37. This report contains brief summaries of changes to procedures and plant modifications, which change the plant as it is described in the FSAR. There were no tests or experiments conducted during this interval, which are not described in the FSAR which require reporting in this report. Changes to FSAR Chapter 14 have been previously submitted by separate letters.

CHANGE TO FACILITY AS DESCRIBED IN THE FSAR TITLE:

PCR-000259, Auxiliary Feedwater System Venting Provision.

FUNCTIONAL

SUMMARY

This plant modification added high point vents on discharge piping downstream of the Auxiliary Feedwater (AFW) Pumps. The addition of these vents will allow purging air or steam from the piping without the need for running the AFW pumps.

SAFETY

SUMMARY

This modification was designed to ASME Section III and Seismic Class I requirements. The added valves fall within the existing safety analylsis. This modification does not increase the probability or consequences of analyzed accidents, nor introduce a different type of accident or equipment malfunction than already evaluated in the FSAR. Thus no unreviewed safety question exists.

SUMMARY

This plant modification added system high point vents to facilitate normal plant operations. These additional vents will allow adequate surveillance testing, reduce maintenance time, eliminate system flush to refill and vent the system, and allow periodic testing for hydrogen migration.

SUMMARY

This modification was designed to ASME Section III and Seismic Class I requirements. The additional valves fall within the existing safety analysis. SUMMARY

This plant modification deletes the Boron Concentration Monitor., The Boron Concentration Monitor has been valved out and abandoned in place.-

SAFETY

SUMMARY

The Boron Concentration Monitor was designed for use as an advisory system and the deletion of this monitor has no safety impact.

It was not designed as a safeguards system or a control system. No credit was taken for this system in an accident analysis. As a general operating aid, it provided information as to when additional checks were. warranted rather than a basis for fundamental operating' decisions.

CHANGE TO FACILITY AS DESCRIBED IN THE FSAR TITLE: PCR-000489, Emergency Service Water (ESW) Auxiliary Feedwater (AFW)

Crossover Legs FUNCTIONAL

SUMMARY

This plant modification adds an orificed drain line with a loop seal to the ESW/AFW Crossover Piping. The spaces between the redundant valves separating the AFW suction and ESW system had previously been vented to prevent leakage from ESW to AFW. It was determined that on switch over to the ESW supply the resulting air _ bubble may damage the AFW pumps. The modification allowed the space to be filled with water and vented to a drain to accomodate any ESW leakage.

SAFETY

SUMMARY

.This change is intended to increase the reliability of the AFW system. The installation of the loop seal prevents the creation of an air-binding malfunction. This change does not increase the probability or consequences of an analyzed accident, nor does it create a new or different type of accident. This change improves the reliability of the AFW System and therefore does not significantly reduce the margin of safety.

SUMMARY

This plant modification will install casing vent valves on the suction and discharge sides of the IA-SA and IB-SB Auxiliary Feedwater (AFW) pumps. Vents fitted with plugs already existed in the pump casings.

The addition of vent valves on the pump casing will enhance filling and venting of the AFW pumps and system.

SUMMARY

Auxiliary Feedwater reliability and availability will not be decreased by the modification as described above. SUMMARY

. This plant modification restores service air piping to bypass an underground air leak in the vicinity of the Diesel Generator (DC)

Building.

SUMMARY

The service air system is not safety related and rerouting the servit: air piping in the vicinity of the DC Building does not impact any safety reisted equipment. The_ rerouting of this piping does not alter which equipment is being serviced with air in the vicinity.

SUMMARY

This plant modification involves the partial removal of HVAC heater elements to eliminate hot spots in low flow areas. The heater capacities as listed in the PSAR were reduced.

SAFETY

SUMMARY

These heating coils are located in the non-safety related normal supply system servicing the Fuel Handling Building spaces below the operating floor. This change affects only local HVAC balancing and heater output. The original design temperatures and flows are not affected. This change does not affect any accident analysis or procedures.

SUMMARY

The FSAR states that the Auxiliary Feedwater (AFW) System will be -used to adjust steam generator. water levels prior to an'd during plant start-up. This was necessary due to a Steam Generator Low Pressure interlock which prevented the use of the Condensate /Feedwater system to be used for this function. This change disables the interlock and allows the Condensate /Feedwater system instead of the AFW system to be used to adjust steam generator water levels prior to and during plant startup.

