APPENDIX A NOTICE OF VIOLATION Houston Lighting & Power Company Dockets: 50-498 South Texas Project, Units 1 & 2 50-499 Construction Pemits: CPPR-128 CPPR-129 During an NRC inspection conducted July 5 through September 4,1987, a violation of NRC requirements was identified. The violation involved a failure to follow procedures. In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1987), the violation is listed below:

Failure to Follow Procedure Criterion V of Appendix B to 10 CFR Part 50 requires that activities affecting quality be prescribed by and accomplished in accordance with appropriate instructions, procedures, or drawings. This requirement is amplified by the approved Quality Assu:.ance Program Description (QAPD) of South Texas Project (STP).

Standard Site Procedure (SSP) 38, Revision 1, paragraph 5.9, references Specification SV279VS1003, Revision 7 " Installation of Safety and Non-safety HVAC Equipment and Ductwork." Paragraph 3.12 of this specification states, " Bolts and nuts for galvanized ductwork shall be hot-dipped galvanized, cadmium-plated, or electromagnetic zinc-coated.

Hot-dipped galvanized nuts shall only be used with hot-dipped galvanized bolts. Cadmium-plated, and electromagnetic zinc-coated nuts may be used with either cadmium-plated or electromagnetic zinc-coated bolts."

Contrary to the above, hot-dipped galvanized nuts were used with electromagnetic zinc-coated bolts, and hot-dipped galvanized bolts were used with electromagnetic zinc-coated nuts.

Pursuant to the provisions of 10 CFR 2.201, Houston Lighting & Power Company is hereby required to submit a written statement or explanation to this Office within 30 days of the date of the letter transmitting this Notice. 2 (2) the corrective steps that have been taken and the results achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the date when full compliance will be achieved. Where good cause.is shown, consideration will be given to extending the response time.
