White j Manager of Nuclear Pcwer j 6 North 38A Lookout Place q 1101 Market Street i Chattanooga, Tennessee 37402-2801 Gentlemen: SUBJECT: SEQUOYAH SPECIAL TESTING INSPECTION (NRC INSPECTION REPORT NOS. 50-327/87-18 AND 50-328/87-18) ' This letter refers to a special testing inspection conducted by an NRC team at the Sequoyah facility during March 16-27 and April 13-17, 1987. The inspection included a review of activities authorized for your Sequoyah facility. At the , conclusion of the inspection, the inspection findings and conclusions were i discussed with those members of your staff identified in the enclosed inspec- ! tion report. Areas examined during the inspection are identified in the report. Within those areas, the inspection consisted of selective examinations of procedures and representative records, interviews with personnel, and observations of activities in progress. The inspection findings indicate that certain of your activities appeared to violate NRC requirements. The violation, references to pertinent require- ments, and elements to be included in your response are described in the enclosed Notice of Violation. We are concerned that the numbers and types of testing deficiencies, which were identified during this inspection using a , small sample of relatively simple tests, may indicate the potential for more
serious problems when a large number of relatively complicated tests are , performed immediately prior to restart of the plant. j The majority of the deficiencies identified in this inspection clearly fall into two major areas--inadequacies in the test procedures and instructions themselves and a lack of organizational discipline in the station staff in the ! area of adherence to procedures. The former aspect has significant programmatic implications and the latter implies lack of attention to detail by the station staff and managers. Therefore, your corrective actions should address the dual nature of our concerns, i.e., broad programmatic issues and failure to adhere to procedural requirements. 8706190122 870610 DR ADOCK 0500 7-
