Item 5: " Generic Security Violations at Several Nuclear Power Plants" - No Region I plants experienced a major breakdown of physical security in the third quarter of 1986. Also, no violations involving the breakdown of access control have occurred during this period.
Appendix B Items (A0 Updates)
Item 11: A0 86-6, " Breakdown of Management Controls at an Irradiator Facility" (Radiation Technology, Inc.) - An update of this A0 was provided on October 2,1986 as part of the second quarter A0 report. The A0 should remain open. We expc :t further activity since the current license will be reviewed in February 1987.
Item 16: A0 86-12, " Willful Failure to Report a Diagnostic Medical Mis-administration" (Mercy Hospital and Valley Radiology Associates, Inc.) - .i This item should remain open, we are awaiting response to a show cause l order. Will likely be closed out next quarter, i l

Augmented Inspection Team report is now in draft. Once the report is approved and issued a summary will be provided to AE00 for inclusion into the A0 report. The summary should be completed by mid-November.
