APPENDIX B NOTICE OF DEVIATION Vermont Yankee Nuclear Power Corporation Docket No.
50-271 Vermont Yankee Nuclear Power Station License No. DPR-28 As a result of the inspection conducted on September 9-12, 1986, and in ac-cordance with the NRC Enforcement Policy (10 CFR 2, Appendix C) the following deviation was identified:
IE Bulletin 80-11, Masonry Wall Design, dated May 8, 1986, informed licensees that existing test data or conservative assumptions could be used to justify the reevaluation criteria to be applied to existing safety-related walls.
In letters dated July 7 and November 10, 1980, the licensee responded to Bulletin 80-11 and indicated its commitment to complete the actions delineated in the Bulletin.
Contrary to the above, the licensee solely relied upon mortar properties found in the construction specification to determine the strength value for type S mortar. No documentation was provided which verified that type S mortar was actually used to construct the walls, no test
- program was undertaken to quantify the mortar strength, nor were conservative assumptions used for the reevaluation criteria.
Vermont Yankce Nuclear Power Corporation is hereby requested to submit to this office within twenty days of the date of the letter which transmitted this Notice, a written statement or explanation in reply, including:
(1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further deviations; and (3) the date when full conformance will be achieved. Where good cause is shown, consideration will be given to extending this response time.
