WEEK ENDING AUGUST 14, 1987 Shearon Harris Nuclear Plant Unit 1 In a unanimous August 10, 1987 decision, the United States Court of Appeals Fourth Circuit affinned the Agency's January 1987 grant of a full power operating license for the Shearon Harris Nuclear Power Plant. In doing so, the court reaffirmed the validity of the Commission's immediate effectiveness procedures and the Agency's determination to grant an exemption from the since superceded requirement that a full scale emergency planning exercise be held one yezr prior to the issuance of a full power operating license.
l Hanford, Washington Site There has been recent press interest regarding the acceptability of theA part i Hanford, Washington site as a possible location for a HLW repository. l of the press interest is related to DOE's recent release of historic near sur- !
face groundwater monitoring data for the Hanford site. The articles have l incorrectly stated that NRC officials have indicated that the Hanford nuclear l reservation probably will not meet nuclear waste repository site criteria because l of geologic concerns. The NRC staff position has been, and continues to be, that data to support any conclusion regarding the adequacy of the site at tne Hanford and other potential repository sites can only be obtained through a site characterization program, and NRC staff have agreed to DOE's approach for beginning deep groundwater testing at the Hanford site to reach technically justified conclusions on this matter. The historic near surface groundwater data will be reviewed by the NRC staff in conjunction with the deep groundwater In j testing program as part of the ongoing 00E site characterization effort. '
addition, the NRC staff is reviewing the newspaper articles to develop a proposed letter to the newspapers to correct the inaccurate reports.
NRC/00E Comparability Physical security license conditions were issued on August 6, 1987 to the four Category I fuel facility licensees to amend their licenses to incorporate the l security plan changes for implementation of the three near-term comparability measures for stationing armed guards at MAA portals, night firing qualification for guards, and conducting 100% search of personnel and packages entering the protected area. The implementation of the 100% search requirement will be phased in during the next 12 months. This action was achieved 33 days ahead of schedule.
Etogtens_et_WbLte_Eliot two milestones were merked this week with the installation of carpet and ADP and broadband cabling on the 16th iloor at One White Flint North. Because this floor is scheduled to be the first one completed, it serves as e prototype f or determining whether the complex scheduling and coordination of the various tasks involved in preparing each floor in sequence for occupancy is realistic. The next phase will be installation of the demountable walls f or the enclosed

offices on the 16th floor, and that will be f ollowed by the f irst per manent installation of systems furniture in the building.

Limerick, Unit 1 4
An Atomic Safety and Licensing Board issued a Memorandum and Order on July 28, l 1987 which provisionally granted intervenor petitions to intervene in the staff's consideration of an amendment to the Limerick license. The proposed amendment concerns the deletion of Technical Specification reporting require-ments relating to Iodine Spiking and deletion of an 800 hour0.00926 days <br />0.222 hours <br />0.00132 weeks <br />3.044e-4 months <br /> cumulative operating limit with elevated Iodine concentrations. The ASLB has required the interveners to file the proposed contentions by August 28, 1987; the staff to file its response by September 16, 1987 and that a prehearing conference will be held in Philadelphia on September 29, 1987.
Fermi-2 An exit meeting was convened at the Fermi-2 plant site on August 7, 1987, during which time the NRC Operational Safety Team Inspection findings were presented to the licensee. The inspection was conducted over a two week period (July 27 - August 7) and principally focussed on operational safety performance. The team was comprised of NRC Headquarters and Regional Inspectors and a consultant, not directly associated with the Fermi-2 project, having the objective of obtaining an independent view of Fermi-2 operational performance. The Project Manager participated as a member of the team.
l The comprehensive inspection performed, did not identify any one specific area which could be construed to be the root cause or primary basis for the operational problems which have plagued the Fermi-2 project since mid-July 1985, and which continued to occur during the inspection period. Rather, the l inspection findings affirmed prior regional assessments that the operational {
problems encompass a broad range of plant activities including the licensee's j operating practices, procedure controls, administrative controls, surveillance, training and the conduct of corrective actions. The licensee was in general agreement with the teams findings and determinations, and in a letter dated j August 4,1987, withdraw earlier requests for concurrence to exceed 50% power, l as a result of continuing equipment problems and personnel errors which have not met the licensee's performance standards. High level organizational
, changes have been made which the licensee believes will strengthen the overall l management of Fermi-2 and improve plant operations performance.
l An inspection report detailing the Operational Safety Team Inspection findings and determinations, is expected to be issued within the next 30 days. The inspection findings will be factored into the decision relative to the readiness of Fermi-2 to operate above the current power limitations of 50%.

