Dear Sirs,
s We are desirous of amending our U.S. Nuclear Regulatory Commission License 06-21263-02G. Under Item 10 for the IIS, inc. Model RSS-06 device we wish to list, as a secondary alternative, the BEBIG Model Am.1.G44, Am241,1000 milliCurie sealed source. It is our desire to use the Amersham AMC-19 sealed source already listed as our primary product offering, with the BEBIG sealed source as a backup to it. Both sources are the same size and have the same characteristics. We will modify and resubmit our sourcehead assembly drawings at such time that this amendment is approved.
Pursuant with 10 CFR 170.31 we have enclosed a check for this amendment in the amount of
$660.00.

j We are desirous of amending our U.S. Nuclear Regulatory Commission License #06-21253-
- 01. For Item 11 A and Item 14 please remove the name of Clark F. Hummel and add the names of John P. Lappie and Richard B. Tamiso. List of their qualifications is attached. Mr. Hummel has been reassigned with-in our organization. His replacement has received his in-house training and will be attending a forty (40) hour course with-in the next month. We will file an amendment for his addition to our license at that time.
We are moving our storage area and program office to a different location with-in our existing facility. We have enclosed a drawing showing our new location. The previous area is being reallocated for office use. It was listed as Attachment 13B. Site Surveys to verify lack of exposure to our employees will be performed as soon as the material is transferred.
Under Item 6 of our license, we are requesting to add the following sealed sources. They are Kr85, Model Amersham KAC.D1,1000 millicuries per source not to exceed 5000 millicuries i
total, Kr85, Model DuPont NER-585,1000 millicuries per source not to exceed 5000 millicuries total, Sr90, Model Amersham SIF.D1,100 millicuries per source not to exceed 500 millicuries total, and Am241, Model BEBIG Aml.G44,1000 millicuries per source not to exceed 20,000 millicuries total.
Under Item 9 of our license we are requesting that the following sources be listed as Research and Development as currently specified in Item 9A. They are Kr85, Model Amersham KAC.D1,1000 millicuries per source not to exceed 5000 millicuries total, Kr85, Model DuPont NER-585,1000 millicuries per source not to exceed 5000 millicuries total, and Sr90, Model Amersham SIF.Dl,100 millicuries per source not to exceed 500 millicuries total. We arc further requesting that Item 9 be amended to read that the Am241, Model BEBIG Aml.G44,1000 millicuries per source not to exceed 50,000 millicuries total be interchangeable with the Am241, Model Amersham AMC-19 currently listed as Item 9B. We will be using the existing Amersham source as our primary product, however, due to occasional delivery difficulties we are listing a s secondary source. Both source capsules are q

U.S.N.R.C. License Amendment John P. Lappie, Sr. Electrical Project Engineer.
Mr. Lappie has been in our employ three times. First from 1980 to 1982 as a Field Service Engineer. During that time he received extensive hands on training with our gauge products. The
. second was from 1985 to 1986 as an Electrical Project Engineer. During that time he had extensive experience designing the electronics interface for our gauging systems. From 1986 to 1990 he was employed at Chase Brass in Shelby, Nodh Carolina. Among his duties there, he was their Radiation Safety Officer. He received formal training from LFE Corporation and received a Specific License from the State of North Carolina. In 1990, Mr. Lappie returned to PS in his
. current capacity. In addition to the above experience he recently attended the Harvard School of Public Health, completing their fony (40) hour course. Copy of certificate enclosed.
Richard B. Tamiso, Electrical Project Engineer Mr. Tamiso holds an ASEE degree from New Hampshire Technical College, and a BSEE from the University of Hartford, Ward College of Technology. From 1986 to 1989 he was employed at UNC Naval Products. They were a major manufacturcr of U.S. Navy nuclear propulsion components. In 1996 Mr. Tamiso joined PS. Since that time he has completed our in-house training and has had hands on field experience with our gauges. Recently he completed the forty (40) hour course at the Harvard School of Public Health with Mr. Lappie. Copy of certificate enclosed.
