ENCLOSURE 1 NOTICE OF VIOLATION Florida Power Corporation Docket No. 50-302 Crystal River Unit 3 License No. DPR-72 During the Nuclear Regulatory Commission (NRC) inspection conducted on August 8

- September 11, 1987, violations of NRC requirements were identified.

The violations involved the failure of a member on the offsite review committee to be properly qualified, failure to perform a cooldown surveillance on the reacter coolant system, and failure to periodically recertify a qualified reviewer.

In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1986), the violations are listed below:

A.

TS 6.5.2.3.b requires members of the Nuclear General Review Committee (NGRC) to have a Bachelor of Science in engineering or a related field and five years related experience of which three of these years must be involved with the operation and/or design of a nuclear power plant.

Contrary to the above, during the period of 1977 through 1982, a member of the NGRC did not have experience involved with the operation and/or design of a nuclear power plant.

This is a Severity Level IV Violation (Supplement I).

B.

TS 4.4.9.1.1 requires that whenever a Reactor Coolant System (RCS) cooldown is being performed below 170 degrees Fahrenheit ( F), the rate of change in RCS temperature shall be determined to be within the cooldown rate limit of 10 F per hour at least once every 30 minutes.

Contrary to the above, during a RCS cooldown below 140 'F conducted on August 23, 1987, the cooldown rate was not determined to be within the acceptable limit at least once every 30 minutes.

This is a Severity Level V Violation (Supplement I).

TS 6.8.2.c requires procedure Qualified Reviewers to be trained, qualified, and certified as governed by Administrative Procedures.

l Administrative Instruction (AI) 401, Origination of and Revisions to P0QAM Procedures, specifies that Qualified Reviewers are trained, qualified, and certified for a period of two years.

Contrary to the above, during the period of July 1,1987 through August 26, 1987, an individual continued to perform the duties of a Qualified Reviewer even though this individual's training and certification had expired on June 30, 1987.

Pursuant to the provisions of 10 CFR 2.201, Florida Power Corporation is hereby required to submit to this Office within 30 days of the date of the letter transmitting this Notice a written statement or explanation in reply including:

(1) admission or denial of the alleged violation; (2) the reasons for the violation if admitted; (3) the corrective steps which have been taken and the results achieved; (4) corrective steps which will be taken to avoid further violations; and (5) the date when full compliance will be achieved.

Where good cause is shown, consideration will be given to extending the response time.
