NOTICE OF VIOLATION

(NRC INSPECTION REPORT.NOS. 50-327/87-30 AND 50-328/87-30)

This refers to the Nuclear Regulatory Commission (NRC) routine resident

inspection conducted by K. M. Jenison on May 6 - June 12, 1987. The inspection

included a review of activities authorized for your Sequoyah facility. At the

conclusion of the inspection, the findings were discussed with those members of

your staff identified in the enclosed inspection report. NRC concerns relative

to the inspection findings associated with control of operational activities

were discussed by J. A. Axelrad, Deputy Director, Office of Special Projects

(OSP) and G. G. Zech, Assistant Director, Inspection Programs, Division- of TVA

Projects, OSP - NRC, with C. C. Mason, Deputy Manager of Nuclear Power - TVA,

in an enforcement conference. held on June 18,'1987. A summary of - this con-

-

ference is contained in paragraph 16 of the enclosed inspection report.

Areas examined - during the inspection are identified in the report.

These

areas included special reviews of licensee groups performing safety review

functions, the Sequoyah Restart Testing Program, recent reactor coolant system

spill events,

and functional testing.

Within these areas, the inspection

consisted of selective examinations of procedures and representative records,

interviews with personnel, and observation of activities in progress.

The inspection findings indicate that certain activities appeared to violate

NRC requirements.

Violation 50-327,328/87-30-02 concerns the failure of. the

. Nuclear Safety Review Board to perform its review function as specified in

Technical Specifications.

Violation 50-327,328/87-30-07 concerns specific

examples of where procedures were not properly established and implemented.

Accordingly, these two violations, references to pertinent requirements,. and

' elements to.be included in your response are described in the enclosed Notice

of Violation.

An additional- violation (50-327,328/87-30-01) was also identified and is

discussed in paragraphs 9,15 and 16 of the enclosed inspection report. This

multiple example violation involves inadequate procedure implementation which

has given cause for concern over your staff's control of Sequoyah's operational

activities, particularly in the areas of system and equipment status and

testing.

As discussed in the enforcement conference of June 18, 1987, this

additional multiple example violation is under consideration for escalated

enforcement action. Accordingly, a Notice of Violation addressing this parti-

cular violation is not being issued at this time, and therefore no response to

this violation is required.

However, please be advised that the number and

characterization of violations described in paragraphs 9 and 15 of the enclosed

Inspecticn Report may change as a result of further NRC review. You will be

advised by separate correspondence of the results of our deliberations on this

matter.

During the course of this inspection, TVA (Site Licensing Group Manager) was

requested to re-evaluate those release calculations generated by TVA in

response to Violation 50-327,328/85-25-01.

The adequacy of these calcula-

tions, as addressed in paragraph 3 of the enclosed report, was discussed with

the Site Director, Plant Manager and others.

You are requested to perform

the re-evaluation utilizing the most conservative decay time that is appli-

cable to the inoperability of the EGTS on September 11, 1982.

Please submit

in writing the re-evaluation of the analysis of the potential radiological

consequences of the violation.

In addition, resubmit TVA's response to

Violation 50-327, 328/82-25-01 if the original calculations were nonconserva-

tive.

The re-evaluation should be completed within 30 days of the receipt of

this letter.

Your attention is called to the unresolved items identified in the inspection

report which insolve:

reactor coolant system sight glass design; changes to

Technical Specification bases; test personnel training; diesel generatcr

circuit cards; heatup and cooldown rate limits; component cooling water system

,

baffle testing; auxiliary feedwater pump flowrate; and recovery verification

of diesel generator air tank pressure.

In accordance with Section 2.790 of the NRC's " Rules of Practice," Part 2,

Title 10, Code of Federal Regulations, a copy of this letter, its enclosures,

and reply will be placed in the NRC Public Document Room.

The responses directed by this letter and its enclosure are not subject to the

clearance procedures of the Office of Management and Budget as required by the

Paperwork Reduction Act of 1980, PL 96-511.