SUMMARY

The Steam Generator low pressure interlock does not protect equipment important to safety during specified plant conditions. This modification does not increase the probability or consequences of analyzed accidents nor introduce a different type of accident or equipment malfunction than already evaluated in the FSAR. SUMMARY

This plant modification eliminates the requirements for fire seals in non-rated floors.

SUMMARY

The F.P. Safe Shutdown Analysis takes no credit for fire seals located in non-rated floors. Fire seals in non-rated floors are not included as part of the protection described in the SSA. Eliminating fire seals for which no credit was taken does not affect the probability or consequences of a fire analyzed in the SSA. The plant margin of safety is not significantly reduced.

1 CHANGE TO FACILITY AS DESCRIBED IN THE FSAR TITLE: FCR-SP-292, Condensate Vacuum Pump Effluent Treatment System (CVPET).

Fan Inlet and Discharge Valve Modification FUNCTIONAL

SUMMARY

Due to orientation of CVPET Fan discharge valves moisture

-accumulates in the vertical duct adjacent to these valves. This plant modification relocates the outlet valve to a horizontal section downstream of the fan and deletes the inlet valve.

SAFETY

SUMMARY

The CVPET is a non-nuclear safety ventilation cleanup system and is not part of the radwaste management system. The purpose of these valves was to prevent air from recirculating through the idle fan.

Eliminating the inlet valve and relocating the outlet valve does not affect this function and does not affect any accident analysis or the margin of safety.

SUMMARY

This plant modification deletes HEPA and Prefilters from 7 of the 9 Fume Hoods Exhaust Systems in the Waste Processing Building Lab Area.

SUMMARY

CHANCE TO FACILITY AS DESCRIBED IN THE FSAR TITLE:- FCR-C-6888, Masonry (Block) Walls in the Reactor Auxiliary Building -

Hot Shop Area FUNCTIONAL

SUMMARY

This plant modification allows non-seismically designed masonry (block) walls to be erected in the Reactor Auxiliary Building (RAB) in the Hot Shop area on Elevation 236', between 'B' and 'H' Lines and 43' and 45' Lines. This area was previously designated for Unit 2.

Since the cancellation of Unit 2, the area is now utilized as a hot shop area. Prior to this change only seismically designed block walls were approved for use in the RAB.

SUMMARY

There are no safety systems or safety equipment located in the subject Hot Shot area. The subject change in design of block walls will not increase the probability or consequences of analyzed accidents, nor introduce a different type of accident or equipment malfunction than already evaluated in the FSAR. SUMMARY

FCR-SI-1011 resolved problems with the Emergency Diesel.

Generator varimeters reading downscale'and conflicts between design documents concerning correct _ polarity. Resolution of the problems included installation of new' scales on meters located on the Auxiliary Control Panel and the Main Control Board which differ in range than currently specified in FSAR Tables

'7.4.1-1 and 7.5.1-3 as indicated below. The following Diesel Generator Power indicators originally had a range of 0-8 MW and now have a range of 0-9.6 MW: EI-6957A2, EI-6957B2, EI-6957A1, EI-6957Bl. The following Diesel Generator Power indicator originally had a range of 0-6 MVAR and now has a range of 0-9.6 MVAR:'EI-6958A.

SUMMARY

The above change is to the meter scale range only and does-not effect the actual output of the Diesel Generator. This change does not increase the probability or consequences of analyzed accidents, nor introduce a different type of accident or equipment malfunction than already evaluated in the FSAR. CHANGE TO FACILITY AS DESCRIBED IN THE FSAR TITLE: FCR-I-3893 Rev. 1, Sequencer Test lA-SA FCR-I-3896 Rev. 1, Sequencer Test IB-SB FUNCTIONAL

SUMMARY

This plant modification consists of relay and wiring changes to prevent relay racing between the sequencer test and the sequencer program relays when the test operation is interrupted. As previously designed, if an accident condition occurred or testing was interrupted while testing of the sequencer internal circuits was in progress, all the circuits would de-energize in 5 seconds. This could result in the test relay de-energizing before the program relay thus initiating an ESF actuation.

