Report of Interview with Mr. Charles F. WILKINS, as recorded by Investiga-tor R. A. MATAKAS on May 23, 1983

Charles F. WILKINS, Pullman Higgins (P-H) Level 2 Nondestructive Examination

{

(NDE) Technician, was interviewed by Investigator R. A. MATAKAS at Seabrook, NH, on May 23, 1983. Investigator MATAKAS identified himself to WILKINS by displaying his credentials. WILKINS has worked for P-H at the Seabrook <

Station ,since January 1980, and his supervisor is Ed BOWLES. During the subsequent interview, WILKINS provided the following pertinent'information. g

On or about April 18, 1983, Richard DAVIS and Ed BOWLES told WILKINS that

l PAD 0 VAN 0's NDE work was in question and there was going to be a sample l l reinspection of PAD 0 VAN 0's work. WILKINS and ACREE were given a list of l

welds to reinspect that were originally inspected by PAD 0VANO between January and April 1983. The identified ' welds were listed on a daily work sheet. Together, both WILKINS and ACREE inspected 42 of the listed welds l using the same NDE inspection method as originally used by PAD 0VANO. All

of the inspections involved socket welds. To the best of WILKINS' memory, <

he and ACREE determined that approximately 11 or 12 of the welds they inspected, in WILKINS' opinion, clearly had rejectable indication (Attachment (9) per-tains). In addition, they tried three different methods to determine if l

any previous magnetic particle (MP) or liquid penetrant (LP) inspection had been performed. They used developer, remover and visual inspection in an attempt to find any type of residuals left from a past inspection. Using ~j these methods, they did not find any evidence that a previous inspection had been performed. Using these same methods, they examined P-H NDE inspections performed by other individuals during the same approximate time period, and in most cases, determined that the inspections had been performed.

WILKINS said that prior to this investigation, there were occasions when he had suspicions wherein he personally questioned, in his mind, if PADOVANO was performing all of his assigned wt..- b; however, he never related his k

.) i suspicions to P-H supervision or management. He said that he knew for a j fact that, on one occasion PADOVAN0 turned in about 40 thermo couple removal and rad plug inspections in one day whereas normally, other in-  !

spectors would only turn in six or seven of these inspections in one day.-

He said these types of jobs are very time consuming due to the unaccess-ability of rad plugs and usually the thremo couple removal jobs are very hard to locate. He said that 40 of these type of inspections can be, and j have been done in the past; however, conditions would have to be, or were,

>31deil. WILKINS 'said that the process sheets andfthe daily work sheets- 'j that are turne( in daily by the inspectors are not routinely reviewed by the supervisor. He said these documents are turned in to the secretary, who forwards them to the records department for filing.

on occasion, been audited by both Yankee' Atomic and the authorized nuclear ,

inspectors but has never been audited by P-H.

' Other than the information provided stipra, WILKINS said that he was not aware of any other actual or suspected falsification of records nor did he know of anyone in P-H management or supervision who may have such informa-tion.
