1.1 Provide guidelines for entry into a reduced lleactor Coolant System (ItCS) l inventory condition, operation at hild l.oop, and exit from a reduced itCS inventory condition.

1.2 This procedure SilAl.l. be used any time reactor vessel water level is being lowered to less than 36 ft 3 inch (3 ft below reactor vessel flange) with fuel in the core and Illill required for decay heat removal.

1.3 This procedure is entered from OPOP03 ZG-0007 Plant Cooldown or OPOP03 ZO.

Tins review SilAI.L be conducted prior to beginning RCS drain to a reduced RCS inventory condition. This review SilALL also be discussed with oncoming shift personnel who may be affected by the reduced inventory condition. <

(Reference 2.4) 3.2 The RCS is borated to cold shutdown xenon free OR refueling condition.

3.3 The fuel transfer tube blind flange is '.astalled OR fuel transfer tube gate valve is closed and tagged.

3.3.1 !E at least 12 inches of water is above the top of the transfer tube,

]JtEN the gate valve may be opened and blind flange removed.

3.4 The following instrumentation SilALL be operable prior to entering a reduced RCS inventor; condition:

3.4.1 RHR heat exchanger inlet and outlet temperature with indication on QDpS OR strip chart recorder ihr all operable trains.

3.4.2 RilR pump flow with indication on QDPS lbr all operable trains.

' A.3 RilR pump motor current indication (amps) for all operable trains.

3.4.4 "RilR PUMP CURRENT LO" Annunciators on Lampbox 1M02 for the operable RilR pumps is NOT removed from service.

3.4.5 RCS kvel sightglass has been walked down in last twelve hours and satistles requirements of Step 4.17, 3.4.6 Both trains of RVWL are operable with at least two QDPS displays available, 3.4.7 Narrow range hot leg level mdication is in service.

3.5 The following containment requirements have been met:

.1 The equipment hatch door is closed and held in place such that no gaps exist in the sealing surface, .

3.5.2 A minimum of one door in each air lock is capable of being closed.

(Refer to OPSP03 XC-0001, Refueling Containment Penetration Status.) a 3.5.4 All Steam Generator (SG) secondary side manways and inspection ports are in place and bolted. WilFN all of the following valves are in the specified conditions for each affected SG, TilFN the SG secondary side may be open.

3.6 Tools seguired to locally sent an RilR pump are available in the RCI). RilR pump vent rigs are installed. LE required by llealth Physics, TIIEN a Radiation Work Permit is in effect to allow RilR pump venting.

3.7 The following components are removed from service by Equipment Clearance Ornis:

. Positive Displacement Charging Pump

. One Centrifugal Charging Pump

. All liigh llead Safety injection Pumps

. All Reactor Coolant Pumps are incapable of rotating.

(RCP motor runs allowed with pump uncoupled)

. Si Accumulator Isolation Valves are SilUT, Ilreakers are OPEN.

(N/A if Si accumulators are d . ressurized)

. All Pressurizer lleaters

. Iloron Thermal Regeneration System

. Flush (purge) water to RT8039, Failed Fuel Monitor 3.8 Adequate capacity is available in radwaste to receive volume drained frorr. RCS.

(Refer to Addendum 3, Determination Of RCS Volume To Be Drained / Filled.)

3.9 The fo!!owing makeup and letdown flowpaths are available:

3.9.1 From RWST through a LilSI pump and cold leg injection line into RCS.

3.9.2 CCP with suction from VCT OR RWST and discharge through normal charging OR seal injection flowpaths.

3.9.3 Low pressure letdown through an operating RilR pump.

. One CCP and one LilSI pump are operable with hot (CCP excluded) and cold leg injection flowpaths into RCS.

. Two 1.11S1 pumps are operable with hot and cold leg injection flowpaths into RCS.

3.11 The 1.11S1 pump and CCP SilAl.l. be powered from separate, independent power supp'ies. (eg, different trains) 3.12 Two RilR pumps are operable during reduced RCS inventory per Technical Specification 3.4.1.4.1 OR 3.4.1.4.2, as apnlicable.

l 3.13 Electrical Power Systems Technical SpeciHeations 3.8.1.2, 3.8.2.2 and 3.8.3.2 1.imiting Conditions for Operation are met.

