SUBJECT: NOTICE OF VIOLATION

(NRC INSPECTION REPORT NOS. 50-259/87-37, 50-260/87-37, AND

50-296/87-37)

This refers to the Nuclear Regulatory Commission (NRC) inspection conducted

by G. L. Paulk on October 1-30, 1987. The inspection included a review of  ;

activities authorized for your Browns Ferry facility. At the conclusion of i

the inspection, the findings were discussed with those members of your staff

!

identified in the enclosed inspection report.

Areas examined during the inspection included operational safety, maintenance

observation, surveillance testing observation, previous enforcement matters,

reportable occurrences, Engineering Assurance (EA) organization involvement on

Browns Ferry matters, inadvertent initiation of fire protection system, restart

review subcommittee, diesel generator overload evaluation, tests and experi-

ments program, design baseline verification program, inadequate core cooling

instrumentation, containment . coatings, environmental qualification testing,

facility modifications, document control program, Q-list, and companfon drawing

discrepancy /CAQR review.

Within these areas, the inspection consisted of selective examinations of

procedures and representative records, interviews with personnel, and

observation of activities in progress.

The inspection findings indicate certain activities appeared to violate NRC

requirements. The violations, references to pertinent requirements, and

elements to be included in your response are described in the enclosed Notice

of Violation.

We are concerned about a lack of management control and involvement in the

area of special testing. Plant Managers Instruction 17.1, Conduct of Testing

was not followed when an immediate temporary change (ITC) was made for

emergency diesel generator load testing. Further, PORC was not involved in i

the ITC approval. A violation was issued in Inspection Report 50-259, 50-260,

and 50-296/87-33 for failure to locate a PORC approved copy of Special Test

87-23 which is a quality assurance record. Additional detail is provided in

paragraph 10 of this report. In your response to that violation, please

address the above concerns including corrective actions.
