APPENDIX A NOTICE OF VIOLATION Vermont Yankee Nuclear Power Corporation Docket No. 50-271 Vermont Yankee Nuclear Power Station License No. DPR-28 As a result of the inspection conducted on September 9-12, 1986, and in accor-dance with the NRC Enforcement Policy (10 CFR 2, Appendix C), the following violations were identified:

1. 10 CFR 50 Appendix B, Criterion V requires that activities affecting quality shall be prescribed by documented instructions, procedures, or drawings and that they shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished. The licensee's Quality Assurance program incorporated these 10 CFR 50, Appendix B requirements during the performance period of the original masonry wall s.urveys conducted in 1980.

Contrary to the above, as of September 9,1986, a written procedure providing the scope, extent, and quantitative and qualitative acceptance acceptance criteria to be invoked was not documented for the 1980 walkdown/ survey of block walls to determine their location in relation to safety-related equipment. As an apparent result, four walls were missed and the engineering organization dispositioned the walls without formal acceptance criteria to judge their adequacy.

This is a Severity Level IV violation (Supplement 1)

2. Criterion XVIII, Appendix B, 10 CFR 50, requires that sufficient records shall be maintained to furnish evidence of activities affecting quality.

The licensee's QA program, incorporated these Appendix B requirements during the period of performance of the original masonry wall surveys. It states that quality assurance records shall include results of reviews, inspections, tests, audits and material analyses. It also requires that records which provide documentary evidence of quality shall be maintained and shall be retrievable as plant history.

Contrary to the above, as of September 9, 1986, completed documentation of the results of the 1980 IE Bulletin 80-11 wall surveys could not be found.

This is Severity Level V violation. (Supplement I)

Pursuant to the provisions of 10 CFR 2.201, Vermont Yankee Nuclear Power Corporation is hereby required to submit to this office within twenty days of the date of the letter which transmitted this Notice, a written statement or explanation in reply, including: (1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved.

Where good cause is shown, consideration will be given to extending this re-sponse time.