. Catawba Units 1 and 2 I I
On August 7,1987, the NRR staff issued a letter, pursuant to 10 CFR 50.54(f), j to Duke Power Company regarding the adequacy of the Nuclear Service Water System (NSWS) design at Catawba Units 1 and 2. The staff's concern relates to the application of General Design Criteria 5 and 44 to at least one scenario in which one unit is shut down (i.e., in Modes 5 or 6) with one of l two diesel generators out of service, and the other unit has a postulated I loss-of-coolant accident. In this situation, the single failure of an NSWS valve (i.e., failure to open) in the suction supply transfer from the normal heat sink to the ultimate heat sink would leave only one NSWS pump to serve both units. This is in conflict with the current FSAR which requires that two out of four pumps be operable. In addition to the scenario discussed above, j other more likely scenarios are also possible. I The staff requested that the licensee provide, within 10 days from the date of the letter, a justification for continued operation when one diesel is out of j service because this occurrence would put the plant in an unanalyzed !
condition. Furthermore, the staff verbally requested that the licensee notify, the NRC Incident Response Center of any such occurrence (i.e., one diesel out of service) within one hour.
It should be noted that this concern is specific to Catawba Units 1 and 2.
The McGuire Nuclear Station has a different NSWS configuration, that satisfies General Design Criteria 5 and 44. Thus, the staff has no similar concern for McGuire Units 1 and 2.
I Susquehanna PP&L announced, in a press release on August 11, 1987, that a shift supervisor at the Susquehanna Steam Electric Station had been relieved of his supervisory responsibilities following the preliminary results of a PP&L investigation of an allegation. The allegation, that he had apparently dozed off at his deck in his office adjacent to the control room while reviewing written material, was made a plant operator in a letter received on August 7, 1987 by a PP&L official. PP&L stated that they believed this to be an isolated incident at l Susquehanna. The licensee has commenced interviews with operators to I determine the extent of the problem. l l
A management meeting with NRC Region I is planned following completion of the licensee's investigation. The licensee discussed its planned actio'ns with NRC Region I in a conference call on August 10, 1987.

NRC TMI-2 CLEANUP PROJECT DIRECTORATE WEEKLY STATUS REPORT FOR AUGUST 11 - 16, 1987 l
- 1. DEFUELING
- Pick and place defueling of partial length fuel assemblies continued. Partial fuel assemblies D-12, F-13, G-12, G-13, G-14, H-11, H-12, H-13, H-14, K-11 and K-13 were loaded into defueling canisters. A total of 78 fuel assemblies have been loaded into
, canisters.

- Six full canisters were transferred to the spent pool.
- A video inspection was performed after partial assembly D-12 was removed. In this location a clear path to the lower head exists as the flow holes in the lower grid, distributor plate, grid forging and elliptical flow distributor align. The video showed a variety ;
of materials in the core support assembly including loose sandy J appearing fines and once molten resolidified material. Ne significant damage to the core support assembly was discernable. ,
The core debris in the lower head appeared to be 12 to 18 inches j deep. These results are consistent with previously obtained information from core boring and previous video inspections.
! 2. ENVIRONMENTAL MONITORING l
- US Environmental Protection Agency (EPA) sample analysis results show that TMI site liquid effluents are in accordance with regulatory limits, NRC requirements, and the City of Lancaster Agreement.
- TMI water effluents from the station (Units 1 and 2) discharge are analyzed by EPA. Gamma spectrum analyses of the seven daily composited samples for July 25 - August 1,1987 indicated no reactor related activity.
- EPA's gamma spectrum analysis of the NRC's TMI outdoor air sample for August 6 - 13, 1987 showed no reactor related radioactivity.
- The water works for the City of Lancaster composited seven daily samples from July 26 - August 1, 1987. EPA's gamma spectrum analysis of the composite sample showed no reactor related radioactivity. j j
- 3. DECONTAf11 NATION ACTIVITIES
- Previously, during a transfer of contaminated demineralized resin, resins were pushed through a vent into the waste gas collection piping. The resin raised the radiation levels around the piping and spilled in a previously decontaminated area. The floor area was cleaned. During this week a section of the waste gas header was !

