1' HIGHLIGHTS OF WEEKLY INFORMATION REPORT WEEK ENDING OCTOBER 10, 1986 Oyster Creek Nuclear Generatino Station On October 1,1986, GPU Nuclear (the licensee) issued a press release stating 2

that Oyster Creek has become only the fourth unit in the United States to have ten specific training programs accredited by the National Nuclear Accrediting Board. The Accrediting Board is an independent group comprised of scholars and executives from utilities, post-secondary schools, non-nuclear training-institutes and individuals nominated by the Nuclear Regulatory Commission. Its activities support the National Academy for Nuclear Training.

The ten training programs accredited are: senior reactor operator, reactor operator, non-licensed operator, shift technical advisor, instrument and control technicians, electrical main-tenance, mechanical maintenance, chemistry technician, radiological protection technicians and technical training of technical staff and managers. With accreditation, Oyster Creek also becomes

a-training branch of the National Academy for Nuclear Training which is administered by the Institute of Nuclear Power Operations.

V. C. Sumer The plant shut down this morning, October 7, 1986, in accordance with technical specifications due to unknown reactor coolant system leakage just exceeding 1.0 gpm. An unusual event has been declared. The licensee is looking for the leakage source and the NRC staff is following their progress.

1 Erwin Tennessee Water Treatment Plant The Tennessee Department of Health and Environment informed Region II of elevated readings of total uranium' activity in a sewage sludge sample from the j

Erwin Tennessee Water Treatment Plant.

State of Tennessee representatives reported values of 700 to 800 pCi/gm in sludge collected during August 1986.

(This corresponds to about 0.12 percent of uranium which is licensable.) Gross i

alpha values from first quarter samples were reported as 300 pCi/gm.

The State indicated that the range in sludge from other areas of the State was 10 to 30 pCi/gm.

Region II and NMSS are reviewing the findings.

The sludge will 4

be reanalyzed to confirm the findings, this includes analysis of uranium isotopes and its daughter products to determine if enriched uranium is found in the sludge.

Pending the findings, an appropriate agency course of action i

will be taken.

NFS has been notified of these findings.

The public hearing on the Volume Reduction Services Facility proposed by B&W for commercial incineration and compaction of low-level waste at its Parks l

Township site was held on September 30 through October 2 in Apollo, PA, by

OFFICE OF NUCLEAR REACTOR REGULATION ITEMS OF INTEREST Week Ending October 10, 1986 Diablo Canyon Nuclear Power Plant On September 30, 1986 PG&E commenced unloading spent fuel from Unit 1 into the original racks, reinstalled into the Unit 1 pool.

In the morning of October 1 a fuel assembly element in the reactor vessel tipped by approximately 2 feet, at the top, into the open space of 3 assemblies which already had been unloaded. The licensee stopped further refueling operations. The licensee informed us that no increase in airborne radio-activity had been measured and no fuel damage was apparent.

On October 2,1986, the tipped assembly was successfully transferred to the spent fuel pool. The operation was perfonned using special procedures and under strict administrative controls.

Its activities support the National Academy for Nuclear Training.

Catawba Unit 1 On October 7, 1986, with Unit 1 in Mode 6 (refueling), about 10,000-12,000 gallons of borated water from the RWST spilled through the detensioned reactor vessel head and the disconnected UHI piping. The cause is a procedural deficiency related to the alignment for a test of the "B" RHR pump. The spill has affected the area around the reactor vessel head (including the CRDMs) and portions of the lower containment. Lower containment contamination OCT0PER 10, 1986 EtlCLOSURE B

s level is about 400 mrad smearable. No airborne activity is detectable. No contamination of individuals has occurred.

This event will cause a delay of about 10-14 days to Duke Power Company's schedule for restart from the first refueling outage. NRR and Region II are following up.

Seabrook Station Unit 1 On August 22, 1986, Seabrook Station Public Service Company of New Hampshire, filed a motion pursuant to 10 CFR 50.57(c), requesting the Atomic Safety and Licensing Board (ASLB) to authorize the Director of Nuclear Reactor Regulation (NRR).to issue a license to load fuel and conduct certain precriticality testing of the Seabrook Station Unit 1.

On October 7,1986, the ASLB granted Public Service Company's request and authorized the Director of NRR to issue a license for the requested operation, subject to his findings required under 10 CFR 50.57(a).