Operation of the test relay is now delayed such that the sequencer program relays de-energize first and then the testing relays.

SUMMARY

The modification decreases any relay racing between sequencer test relays and program relays. The change does not increase the probability or consequences of analyzed accidents, nor introduce a different type of accident or equipment malfunction than already evaluated in the FSAR. The Turbine originally included an automatic Turbine Trip in the event the exhaust hood temperature reached 250*F.

This plant modification disarms the automatic trip function. Turbine Exhaust Hood Temperature data is still available to the operator and if necessary a manual trip could be initiated for High Exhaust Hood Temperature.

SUMMARY

The axial location of the californium source within the primary source rod was changed. The californium source in the primary source rod has been repositioned 8 and 1/2 inches higher in order to meet the Westinghouse requirement that the primary source be located mid plane of the source range detector. This change results in a change to FSAR Figure 4.2.2-13 (i.e., the bottom spacer height is now 41.0 inches, instead of 32.5 inches).

SAFETY

SUMMARY

The required minimum source range audible count rate is not changing. This change repositions the primary source in order to meet the requirement that it be located mid plane of the source range detector. As such, this change fulfills the original design requirements instead of revising them. The change does not increase the probability or consequences of analyzed accidents, nor introduce a different type of accident or equipment malfunction than already evaluated in the FSAR. Thus no unreviewed safety question exists.

This plant modification deletes the flow of service water to the cooling coils for the air handling unit (AH-85) which services the Electric Equipment Room in the Diesel Generator Building. As part of this change, the systems return air damper is now locked fully open and the outside air damper opens and closes as outside and room temperature change.

SUMMARY

Calculations were performed to demonstrate that with service water isolated from AH-85 the ability to maintain temperature control in the Electrical Equipment Room is retained. SUMMARY

This plant modification pipes the Heater. Drain Pump mechanical seal drain / bleed off line to the pumps suction. As previously designed, the seal dealn/ bleed off line went to an open floor drain. In this configuration, with the pumps running, excessive steaming occurred presenting a personnel hazard and the potential of excessive moisture in motors.

SUMMARY

This plant modification enables the DEH system to successfully respond to a large generator load rejection. The previous configuration of the DEH system computer and fluid components were such that it was not capable of handling a large generator load rejection. As part of Westinghouse recommendations, the overspeed protection control setpoint was increased from 103% as described in FSAR Section 10.2.2.5 to 105% of the rated turbine speed.

SAFETY

SUMMARY

Increasing the overspeed protection control setpoint is necessary to enable the system to respond to a large generator load rejection. This change does not increase the probability or consequences of analyzed accidents, nor introduce a different type of accident or equipment malfunction than already evaluated in the FSAR. This does not impact on the turbine mechanical or electrical trip for overspeed which is 110% of rated speed. Thus no unreviewed safety question exists.

PCR-000310, Vendor Mobile Solidification System Hook-Up FUNCTIONAL

SUMMARY

The previous design of the Radwaste Solidification System did not provide the capability to hook up a vendor's mobile solidification system as a backup to the installed Stock Solidificatien System. This modification allows a vendor to hook up a mobile unit.to the plants Solidification Systems Pretreatment Tank, Spent Resin Storage Tank, and Filter Particulate Concentrates Tank.

SAFETY

SUMMARY

The consequences of a spill of the Liquid' Waste Processing System will not increase since the contents will be contained within the Waste Processing Building (WPB). The modification will not increase the inventories or sources contained in the WPB which have already been analyzed.

CHANGE TO FACILITY AS DESCRIBED IN THE FSAR TITLE PCR-000248, Addition of Nitrogen Sparger System To The Reactor Makeup Water Storage Tank (RMWST)

FUNCTIONAL'

SUMMARY

This plant modification installs a nitrogen sparger in the RMWST in order to effectively strip dissolved oxygen from the contents of the tank. The supply of nitrogen to this sparger is limited by a one quarter inch pressure regulating valve and is normally discharged through four three inch non-nuclear safety vent valves. A six inch ASME Sect. III, Class 3 relief valve has been placed on the overflow of the tank for additional protection.

