S.O.R. OPERABILITY EVALUATION TEST I.

INTRODUCTICW During a feedwater transient evente at LaSalle County Station Unit 2 on June 1, 1986, it was noted that three-out-of-four of the Reactor Water Level 3 instruments required to initiate a Level 3 scram at a design reactor water level of +12.5" apparently did not actuate at the design point.

It was observed that those three devices did actuate at lower reactor water levels, which indicates " apparent" drift of the instrument set point. The magnitude of the drift was examined and it was determined that the instruments were "out-of-tolerance" from their calibration values.

subsequent testing of the four switches was conducted. The instruments were first monitored to determine their "as-found" condition using the station calibration check procedures.

(Note that the station calibration check i

procedure is done with the switches at zero static pressure.) It was noted that during this exercising, the switch setpoints were found to be in error; however, the magnitude of the error was determined to be much less than the apparent error during the feedwater transient event. It was then postulated that the apparent instrument error might be due to a static pressure effect, because the instruments are exposed to a static pressure of approximately 1000 psig at normal operating power (as during the feedwater transient), whereas they are calibrated at ambient pressure per the station procedures.

Additionally, the historical calibration data for these switches were examined to determine if, in addition to static pressure effect, there may be other effects which cause the instruments to drift (with time) from their original set points. The previous "as-found" and "as-left" data were compared to determine the magnitude and direction of the drif t.

It was noted that the majority of comparisons of the prior "as-lef t" to the next "as-found" conditions showed that the instrument set point appeared to have drifted toward a higher magnitude of differential pressure.

This observation at LaSalle was consistant with a set of historical calibration data from the Oyster Creek Station.

(See Section III -

Hypothesis).

The LaSalle Station staff also performed a reactor vessel level drop test in which the switches were exercised by raising and lowering reactor water levels at different elevated static pressures.

(Refer to Figure I.2 for data from this test.) The results of this test showed that one switch performed consistently poorly at elevated static pressures, but improved its performance with decreasing static pressures. One switch performed better than all the others (but still showed a small error in switch actuation) at elevated static 1

pressures and improved its performance at zero static pressure levels.

Two switches responded similarly to one another (both better than the worst, but i

not as well as the best of the four switches). A comparison of this drop test data with that of factory calibration data (submitted by S.O.R. when the switches were fabricated) confirmed that the relative magnitudes of the instrument errors due to static pressure effect were consistent.

Based on the above described observations it was decided to conduct an on-site testing program to determine whether the magnitude and type of observed errors in switch actuations were switch-unique or model-unique.

It also was decided that a thorough testing program could be used to justify continued use of the S.O.R. switches provided that the switch performance is predictable. For instance, once the magnitude of errors resulting from each effect or phenomenon are known for each switch, continued use of the S.O.R.

switches may be allowed, provided that the magnitude and direction of error effects are predictable and fall with an acceptable tolerance band.

S-O-R. OPERABILITY EVALUATION TEST II. PURPOSE The purpose of the testing described herein is three-fold:

a)

To determine _ the magnitude of the static pressure effect on safety-related differential pressure switches (as manufactured by Static-O-Ring) that are installed or scheduled for installation at LaSalle county Station. This magnitude will be compared with an acceptance criteria to decide whether the switch should be used at LaSalle or whether it should be replaced with one with having better performance at elevated static pressures.

b)

To determine the magnitude of the variance in set point due to exercise cycling of the switches to see if the setpoints encountered on successive exercise cycles are bounded by the first actuation encountered in the cycling. The knowledge gained from the test should allow conclusions to be drawn about the predicted performance of the switch once it has been calibrated and placed back in service. This expected performance will be factored into the determination of revised set points, allowable tolerance bands, and/or calibration procedures so that the next safety action requirement will be performed satisfactorily.

c)

To determine the repeatability of performance of the S.O.R.

differential pressure (d.p.) switches frca successive actuations.

From the results of steps a) and b) above, it is expected that a predictable performance of the S.O.R. switches will be determined.

Recommendations for revised set points, allowable tolerance bands, and/or calibration procedure sequences or steps will be made so that the station calibration procedures will take these into account.

S-0.R. OPERABILITY EVALUATION TEST

- III.