Public Service Company estimates being ready to load fuel on or about October 16, 1986.

We are in the final stages of preparing our Safety Evaluation Report and the license (with appropriate conditions reflecting the ASLB Order) on a schedule to support the applicant's date.

^D Region I can support the October 16 fuel load date and will have completed the review of all preoperational tests required to support the license to' be issued. The. Region will ha m completed the review of all the preoperational tests required to support low power testing prior to issuance of a low power license.

Communications on Nuclear Criticality Safety Issues On Friday, October 3, 1986, Region II notified NMSS that they were informed by the Babcock & Wilcox Lynchburg Research Center (LRC) that the latter had discovered errors in a Table in the Nuclear Safety Guide, TID-7016, Revision 2, dealing with the criticality of uraniurh-carbon-water mixtures.

Region II also notified personnel at other NRC Regional Offices of the errors.

NMSS knew of no licensee that based the nuclear criticality safety of its operations on the subject table, but discussed the problems of the Table with the LRC, Regions IV and V, GA Technologies, Inc., and personnel of the Fort St. Vrain Power Station, and confirmed that no licensed facility used the table in their nuclear criticality analyses.

The safety margins at all facilities met the licensed conditions.

OCTOBER 10, 1986 ENCLOSURE C

2 Illinois Fee on Transportation of Spent Fuel On September 3, 1986, DOE, a utility and utility group appealed to DOT's Research and Special Programs Administration (RSPA) a June 4, 1986 decision of the Office of Hazardous Materials Transportation (OHMT).

The OHMT decision did not consider a State of Illinois fee of $1,000.00/ shipment on spent fuel transportation inconsistent with the Hazardous Materials Transportation Act (HMTA) or Hazardous Materials Regulations.

Several arguments were raised in the appeal, including assertions that OHMT failed to examine and attribute appropriate importance to the consequences of such a fee on shipments of spent fuel.

On September 29, 1986, a public notice was issued (51 FR 34527) inviting the public to comment on whether the challenged transit fee is inconsistent with HMTA or the regulations issued.

Comments will be considered before an administrative ruling is issued by the RSPA Administrator.

FBI Criminal History Rule The proposed rule has been revised in response to CRGR suggestions and forwarded to the EDO for approval and transmittal to the Commission.

Access Authorization Policy Statement and Industry Guidelines The Access Authorization Policy Statement and the Industry Guidelines have been modified based on discussions with NUMARC. -The revised package is in review by cognizant offices for reaffirmation of previous concurrences.

Subsequent to office concurrence, the package will be forwarded to the CRGR.

Items of Interest a.

Advance Medical Systems Inc.

An Order Suspending License and Order to Show Cause, effective immediately, was issued today to Advanced Medical Systems, Inc.

In addition, Orders Modifying Licenses are being issued to six medical facilities. These actions are based on findings that since the spring of 1985 and as recently as September 1986, employees of Advanced Medical Systems, Inc. were directed to perform certain service and maintenance on teletherapy equipment at medical facili-ties notwithstanding their lack of NRC authorization, their lack of required training to perform the directed maintenance, their lack of appropriate radiation detection and monitoring equipment or required service manuals, and their express objections to per-forming such maintenance without proper training.

In addition, one medical facility at which such service and maintenance was performed has indicated its belief that the licensee's employees were unqualified to perform the maintenance on its teletherapy equipment.

Civil Penalties Paid (1)

Florida Power and Light. Company (Turkey Point) in the amount of $300,000 based on violations involving inadequate main-tenance of the auxiliarly feed system and supporting backup nitrogen system.

IE Compliance Bulletin No. 86-03, Potential Failure of Multiple ECCS Pumps Due to Single Failure to Air-0perated Valve in Minimum Flow Recirculation Line was issued to all nuclear power reactors holding an operating license or a construction permit.

A Notice of Violation and Proposed Imposition of Civil Penalties was issued to the University of Utah in the amount of $3,000. This action is based on transportation, health physics, safeguards, and operation violations.

OFFICE OF NUCLEAR REGULATORY RESEARCH Items of Interest Week Ending October 10, 1986 Nuclear Plant Analyzer / Nuclear Plant Data Bank (NPA/NPDB)

The Nuclear Plant Analyzer (NPA) is being developed at INEL to provide simultaneous graphics displays and user interactive control of on-going computations.