l The Advisory Fanel for the Decontamination of Three Mile Island Unit 2 l
met on August 12, 1987 at the Holiday Inn in Harrisburg, Pennsylvania.
The Panel received status reports from both the NRC staff and GPUN on the cleanup. The NRC staff also gave a presentation on the Final Report of Supplement No. 2 to the Programmatic Environmental Impact Statement ;

Items of Interest Week Ending August 14, 1987 SAFEGUARDS NRC/ DOE Comparability Physical security license conditions were issued on August 6, 1987 to the four Category I fuel facility licensees to amend their licenses to incorporate the security plan changes for impl9mentation of the three near-term comparability measures for stationing armed guards at MAA portals, night firing qualification t for guards, and conducting 100% search of personnel and packages entering the protected area. The implement'ation.of the 100% search requirement will be phased in during the next 12 mcnths. This action was achieved 33 days ahead of schedule.
DOE /NRC Agreement Concerning Coordination on Threat Information A letter has been prepared documenting the agreement between NRC and DOE at a meeting on August 7, 1987, regarding the coordination of intelligence and threat information between our agencies. The agreement stipulates that such informa-tion, which could impact the licensed industry, be coordinated with the NRC prior to dissemination.

NFS, Amendment Incorporating Conditions on Purchase Nuclear Fuel Services, Inc., Erwin, Tennessee, in accordance with NFS Services, Ltd., applied July 22, 1987, for a license amendment to incorporate the five conditions of the purchase set forth in the NRC consent letter.
The amendment, No. 43 to license SNM-124, was approved on August 11, 1987.
The conditions provide for initiation of decontamination and decommissioning of inactive facilities and for controls on escrow funds and other assets to assure the ability to pay for future decommissioning activities.

Uranium Recovery Atlas Corporation In a telephone conversation on August 11, 1987, NMSS staff was informed by the ,
Uranium Recovery Field Office (URFO) in Denver that Atlas Corporation is I requesting a 10-day extension to the 20-day limit for requesting a hearing specified in the July 31 Order accompanying the denial of their license renewal for the Moab, Utah, uranium mill. NMSS indicated that it would support the Regions's decision in the matter. Atlas had also identified to URF0 two problems created by the conditions of the Order: (1) how to dispose of elution material from the Velvet Mine ion-exchange circuit used to comply with NPDES discharge requirements, and (2) how to handle the disposition of :
product (i.e., packaged yellowcake) created from past milling operations present )
at the mill site. Region IV and URF0 are exploring solutions to these '
W problems and agreed to keep NMSS fully informed. )

OFFICE OF NUCLEAR REGULATORY RESEARCH Items of Interest Week Ending August 14, 1987 Thermal Hydraulic Technical Support Center On August 5-6, 1987, RES staff met in Idaho falls with EG&G Idaho, Inc. and DOE-ID staff to discuss objectives and operation of the Technical Support Center (TSC).
Some of the major conclusions of the meeting were:
- 1. The TSC approach defined in NUREG-1244 " Plan for Integrating Technical Activities Within the USNRC and Its Contractors in the Area of Thermal Hydraulics" is sti.ll appropriate and will remain the basis of continued operation.
- 2. TSC programs should be issue oriented and will be classified as short term (priority) or long term issues as appropriate.
- 3. A flow chart for issue resolution which characterizes the roles of NRC, TSC and other research organizations was developed.
- 4. Responses to recent ACRS concerns were formulated and agreed upon.
- 5. The TSC has been doing a good job of performing those tasks defined in NUREG-1244.
In addition, the meeting provided NRC staff an opportunity to discuss TSC issues and problems with the new TSC principal investigator, Dr. Rommey Duffey.

