ENCLOSURE I NOTICE OF VIOLATION Florida Power Corporation Docket No. 50-302 Crystal River License No. OPR-72 During the Nuclear Regulatory Commission (NRC) inspection conducted on Octcber 10 -
November 6,1986, a violation of NRC reouirements was identified.
The viola-tion involved failure to adhere to and properly maintain plant procedures.
In accordance with the " General Statement of Policy and Procedure for NRC Enforce-ment Actions," 10 CFR Part 2, Appendix C (1986), the violation is listed below:
Technical Specification 6.8.1 requires that written procedures covering the following activities be implemented and maintained:
- Procedures recommended in Appendix A of Regulatory Guide 1.33, November 1972;
- Procedures for Refueling Operations; and,
- Procedures for Emergency Plan implementation.
Regulatory Guide 1.33, Appendix A, recommends procedures and/or written instructions for operation of the emergency core cooling systems. Operating procedure OP-404, Decay Heat Removal System, Valve Checklist II, requires valve DCV-96 to be in the open position.
Refueling Operations procedure FP-601, Fuel Handling Equipment Operations,_
requires in Enclosure (7) that during normal startup of the spent fuel bridge that the control console cabinet doors be locked.
This Enclosure al so requi res 'that if any step of the procedure cannot be successfully completed, that the operator notify the immediate supervisor.
Emergency Plan procedure EM-202, Duties of the Emergency Coordinator, specifies the procedures to be used in the " Classification of Postulated Accidents" table depending upon the Emergency Action Levels (EALs).
Contrary to the above:
a.
On October 23, 1936, procedure OP-404 was not implemented in that valve DCV-96 was in a throttled position instead of an open position.
b.
On October 22, 1986, procedure FP-601 was not implemented in that the control console cabinet doors on the spent fuel bridge were not locked during bridge startup and the operator failed to notify the immediate supervisor.

On November 3,- 1986, procedure EM-202 (Revision 26 dated February 13, 1986, with a housekeeping change made on August 14, 1986) was not maintained in that the following specified procedures were either canceled or non-existent:
- AP-102 and AP-112 were canceled on July.14, 1983;
- EM-303 and EM-204(C) were canceled on February 7, 1986;
- AP-241, AP-242, AP-243,'AP-244, AP-245, and AP-251 were canceled on May 12, 1986; and
- EM-114 and EM-390 were non-existent.
This is a Severity Level IV Violation (Supplement I).
Pursuant to the provisions.of 10 CFR 2.201, Florida Power Corporation is hereby required to submit to this Office within 30 days of the date of the letter transmitting this Notice a written statement or explanation in reply including:
(1) ' admission or denial of the alleged violation; (2) the reasons for the violation if admitted; (3) the corrective steps which have been taken and the results achieved; (4) corrective steps which will be taken to avoid further violations; and (5) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