3.14 Instrument air compressor 12(22) is operable and aligned to provide air to AOVs in the RCB.

3.15 CL-ACW Emergency Cooling Pump is operable OR in operation.

3.16 Four operable RCFCs with cooling available from respective CCW train.

3.17 One containment carbon enit is operable.

3.18 Install the Reactor Vessel llead Vent Rig per "0 POP 02-RC-0003, Addendum 1, Filling and Venting the RCS".

3.19 ENSURE 1&C Maintenance has performed a loop check of the core exit thermocouple recorder and annunciator in the last 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br />. The core exit recorder and annunciator are in service.

3.20 A dedicated, rehable communication line, headphones being the preferred method, is established between Control Room personnel and RCB personnel.

, Mid Loop Operation 3.21 Prior to assuming shift, the on-coming Control Room Operations staff has received training in Mid 1.oop operation and loss of RilR condition. Training SilALL be done priar to standing watch during a reduced RCS inventory condition.

(Reference 2.4) 3.22 Scope of this procedure has been reviewed and understood by all on-shift Control Room operations staff.

3.23 ALL switchyard activities SilALL cease during Mid Loop Operations, with exception of a condition which if not responded to could lead to a loss of an offsite power source. E such a condition occurs, TilEN_ Operations Management (Shift Supervisor or above) SilALL determine whether the switchyard activity may be perfbrmed. (Reference 2.1.5) 3.24 Non-outage unit should cease all activities (including surveillances) that could result in a reactor trip OR a LOOP. Confirm with non-outage Unit / Shift Supervisor. (Reference 2.1.2) 3.25 Load Dispatcher has been notified of intent to enter into Mid-Loop operations.

(Reference 2.1.2) 3.26 Main and Standby Transformer barriers are in place. (Reference 2.1.5 and 2.10) 3.27 ENSURE the incore flux thimble tubes are inserted and the high pressure seals are installed per 1(2)PMP07-II-1002, Withdrawal / Insertion of incore l' lux Thimbles.

3.28 WilEN nuclear fuel is in the vessel, TilEN all control rods should be fully inserted into the core and the reactor trip breakers open. E NOT practical, T11EN an Engineering Analysis SilALL be perfbrmed to evaluate the reactivity management aspects of performing an unrodded Mid-Loop operation.

3.29 E ECW pond temperature is 93 F or GREATER, TilEN an Engineering Analysis SilALI be performed to calcult.te decay heat removal ability.

3.30 E a front end Mid-Loop is to be performed, TilEN all three trains of Residual lleat Rernoval should be functional.

OPOP03-ZG-0009 Rev.16 >>are io or 63 Mid Loop Operation 4.0 Notes and Precautions 4.1 Steps within this procedure should be perfonned in order. Shift Supervisor discretion may be used to deviate from the sequence listed.

4.'l Steps within this procedure may be omitted, as determined to be unnecessary for current plant conditions by the Shift Supervisor. Designate omitted steps by placing "N/A" in the signoff OR initial blanks. Provide a written explanation adjacent to the step to detail conditions preventing performance.

4.3 The Unit / Shift Supervisor SilALL signoff or initial all steps unless otherwise designated within this procedure.

4.4 Prior to performing plant evolutions during Mid-Loop Operation, evolutions SilALL be evaluated for impact on inventory control and ability to maintain shutdown cooling capability. (Reference 2.2.5) 4.5 Plant evolutions that could potentially affect RCS inventory OR shutdown cooling during Mid-Loop operations performed outside of OPOP03.ZG-0009, SilALL NOT be conducted unless authorized by the Plant Manager. (Reference 2.2.5) 4.6 Condition Reports having the potential to cause RCS perturbations SilALL be reviewed by the Mid Loop Coordinator. The Mid Loop Coordinator SilALL recommend approval OR disapproval to the Shift Supervisor. The Shift Supervisor SilALL be final approval authority. (Reference 2.1.2) 4.7 Form 1, Mid Loop Operations I ogsheet may be used to keep temporary logs as determined by the Shift Supervisor.

4.8 Form 2, Mid Loop Checklist SilALL be performed once every 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br /> by the Reactor Operator during reduced RCS inventory operations.

