SUBJECT: NRC INSPECTION REPORT 50-456/97013(DRP); 50-457/97013(DRP) AND

NOTICE OF VIOLATION -

Dear Mr. Tulon:-

< On August 11,1997, the NRC completed an inspection at your Braidwood Units 1 and 2

reactor facilities. The enclosed report presents the results of that inspection.

- During the 6 week inspection period, your conduct of activities was generally characterized-

by safety-conscious operations.~ Specifically, two maintenance activities performed on a- ~

feedwater isolation valve and on a main steam isolation valve were characterized by

excellent planning, execution, and team work.

We found that your root cause investigation performed in response to an over' dilution of the

Unit 1 reactor coolant system was accurate, thorough, and self critical. Additionally, the ,

rapid recognition and immediate corrective actions by operators limited the magnitude of the

transient and minimized the safety consequences of the event. However, we are concerned

about the lack of strict procedural adherence that your_ staff identified as a root cause for

the over dilution. Since the event was non-repetitive, self-identified,'and corrected, the

procedural adherence violation is being treated as a non-cited violation.

We are also concerned that the inspectors found a welding machine unattended and

energized. This violation of the Technical Specification requirements to follow procedures -

.

that implement your fire protection program could have resulted in a serious fire hazard.' A

similar event at the LaSalle Station in March.1996 resulted in a fire. We do understand that

corrective action was taken with welders and fire watches to reinforce the seriousness of <

- leaving the wolding machine energized and unattended. The violation is cited in the

. enclosed Notice of Violation (Notice), and the circumstances surrounding the violation are

described in detail in the enclosed report..
