APPENDIX A NOTICE OF VIOLATI_0N South Texas Project, Unit 1 Docket: 50-498 Permit: CPPR-128 During an NRC inspection conducted on April 13 through May 1,1987, a violation of NRC requirements was identified. The violation involved unqualified motor lead splices in Limitorque valve operators. In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1987), the violation is listed below:

Criterion XV of 10 CFR 50, Appendix B, states, in part, " Measures shall be .

established to control materials, parts, or components which do not conform to requirements in order to prevent their inadvertent use or installa-tion . . . .," and Section 15 of the HL&P QA program requires that HL&P and prime contractors have systems to identify, segregate, and disposition nonconforming components.

Contrary to the above, measures had not been established to prevent the i installation of certain Limitorque valve operators which had not been i qualified since the installed configuration contained blind barrel crimp splices which had not been documented on the qualification test report as having been tested and qualified.

This is a Severity Level IV violation. (SupplementII)

Pursuant to the provisions of 10 CFR 2.201, Houston Lighting & Power Company, is hereby required to submit to this office within 30 days of the date of the letter transmitting this Notice, a written statement or explanation in reply, including for each violation: (1) the reason for the violation if admitted, (2) the corrective steps which have been taken and the results achieved, (3) the corrective steps which will be taken to avoid further violations, and (4) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
