October 16, 1990.- The inspection. included a review of activities authorized

for your Arkansas-Nuclear One, Units 1 and 2 facility. At the conclusion of-
'the inspection, the findings.were discussed with those members of your staff
identified in the enclosed report.
Areas' examined during thel inspection are identified in the report.

findings. 'Within'these areas, the-inspection consisted of selective
examination of. procedures and representative records, interviews with
personnel, and observations ~of activities in progress.

Based-on thecresults of this inspection, certain of your activities appeared to-

be41n, violation of NRC requirements, as specificd_in.the enclosed Notice of
LViolationu(Notice). We are concerned about the violation because an' inadequate
technical' review of a temporary. modification. allowed six pressurizer heater
,
elements;to-be energized after the pressurizer was drained. . Failure to perform
.

an'. adequate review of a, safety analysis is.a violation of Technical
!
.
1 Specification requirements.
Response by the operations staff to the energizing;
- of;the heaters was rapid and effective, limiting the. total event time to-
,
- approximately 6 minutes. However,fthis. citation is being issued-because the-
'
Linadequate_ review of the safety analysis was not discovered prior-to the-
self-disclosing consequences of the event.