It has just been successfully demonstrated on the LANL CRAY-1 mainframe computer using the TRAC-PWR code. This now expands the NPA to include the 3-dimensional capability of the TRAC code as well as providing implementation of the NPA on an additional computer mainframe type. Use of the NPA on the CRAY-1 mainframe will expand use of the NPA to users requiring access to a high speed machine.

The Nuclear Plant Data Bank (NPDB) is under development at LANL to provide a source of plant design and operational data for the generation of computer models for the NPA. The software program used to generate Westinghouse 4-Loop PWR plant models for the TRAC code has been fully defined and programming is now in progress.

University of Maryland 2x4 B&W Simulation Loop The purpose of the University of Maryland at College Park (UMCP) 2x4 Babcock and Wilcox (B&W) Simulation Loop is to perform integral effects tests on thermal hydraulics phenomena relevant to post.small break loss-of-coolant (SBLOCA) transients in reactors of B&W design. The data will be used in conjunction with data from the larger-scale, full-pressure MIST facility to assess thermal hydraulic code capebility.

The UMCP 2x4 B&W loop is vclume scaled to the proportion of 1:500 with a maximum power output of 200 kW and pressure of 300 pounds per square inch. Since it is a low pressure test facility, the data will be also be used to address implications and distortions inherent in scaled experiments.

In the month of September,1986, tests were conducted to simulate the Boiler-Condenser Mode (BCV) during SBLOCA. The tests were started with single-phase natural circulation. The secondary water levels in the steam generators were maintained at a constant level. Then the break was initiated. When the top of the hot leg U-bend was voided, the natural circulation was interrupted. After interruption, the liquid was found to remain essentially stagnant except for periodic oscillations until the OCTOBER 10, 1986 ENCLOSURE E

2 primary side had lost so much inventory that the thermal. center in the hot leg side dropped below that of the coolant level in the secondary sides of the steam generators. Durine the interrupted period, the heat transfer was very poor and the primary pressure rose steadily. The vapor was removed either in a " burping" mode, if the hot leg nozzle was submerged, or in continuous flow if the nozzle was exposed. Once the thermal center in the hot leg dropped below the secondary side coolant level, two-phase natural circulation set-in and efficient cooling was again achieved. Several runs with power-control or pressure-control were conducted and essentially the same physical phenomena were observed. The preliminary conclusion from the tests is that during a SBLOCA, there is a range of primary coolant level (below the top of the hot leg U-bend and above the secondary-side coolant level) in which the primary to secondary heat transfer is poor.

In such situations, if the reactor operator were instructed to allow the primary inventory to drop below that of the secondary side _ coolant level, efficient heat transfer could resume.

Because of the small scale of the facility and differences in piping configurations, considerable analysis will have to be performed before these preliminary conclusions could be applied to a full scale nuclear plant.

Multi-Loop Integral System Test (MIST) Program The MIST facility is an integral test. facility designed to simulate thermal hydraulic aspects of transients in reactors of B&W design. The data will be used in assessing thermal-hydraulic code capability. MIST has already completed transient testing ranging from tests that were set up to study the effects of variation of boundary conditions on the initial post-SBLOCA transient interactions to tests that examined Feed and Bleed capability, and system response after a Steam Generator Tube Rupture. Currently, testing on the effects of venting and non-condensible gases on SBLOCA behavior is on-going. After the venting and non-condensible gas tests, the loop ~will be shutdown for modification in order to perform simulated Davis Besse continuous vent lire tests for Toledo Edison Company. These tests are scheduled to be completed in mid November, 1986. After the Davis Besse continuous vent line tests, the loop will be reconfigured to conduct tests on the effects of reactor coolant pump operation on SBLOCA. All currently planned testing will be completed by January, 1987.

3 Symposium on " Seismic and Geologic Siting Criteria for Nuclear Power Plants A symposium on " Seismic and Geologic Siting Criteria for Nuclear Power Plants," Appendix A to 10 CFR Part 100, was held on October 7, 8, and 9, 1986. The meeting was held for the purpose of identifying issues in the application of Appendix A for the developing of an expanded basis.for the anticipated revision of the. regulation. There was broad representation from the Utility industry, EPRI, AIF, consultants, A&E firms, constructors, and other Federal agencies; e.g., USGS, DOE, NES, and National. Academy of Sciences. A complete transcript was made of the proceedings and will be available in about 10 days. Opening remarks which se.t the tone of the meeting were made by Mr. Eric Beckjord, Director, Office of Nuclear Regulatory Research, and Mr. Richard Vollmer, Deputy Director, Office of Nuclear Reactor Regulation. Approximately 200 persons comprising staff speakers and interested parties attended.

