NOTICE OF VIOLATION Houston Lighting and Power Dockets: 50-498 South Texas Project Units 1 & 2 50-499 Permits: ],-]
l During an NRC inspection conducted March 27 through May 8,1987, one violation of NRC requirements was identified. The violation involved three examples of failure to follow procedures.
In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C l
(1987), the violations are listed below:
Failure to Follow Procedure Criterion V of Appendix B to 10 CFR 50 requires that activities affecting quality be prescribed by and accomplished in accordance with appropriate instructions, procedures, or drawings. This requirement is part of the approved Quality Assurance Program Description (QAPD) of South Texas Project (STP).
a.
Specifications 3C080AS1001 for coatings inside the reactor containment building (RCB) and 7A810AS1000 for coatings outside the RCB state that machined surfaces shall not be coated.
Contrary to the above, Pipe Supports FC-1009-RR-0002 and FC-9009-RR-0001 contained coatings on their machined parts.
b.
Standard Site Procedure (SSP) 14, Attachment SSP 14-6, Table 4, requires a minimum edge distance for a 5/8-inch Hilti Bolt to be 1 inch actual edge distance.
Contrary to the above, safety-related Heating, Ventilating, and Air Conditioning (HVAC) Support 2-1-0087-S002 did not meet minimum edge distance as required by SSP 14 for the 5/8-inch Hilti bolt attaching the support angle to the building structure, in that, edge distance was 3/4 inch.
c.
Standard Site Procedure (SSP) 38, Revision 1, requires that the support configuration be in accordance with the cut sheet and/or SV010M29500, Drawing General Erection Notes.
Contrary to the above, Class 7 HVAC Support Nos. 2-1-0096-5009, 2-1-0096-S011, and 2-1-0096-S013 violated the installation cut sheet for which a minimum 11/2-inch overlap was specified. Measurement of these supports indicated 1-to 1 1/4-inch overlap.
This is a Severity Level V violation.

2 Pursuant to the' provisions of 10 CFR 2.201, Houston Lighting & Power Company is hereby required to submit to this office within 30 days of the date of the letter transmitting this Notice, a written-statement or explanation in reply, including for each violation:
(1) the reason for the violation if admitted, (2 the corrective steps which have been taken and the results achieved, (3 the corrective steps which will be taken to avoid further violations, and (4 the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