4.9 Form 3. Reduced RCS Inventory Operations Shift Turnover Sheet SilALL be completed and reviewed at the end of each shift by:

4.9.1 Off-going Mid Loop Coordinator 4.9.2 Off-going RCB Coordinator 4.9.3 On-coming Mid-Loop Coordinator 4.9.4 On-coming RCB Coordinator 4.10 WilEN narrow range hot leg indication is in service and RCS level is below top of the hot leg, TilEN narrow range hot leg sightglass OR control room indications SilALL be used for RCS level.

4.12 RCS level changes SilALL be made slowly in a controlled manner to minimize effects on reactor vessel level indications.

4.13 WilEN RCS level is below reactor vessel 11ange, TilEN low pressure letdown flowpath SilAl.L be the only method used to reduce reactor vessel water level.

4.14 DO NOT start a RilR pump with vessel level below 32 ft 9 inch.

(6 inches ab(we hot leg centerline) 4.15 MINIMlZE initiation of transient conditions while in reduced RCS inventory.

(e.g. RIIR pump starting and stopping, changing flow rates, changing heat exchanger temperatures, OR RCS draining activities) 4.16 CVCS demineralizer flush operations SilALL NOT be performed during reduced RCS inventory condition.

4.17 WilEN RCS level sightglass is in service, TilEN ENSURE the following valves positions at least once per shift: (documented on a temporary log) l 4.17.1 "l(2)-RC-0166 PZR PORV LINE VENT VAL.VE" OPEN.

4.17.2 "l(2)-RC-0200 LOOP 1 RCS LVL SIGilTGLASS CONN" OPEN.

4.17.3 "l(2) RC-0057A LOOP 1 DRAIN TO RCDT" OPEN.

4.17.4 "l(2)-RC-0517, RCS LEVEL SIGIITGLASS UPPER ISOLATION" on RCS sightglass upper connection assembly OPEN. {RCB top of PZR}

4.17.5 "l(2)-RC-0058A LOOP 1 DRAIN TO RCDT" CLOSED.

{RCB RCS Loop 1) 4.18 WHEN a difference of six inches O_R more exists between RCS level sightglass and narrow range hot leg level indications, TilEN narrow range hot leg level indication SilALL be used as the correct indication.

0 POP 03-ZG-0009 Rev.16 Page 12 or 63 Mid Loop Operation 4.19 E a difference of greater than six inches in indication exists between RCS level sightglass AND RVWL O_R RCS level sightglass AND pressurizer cold cal, TilEN:

4.19.1 PERFORM a walkdown of the sightglass in accordance with Step 4.17.

4.19.2 CORRECT any discrepancies noted in the sightglass walkdown.

4.19.3 OBSI!RVE caution when determining which level indication is to be used. Inadequate head venting due to a rapid draining rate can cause RVWL to remain higher than actual loop level. (Reference 2.6) 4.19.4 USE RVWL OR pressurizer cold cal as the correct level.

4.19.5 PROCEED with RCS draindown with Shift Supervisor's permission.

4.20 RCS Ilot leg level of +11 to +6 inches corresponds to elevation of 33' 7" to 32' 9". Leg level gauge range is +14.5 to -3.5 inches of cemerline.

4.21 Plant Operations personnel SilALL use all pertinent instrumentation available to monitor approach to OR operations in a reduced RCS inventory condition OR mid-loop operation.

4.22 The RCS level sightglass SilALL be continuously monitored during all draining and refill operations.

4.23 One of the following pressurizer vent paths SilALL be established prior to draining the pressurizer: (Reference 2.2.3)

. The Pressurizer spray line vent valves RC-0502 and RC-0503 OPEN to atmosphere. (Preferred method)

. A minimum of one Pressurizer Code Safety Valve REMOVED.

4.24 Fluctuations in RilR pump How, amps OR discharge pressure SilALL be anticipated at all times and at any RCS level during approach to, OR operations in, a reduced RCS inventory condition OR mid-loop operations.

4.25 E RilR cooling is lost while the RCS is drained to mid loop, TilEN the following is applicable:

4.25 ' Time to core buling is dependent on RCS temperature and time after reactor shutdown, REFER TO Addendum 4.

4.25.2 Core boiling can occur in as little as six minutes.

4.25.3 Core uncovery can occur in 45 minutes.

Mid-Loop Operation 4.26 E a change in boric acid concentration M chemical addition to RCS is performed, TilEN two RilR trains SIIALL be in operation with a total flow greater than 3000 gpm.

