The enclosed report refers to a special onsite review by the NRC Augmented

Inspection Team on June 2-13, 1986 relating to the failure of the LaSalle

Unit 2 reactor to scram during a feedwater transient on June 1, 1986. The
'

report also refers to the followup activities of your staff, NRC Region III-
. personnel, and NRC Headquarters personnel and to the authorization for the
restart of LaSalle Unit 2.
The report identifies specific areas which were inspected between June 1
and 13, 1986. Within the areas, the investigation consisted of a review
of representative records, observations, and interviews with personnel.
In accordance with 10 CFR 2.790(a), a copy of this letter and the enclosure
will be placed in the NRC Public Document Rocn.