Dr. Ross, Deputy Director, Office of Nuclear Regulatory Research, provided the closing remarks.

ITEMS OF INTEREST OFFICE OF INTERNATIONAL PROGRAMS WEEK ENDING OCTOBER 10, 1986 Foreign Visitors On Monday, Tuesday, and Wednesday the NRC staff met with representatives of the Japanese Agency of Natural Resources and Energy (ANRE) for discussion of a wide range of regulatory and other technical issues.

The nine-member Japanese delegation was headed by Mr. Kunikazu Aisaka, Deputy Director General of ANRE, and their visit marked the third regular meeting between the NRC and the Japanese regulatory contact.

Members of the Japanese team toured the TMI and Diablo Canyon facilities on Thursday and Friday, following the formal sessions.

On Tuesday a delegation of Taiwanese officials representing the Atomic Energy Council of Taiwan and Taipower (a utility) visited NMSS to discuss waste management issues. Taiwan has a low-level waste storage site in operation which it is planning ~to enlarge to accommodate the nuclear waste generated by its various facilities.

OFFICE CF SMALL AND DISADVANTAGED BUSINESS UTILIZATION AND CIVIL RIGHTS Item Of Interest Week Ending October 10, 1986 Small and Disadvantgged Business Program In support of the President's proclamation designating the week of October 5 through 11, 1986, as Minority Enterprise Development (MED) Week, a commemorative program took place at the Hyatt Regency Hotel on Capitol Hill.

Featured speakers included Secretary of Commerce Malcolm Baldridge, Secretary of Transportation, Elizabeth Dole, Acting Administrator for the Small Business Administration, Charles L. Heatherly, and the Director, Minority Business Development Agency, James Richardson Gonzales. The U.S. Nuclear Regulatory Commission participated in the procurement marketplace. About 25 small and disadvantaged business firms picked up NRC literature and discussed their expertise and the opportunity to do business with NRC in the future. The major Federal agency corporate awards were_ presented at the White House on October 8,1986.

., 1986, the Executive Director for Operations approved a proposed rule that would amend 10 CFR Part 70, " Domestic Licensing of Special Nuclear Material," and 10 CFR Part 74, " Material Control and Accounting of.

Special Nuclear Material," by adding the requirement to report the results of physical inventories in 970.53(b) and $74.17. This action codifies the previously voluntary submittal of such reports by fuel facility licensees.

This constitutes notice to the Commission that,-in accordance with the rulemaking authority delegated to the EDO, the EDO has signed this proposed rule for 4

publication in the Federal Register.

SECY-86-271 - Options for Response to Presiding Officer's Recommendation that Formal Hearings be Convened for Sequoyah Fuels Corporation UF6 to UF4 Conversion Facility The Commission by a 3-2 vote,** approved an order in response to a memorandum dated July 3, 1986 from Judge Frye the presiding officer for the Sequoyah Fuels informal adjudicatory proceeding.

The memorandum suggested that the Commission convert the hearing into a' formal, trial-type adjudication under the procedures set j

forth in 10 CFR Part 2, Subpart G.

In the order.the Commission 1

declines again to exercise its discretion to apply its procedures governing formal adjudication to this particular licensing proceeding.

  • Section 201 of the Energy Reorganization Act, 42 U.S.C.
S5841, provides that action of the Commission shall be determined by a

" majority vote of the members present."

Chairman Zech was not present when this item was affirmed.

Accordingly, the formal vote of the Commission was 4-0 in favor of the decision.

Chairman Zech, however, had previously indicated that he would approve this paper and had he been present he would have affirmed his prior vote.

    • Section 201 of the Energy Reorganization Act, 42 U.S.C. S5841, provides.that action of the Commission shall be determined by a

Chairman Zech was not present when this item was affirmed.

In order to allow the will of the majority to prevail, Commissioner Asselstine did not participate in the formal vote on this paper.

If they met that threshold he would have convened a formal hearing.