4.27 E Nitrogen purge of the Steam Generator U-Tubes is NOT performed, TIIEN the SG U tubes will not drain at the same rate as the rest of the RCS. The shape of the U-tubes form a trap which inhibits draining. When RCS level is approximately 32 feet below the top of the SG tubes gravity will start causing the SG tubes to drain slowly. When RCS level is lowered to below the top of the hot leg air is admitted to the SGs and the SG tubes will drain as air is allowed to replace the water. This will be observed as a level oscillation. As RCS level is lowered, SG tubes drain to refill hot leg Draining SilALL continue until level cycling for SG tube draining is completed. ENSURE this level oscillation no longer occurs to verify SG tube draining is completed. Consequently, up to eight hours may be required to drain the U-tubes in a controlled manner.

4.28 E RCS boration was performed without a RCP in operation, THEN draining of Steam Generator U-Tubes may cause a RCS boron dilution. E boron dilution during draining S/G U-tubes is suspected TilEN monitoring of RCS boron concentr, tion will be necessary to compensate for the dilution of boron.

(Reference 2.2.4) l 4.29 Strict control and attentiveness are required anytime the RCS is in OR approaching l a reduced RCS inventory condition OR hiid Loop operation, i

4.30 N1AINTAIN reactor vessel metal temperature greater than 70 F at all times prior to detensioning reactor vessel head.

4.31 WilEN RCS water level is at M above reactor vessel flange, TilEN h1AINTAIN RilR heat exchanger discharge temperature greater than 80 F.

4.32 WilEN the plant is in hiode 5, TilEN h1AINTAIN RCS core exit temperature less than 140 F.

4.33 E the core exit thermocouple alarm becomes inoperable during reduced RCS inventory operations, TIIEN RECORD the 5th highest core exit thermocouple indication on Forra 1, Niid-Loop Operations Logsheet at ten minute intervals from TR-0001 on CP-018 OR QDpS. (Reference 3.19) 4.34 E the narrow range hot leg level alarm is inoperable with RCS level below 66%

plenum level (33' 5.5", RVWL Point 4 uncovered), TilEN closely N10NITOR RCS level on the narrow range hot leg level recorder.

4.35 E the RCS draindown begins near the vessel flange level (i.e., after head installation), TilEN PERFORh1 all the steps in Section 5.0 except Steps 5.13, 5.15, 5.16, 5.19, 5.21, 5.22, 5.24 which S!!ALL be marked N/A.

OPOP03-ZG-0009 Rev.16 Pase 14 or 63 Mid Loop Operation 4.36 DO NOT drain to an elevation ofless than 36 ft 3 inch (3 ft below reactor vessel flange), without the Plant Operations Department Manager's approval.

(Reference 2.3) 4.37 The two narrow range hot leg (Loops 1 and 3) water level instruments including associated remote indications, sightglasses and alarms SilALL be available for l draindown to Mid-Loop level. (Reference 2.4) 4.38 WilliN nozzle dams are installed during RCS refill from mid-loop conditiona, TilEN a refill vent path is required.

4.38.1 This vent path is required to prevent overpressurizing the nozzle dams.

4.38.2 The refill vent path SilAl.l. be specified and verilled open befbre the last nozzle dam is installed.

7.10 E return to Niid Loop operation prior to the end of the outage is NOT planned, TIIEN NOTIFY IRC hiaintenance to perform the following:

7,10.1 REh10VE core exit thermocouple alarm from service AND ADJUST range per Shift Supervisor. (approximately scale range 100 to 2200 F) 7.10.2 PLACE RCS Loop level gauges in wet layup with demineralized water.

7.11 E further operations in hiid-Loop conditions are NOT anticipated, TilEN RENIOVE all administrative tagging requirements established for hiid Loop operation as required by Precaution 3.7.

7.12 STOP any special logging requirements.

7.13 CLOSE PZR Spray Valves RC-PCV-655B AND RC-PCV-655C.

7.14 E hiode 6 operations are planned, TilEN GO TO OPOP03-ZG-0010, Refueling Operations.

7.15 E nozzle dams are removed AND RCS fill / vent operations are planned, TilEN GO TO OPOP03-ZG-0001, Plant lleat-Up.
