W PRELIMINARY NOTIFICATION OF SAFEGUARDS EVENT PNS-V-86-07 Date: Dec. 3, 1986 This preliminary notification constitutes EARLY notice of events of POSSIBLE national c curity or public interest significance. The information presented is as initially rscaived without verification or evaluation and is basically all that is known by the Region V staff on this date.

SUBJECT:

UNAUTHORIZED MANIPULATION OF PLANT COMPONENTS At 2:00 p.m. on November 29, 1986, a Unit 1 plant operator determined that the red and green indicating light lenses had been reversed for five ventilation fan switches in the 4KV cwitchgear room. Inspection also determined that similar indicating light lenses had been rsversed for one diesel-generator output breaker. The operator who identified the anomalies had been in the room at 8:00 a.m. on the same day and knew that the lenses had been correctly po:itioned at that time. An immediate inspection of the 4KV and 480-volt switchgear rooms cod both diesel generator rooms identified no other mischief, and determined that no manipulation of plant. components had occurred. An investigation was initiated by corporate l cacurity investigators, including interviews of 25 individuals who had been logged into the erss (by the security computer) during the time period of concern. The Senior Resident In:pector also conducted independent inspections of other important plant areas, including battsry rooms.

On December 2, 1986, the licensee determined that unauthorized manipulation of ten n:n-:sfety related circuit breakers had occurred in Unit 3. The time period during which th2 manipulations occurred was between 7:15 and 8:00 a.m. on December 2, based on a related canunciator received in the Control Room. The ten circuit breakers were associated with oil lift pumps and cooling water throttle valves related to reactor coolsnt pumps (RCPs);

ventilation fans; space heaters for electric motors; the in-core probe drive motor; and sump pumpn in the fuel handling building. The licensee's security computer identified 25 cdditional individuals who had access to the Unit 3 area during the time period of conesrn. The licensee's investigation was expanded to include interviews of these cdditional persons, particularly one individual who was common to both groups (a contract fira watch). As discussed between Region V and licensee management, the licensee has initiated the following actions to assure appropriate levels of safety, security, and plant cy:tta integrity during continuing plant operations.

  • Investigation assistance has been requested from the FBI, and two agents are expected onsite by noon on December 3.
  • Component and systems verification inspections are being conducted for all three units to determine whether other tampering has occurred. Initial inspections completed as of 8:00 a.m. on December 3rd included approximately 70% of safety related and effluent control systems on Unit 1, and 50% on Units 2/3. No further indications of apparent tempering have been identified.

O The individuals (approximately 50) who had access to the affected areas at the estimated times of apparent tampering have had their security badges withdrawn to prevent their entry into protected and vital areas until investigations or other appropriate actions are completed.

be specifically discussed with the individuals being interviewed as part of the

investigation by SCE c.orporate security personnel.

The San Onofre resident inspectors are providing onsite coverage of the license.'s actions, cod the Chief of the Region V Safeguards Section has been dispatched to the site. Steady j ct:ta plant operation is continuing on the three units at 93,100 and 83 percent power.

Th2 circumstances surrounding these events w.re discussed with the Incident Assessment Team, NRR, and IE during the evening of December 2, and the Commissioners' aides were brisfed during the morning of December 3. A confirmatory action letter addressing the above