, Trends and Patterns Analysis Branch NUREG-0090, Vol. 9, No. 4 (" Report to Congress on Abnormal Occurrences:
October-December,1986") was forwarded to Congress and released for general distribution. For this reporting period there were three abnormal occurrences at the nuclear power plants licensed to operate. The events were (1) the loss of low pressure service water systems at Oconee, (2) degraded safety systems due to incorrect torque switch settings on Rotork motor operators at Catawba and McGuire Nuclear Stations, and (3) a secondary system pipe break resulting in the death of four persons at Surry Unit 2. There were six abnormal occurrences at the other NRC licensees. One involved release of americium-241 inside a waste storage building at Wright-Patterson Air Force Base; three involved medical misadministration, one therapeutic and two diagnostic; one involved a suspension of license because unauthorized and unqualified people were used to service teletherapy and radiography units; and one involved an immediately effective order modifying a license and an order to show cause issued to an industrial radiography company because of false statements made to, and information withheld from, the NRC. Thcre were no abnormal occurrences reported by the Agreement States.
Incident Response Branch On August 10, 1987, the Director, AE0D, the Director of DOA, the Chief, Incident Response Branch (IRB) with the support of the cognizant IRB staff held a conference call with four Regional Administrators to discuss their observations on the Federal Field Exercise-2 (FFE-2) and suggestions for improving NRC's response to radiological emergencies. The suggestions made during this conference call will be incorporated into the lessons learned report as appropriate. A meeting of the FFE management group is planned for September 1 to review the compiled lessons learned.
On August 11, 1987, an IRB staff representative met with representatives from the Florida Power and Light Company to discuss the Emergency Response Data ,
System (ERDS). The licensee plans to include provisions for ERDS as part of I design modifications to their plant's Safety Parameter Display Systems. I On August 12, 1987, representatives from IRB and Telecommunication Branch met with an industry group to discuss the merits of using a national paging system j in conjunction with the NRC emergency response program. The system would allow NRC to page an individual anywhere in the continental U.S. from an NRC central location such as the Operations Center.
On August 13, 1987, the Chief, IRB met with representatives from the State Department to discuss attendance at the forthcoming International Atomic Energy Agency Advisor Group meeting intended to develop a Safety Series document dealing with the emergency response for a transboundary release of radioactive materials.

l On August 13, 1987, the IRB staff conducted formal training for NRC Emergency Officers. This constitutes the initial training for new Emergency Officers to familiarize them with the Operations Center and Emergency Response Procedures.
On August 19, 1987, the Emergency Preparedness Reviewer for the Operational Safety Analysis Review Team (05 ART) at Calvert Cliffs will visit the NRC Operations Center for a briefing on the NRC Incident Response Pr: gram. Also on August 19, 1987, two additional OSART team members will be briefed on NRC's approach to operational experience, assessment and feedback, and events analysis.
Diagnostic Evaluation and Incident Investigation Branch On August 12, 1987, the Dresden Diagnostic Evaluation Team held its second meeting in preparation for the Dresden diagnostic evaluation. The agenda included presentations by the Director, AE00, on the enforcement aspects of the evaluation and the Peach Bottom Senior Resident Inspector on the events and circumstances surrounding the Peach Bottom shutdown order. The Diagnostic Evaluation will begin at Dresden August 17, 1987.
Technical Training Center l On August 12-13, 1987, the Training Advisory Group (TAG) met at the Technical Training Center (TTC). The TAG members discussed a number of areas including: ;
the history, mission and future role of the TAG; the TTC curriculum and changes since the last meeting; the FY 1988 technical training schedule compared to the needs survey and resources; development or modification of the qualification and training programs; levels of involvement in the training process; potential methods for satisfying Agency technical training needs; the Fundamentals of Inspection Course (F01C); and various training center policies. All of the regions, AE00, NRR, NMSS, and RES were represented at the TAG meeting.

The BWOG also committed to be the focal point of keeping the Commission and staff informed on the status of each plant's response to the BWOG recommendations and to assure the Cormis-sion that all of the owners have implemented the applicable recommendations to the satisfaction of the BWOG and in conformance to BAW-1919.
Chairman Zech made the following requests:
- 1. Upon completion of review, the staff should verify that present requirements and regulations for B&W reactors are appropriate for the long term and lead to a level of safety at B&W plants that is comparable to other pressurized water reactors.
- 2. The staff should comment on the adequacy of B&W once-through steam generators, in regard to safety when compared to U-tube steam generators.
- 3. The staff should comment on the acceptability of the emergency feedwater initiation and control system for each B&W plant. Details, such as non-automatic response to steam and feedwater line breaks should be addressed.
- 4. The Owners Group should show aggressive pursuit of its recommendations (BAW-1919).
- 5. The Owners Group should not drop its emphasis on balance of plant systems.
The staff committed to issue its SER on BAW-1919 in October, 1987, with a supplement on the ICS/NNI at a later date.
