Inspection Report No. 50-271/86-17
This refers to the special announced inspection conducted by Mr. A. A. Varela
of this office on September 9-12, 1986 at Yankee Atomic Framingham,
Massachusetts Office and the Vermont Yankee Plant of activities authorized
by NRC License No. DPR-28 and to the discussions of our findings held by Mr.
Varela with Mr. P. Donnelly of your staff at the conclusion of the inspection.
In addition, this refe'rs to the supplemental information provided in your
September 19, 1986 letter regarding your IEB 80-11 reverification activities.
Areas examined during this inspection are described in the NRC Region I
Inspection Report which is enclosed with this letter. Within these areas, the
inspection consisted of selective examinations of procedures and representative
records, interviews with personnel, and observations by the inspector.
Based on the results of this inspection, it appears that two of your activities
were not conducted in full compliance with NRC requirements, as set forth in
the Notice of Violation, enclosed herewith as Appendix A.
These violations
have been categorized by severity level in accordance with the " General State-
ment of Policy and Procedure for NRC Enforcement Action," 10 CFR Part 2,
Appendix C (Enforcement Policy 1986). You are required to respond to this
letter and in preparing your response, you should follow the instructions in
Appendix A.
Another activity appears to be a deviation in that the evaluations performed
regarding mortar strength were based on assumptions and specifications and not
on construction records or tests.
This deviation is described in Appendix B.
You are requested to also respond to the deviation following the instructions
provided in Appendix B.
Based on the results of this inspection, we were not able to verify that you
had fulfilled the commitments made in your responses to IE Bulletin 80-11,
Masonry Wall Design.
The lack of documentation regarding the procedures and
criteria used in the 1980 surveys performed to identify those walls which would
be subjected to detailed analysis and regarding your verification of mortar
strength led us to question the adequacy of your actions. However as a result
of our review of your September 19, 1986 submittal, we understand that, with
the exception of four walls in the Turbine Building, you have concluded that
the reverification program substantiated the adequacy of your original survey
and analyses activities. We will confirm this during future inspections,