HYPOTHESES From review of the data collected following the apparent improper response of the reactor protection System (RPS) at LaSalle Unit 2 on June 1, 1986, the following hypotheses are made relating to the performance of Static-O-Ring Differential Pressure Switch, Model 8103AS-5212-NX-JJTTX6:

" Apparent" Drift of Setpoint, Static Pressure Effect, and Hysteresis Effect - Shift with overrange/underrange.

A)

" Apparent" Drift of SetDoir.t.

A plant shutdown was conducted on March 27, 1986 at the Oyster Creek (O.C.) Nuclear Generating Station Unit #1, due to repeatability / drift problems with the Reactor Low Water Level Scram Switches.

Because of the Oyster Creek event, additional calibration data was taken at LaSalle on 3/31/86, 5/10/86 & 6/1/86 which revealed similar switch behavior on the same model switch.

(Refer to Figure III.A.I.)' Oyster l

Creek had concluded that the drift could not be shown to be time dependent.

(Refer to Figure III.A.2 for historical data from Oyster Creek's comparable d.p. switches.)

1 A detailed review of the calibration records for LaSalle Unit 2 (15 months) confirmed Oyster Creek's conclusions that the drift is not time dependent.

B)

Static Pressure Effect The original S.O.R. factory test data sheets (Refer to Figure III.B.1) for the subject differential pressure switches model 8103AS-B212-NX-JJTTX6 show the effect of static pressure upon instrument set point.

Following the June 1 event, LaSalle Station obtained five sets of data through level drop tests (See Attachment) at 950 psig 500 psig, and 0 psig reactor pressure for four differential pressure switches 2B21-N024A, 2821-N024B, 2B21-N024C, and 2B21-N024D. (Refer Figure III.B.2)

The data obtained for three of the switches (A, B & C) was consistent and in agreement with the original Static-O-Ring test data in that the instruments

  • actuation point increased with an increase in static pressure.

Switch B was the most susceptible to an increase in static pressure in the original Static-O-Ring factory test data, as well as during the level drop test at LaSalle.

However, from the S.O.R. factory test data, the "D" switch behaved such that on increasing static pressures the magnitude of the error decreased.

l We hypothesize that this phenomenon may be explained via the hysteresis phenomenon.

- "D" had the smallest error during the LaSalle level drop tests.

In general, all four switch performances improved (smaller and smaller error) when they were exercised at lower and lower reactor pressures during the LaSalle level drop test.

The "D" switch consistently had the smallest error throughout the tests. The "B" switch consistently had the greatest error throughout the tests.

From this it is hypothesized that there is a static pressure effect on these switches. It is also hypothesized that the magnitude of this effect varies from switch to switch of the same model number and range.

C)

Cycline and Hysteresis Effects From previous LaSalle calibration data and from observations made by the instrument mechanics at LaSalle, it appears that there is also both a " cycling" and a " hysteresis" effect.

The cyclino effect can be described as follows:

On successive exercising of the switch during the calibration check procedure, the switch appears to actuate at lower and lower values, starting at a point higher than the desired trip set point, and approaching the set point on successive actuations.

The Hysteresis effect is described as follows:

When the switch is first actuated by increasing the differential pressure across it starting from zero differential pressure, the "first-actuation" is hieher than the desired trip set point.

Then through successive cycling (exercising the switch such that the d.p. across it is increased and decreased only to the point of tripping and resetting the switch) the switch actuation approaches the desired set point. If the d.p. is decreased to zero again and the cycling is repeated, the next "first-actuation" of the switch is very close to the "first-actuation" previously observed.

However, instead of reducing the d.p. to zero (as above), if the d.p. is increased to 100% (or more) of the instrument adjustable range, the "first-actuation" of the switch, when cycled, is a value lower than the desired trip set point. Successive cycling of the switch (sets / resets) then shows an actuation point of higher and higher values approaching the desired set point.

An example of this effect is shown in the LaSalle Unit 2 March 31, 1986 calibration surveillance data for 2821-N024A,8,C, & D.

The first six sets of data are the result of exercising the switch, having started from a zero d.p. condition. The seventh set of data for each switch was obtained by increasing the d.y.

across the switch until 100% of adjustable range was reached, then lowering the d.p. until the switch reset point was reached, and finally increasing the d.p. again until the "first-actuation" point was reached again. For all four switches, that "first-actuation" point was lower than the desired set point; (whereas the "first-actuation" point reached when increasing the d.p. from a starting point of zero differential pressure showed an actuation point higher than the desired set point.)

