper-
able. This condition apparently existed since a plant startup on August 10, 1988,
and continued until corrected on September 2, 1988.
In addition to this unknown
system status, the "B" isolation condenser was made inoperable, for maintenance
purposes, during August 23-28, 1988 and August 29 - September 2, 1988. Both iso-
lation condensers were apparently inoperable for a period of approximately 10' days.
This condition violated operability requirements for the isolation condenser as
specified .in the Technical Specifications and is considered to be a serious safety
matter in that en entire safety system was potentially inoperable for'an extended
period of time.
We would like to discuss this event with you in an enforcement conference in Region
I during the week of October 10, 1988.
In preparing for this conference, you
should be prepared to discuss the event, its root causes, safety significance, and
your sort, and long term corrective actions.