SUMMARY

The RMWST is not required for safe shutdown of the plant.

This modification does not increase the probability or consequences of analyzed accidents, nor introduce a different type of accident or equipment malfunction than already evaluated in the FSAR. Thus no unreviewed safety question exists.

CHANCE TO FACILITY AS DESCRIBED IN THE FSAR TITLE: PCR-000422, Feedwater Heater Vents and Drains FUNCTIONAL

SUMMARY

This is not a modification to the plant but is a correction to design documents.

FSAR Figure 10.1.0-5 describes the valves (1HD-157 and 1HD-457) on heater drain lines which come off the discharge side of the Heater Drain Pumps (1A & IB) going to the Sampling System as normally closed. This change will reflect the correct operation of the system on the design drawings and FSAR Figure by showing these valves as normally open.

SUMMARY

The Heater Drain System is a non-safety system and does not affeet safety related equipment. This change does not increase the probability or consequences of analyzed accidents, nor introduce a different type of accident or equipment malfunction than already evaluated in the FSAR. CHANCE TO FACILITY AS DESCRIBED IN THE FSAR TITLE Containment Cooling System Normal Operation FUNCTIONAL

SUMMARY

This plant modification revises the normal operation of the containment safety fan coolers dampers. The nozzle dampers are interlocked so that under normal operating conditions the dampers are positioned depending on the containment average temperature.

If the containment average temperature is below or equal to ll8'F, the nozzle dampers are closed. If the containment average temperature is above 118'F, the nozzle dampers for safety fan coolers AH-3 and AH-4 are open and the nozzle dampers for safety fan coolers AH-2 and AH-1 are closed.

The design of the process radiation monitors for the Auxiliary Steam (AS) Condensate return was supplement ed to provide automatic termination of releases to the condenser or the auxiliary boiler and automatic isolation of the tank vents when radiation is detected in the AS condensate.

SUMMARY

The plant design uses an auxiliary steam system to provide process steam for radioactive waste evaporators. The steam used in these evaporators is condensed and collected for reuse.

In the'unlikely event that the steam side of the evaporator becomes contaminated due to leaks within the evaporator, the radiation monitors will provide isolation of the transfer of this water to a noncontaminated system such as the auxiliary boiler. These monitors were previously described in the FSAR, but the automatic isolation function was not. The addition of the isolation function provides a protection against the undetected release of radioactive material from the plant.

The PSAR li ts exceptions to ANSI 45.2.2 as endorsed by Reg. Guide 1.38.

This Reg. Guide deals with QA requirements for the storage of safety-related items prior to installation in the plant. The FSAR provides exceptions and clarifications with regard to this Reg. Guide. These items were supplemented with an additional exception to include the latitude to evaluate storage recommendations (including maintenance required during storage) made by the vendor in view of the specific storage condition provided at the plant.

If after evaluation, it is concluded that equivalent protection is provided, the vendors specific recommendation may be modified.

SAFETY

SUMMARY

The purpose of the regulatory recommendations is to assure that storage conditions do not degrade safety-related equipment. The change assures that any change to the vendors recommendations are documented and provide an equivalent level of protection during storage.

This modification allows improved chemistry control. This modification deals with the non-safety portion of the condensate and chamical feed system. This modification does not change the elevation of any penetrations in the CST. The change does not increase the probability or consequences of analyzed accidents, nor introduce a different type of accident or equipment malfunction than already evaluated in the FSAR.

Thus no unreviewed safety question exists.

This procedural change revises Fire Protection procedures for the semi-fixed foam system for the auxiliary boiler fuel oli storage tanks located in the yard. After the foam solution is depleted, water will be manually shut off to prevent diluting the foam blanket instead of the previous Fire Protection practice of allowing the water to centinue to flow.

SAFETY

SUMMARY

This procedural change has no safety impact since this foam system is not used to protect safety related systems or areas. The procedural change actually improves the fire protection provided to the auxiliary boiler fuel oil storage tanks and as such does not decrease the margin of safety.

This change does not affect the Fire Protection Safe Shutdown Analysis.