From the above observations, it is hypothesized that S.O.R. d.p. switches exhibit a cycling and hysteresis effect.

It is not possible to conclude without further testing that the magnitude of this effect varies from switch-to-switch or from model-to-model.

S.O.R. OPERABILITY EVALUATION TEST IV. prooosed Method of Testine HVDothesis In order to determine the predicted performance of each switch due to static pressure and cycling / hysteresis effects, we propose to perform the following tecting:

Test gl On 311 S.O. R. d.p. switches (refer to listing in Figure IV.1) a static pressure effect cycling will be performed.

(Refer to Figure IV.2) Each unit will first undergo a re-calibration, using the regular station calibration procedure (LIS series) to put the unit into the allowable tolerance band (and known static pressure calibrated condition) that the station normally requires. Then each switch will undergo cycling at zero static pressure (5 cycles) and elevated static pressure (5 cycles.)

Rach set and reset will be recorded. This test will be repeated 5 times for each switch to determine repeatability of performance of "fitst-actuations".

The purpose of this test is to determine the magnitude of the error encountered for 33sh switch due to static pressure effect. The results of the test will be used to first select those installed switches which are considered to be " good" performers with respect to minimal static pressure effects.

Then the magnitude the static pressure effect errors of the

" good" switches will be evaluated and used to help determine how the setpoint, allowable tolerance bands, and/or calibration procedures for these switches may be adjusted to obtain consistent acceptable performance.

Those switches deemed to be " bad" performers with respect to static pressure effects will be replaced with " good" performers.

Switches which perform their safety actions on increasing d.p.

will be cycled at elevated static pressure starting from zero d.p. and exercised to record the set / reset points.

Switches which perform their safety actions on decreasino d.p.

will be cycled at elevated static pressure starting from both zero d.p. and 100% of the switches adjustable range.

The static pressures used for the test will be the worst case static pressures that will be seen by the d.p. switches when

~

they are required to perform their safety actions.

t All set / reset values will be recorded.

During the test the static pressure effect on the device used to measure the set / reset values will be taken into account.

Test 92 on four switches of each service function, a second test will be conducted.

(Refer to Figure IV.3) This test is designed to determine the magnitude of the error introduced by the hysteresis effect. The switches will be cycled (at their appropriate elevated static pressures) first starting from zero d.p. (10 cycles) and than starting from the switch's 100%

adjustable range (10 cycles). This entire cycling will be conducted twice for each switch tested.

By examining the data from both tests, it is proposed to evaluate whether the static-pressure-effect-error bounds, or is bounded by, the error from the cycling / hysteresis effects. The knowledge gained from these tests will be used to evaluate a new allowable tolerance band for each service function.

15. Instrument calibration could be affected if air is entrapped in the process sensing lines of the instrument. Ensure during performance of this procedure, that air does not become entrapped in the liggit filled sensing lines. Ensure that test tap lines are filled with liquid prior to connection of test equipment. Purge the air from either hi$ pressure side to low pressure side or vice versa. Repeat until ao air bubbles are observed, then proceed with test. S.O.R. OPERABILITY EVA1.UATION TEST VI Proposed Method of Evaluatine Test Data - Acceptance Criteria Several discussions with the manufacturer of these switches (S.O.R.) have been conducted to determine what the maximum repeatable allowable tolerance band should be.

When the switches were first put into service.

i the allowable tolerance was chosen so that the difference between the desired calibrated trip setpoint and the nominal Technical Specification l

Trip setpoint was equal to 1/2 of the allowable tolerance band.

Historical operating evidence has shown that this value is too small.

(currently approx. 1 1%) of adjustable range.)

j The most recent discussions with S.O.R. has shown that 12 % of adjustable range might be a more appropriate allowable tolerance band.

It is i

proposed that the results of the switch performances during the test program will be compared to the 1 2% of adjustable range values. The testing may show, however, that this 1 2% value may be acceptable for some ranges of switch and not others. Therefore a careful examination will be made of the Test #1 data to determine a more appropriate allowable tolerance band for those switches which do not meet the criteria.
