1.1 PURPOSE 1.1.1 The purpose of the continuing training (requalification) program is to enhance nuclear plant safety and reliability by maintaining a level of skill and knowledge in Control Room Operators, Shift Supervisors and Shift Superintendents as required to achieve high standards of job performance.

1.2 -DISCUSSION j 1

1.2.1 To achieve this purpose, a continuing training program must be i established which is sufficiently broad in scope to review areas of knowledge necessary for safe plant operation and flexible enough to cover recent operating experience and operational changes. The continuing training program should satisfy the following broad objectives:

a. Maintain and improve skills and knowledge required to accomplish routine and emergency duties.
b. Establish a system for evaluating and documenting i performance levels on a regular (at least annual) basis.

c. Provide a vehicle for dissemination of lessons learned from l industry operating experience.
d. Improve the teamwork and diagnostic skills.  !

1.2.2 This procedure does not address the requirements for general employee training nor training for fire brigade members or leaders.

1.2.3 Licensed staff personnel are encouraged to participate in the i continuing training program because of the potential benefits to other trainees as well as maintenance of their own skills and knowledge. They can provide technical input, management viewpoint and visible evidence through their presence that management considers training to be important. In addition, participation in the continuing training program by licensed staff personnel affords them an opportunity to interact with operations personnel to promote overall improvements in operations. Should licensed staff personnel be assigned job responsibilities of control room personnel, they should be pre-pared as discussed in Section 6.10, Inactive Status Retraining.

1.2.4 Licensed staff personnel may be exempt from participation in specific areas of the continuing training program to the extent that their normal duties preclude the need for retraining in the specific areas. This exemption from retraining shall be approved by the Training Superintendent.

'l Requalification Training l 01-S-04-2 l l l 2.0 RESPONSIBILITIES 2.1 Manager, Plant Support 2.1.1 The Manager, Plant Support is responsible for the overall administrative control and conduct of the Continuing Training Program. This effort should be coordinated with the Manager, Plant Operations and the Manager, Plant Maintenance.

2.2 Manager, Plant Operations 2.2.1 The Manager, Plant Operations is responsible for ensuring that all licensed personnel under his cognizance whose license are to remain active attend the required portions of the continuing training program. He is also responsible for furnishing any desired training topics which he may wish to be presented to the Training Review Group for consideration.

. 2. 3 Manager, Plant Maintenance 2.3.1 The Manager, Plant Maintenance is responsible for ensuring that all licensed personnel under his cognizance whose licenses are to remain active attend the required portions of the continuing training program. He is also responsible for furnishing any desired training topics which he may wish to be presented to the Training Review Group for consideration.

2.4 Training Superintendent 2.4.1 The Training Superintendent is responsible for providing and implementing a continuing training program which meets the requirements of this procedure. This effort is to be coordinated with the Operations Training Review Group.

3.8 Training Section Instruction 14-S-02-14, Examination Safeguards 4.0 ATTACHMENTS None 5.0 DEFINITIONS 5.1 Critical Safety Functions - functions needed to control reactivity, remove core heat, provide an ultimate heat sink and contain radioactivity.

5.2 Diagnostics - A systematic, logical analysis of plant conditions to identify problems and potential causes of these problems, assign probabilities to each potential cause, and develop a logical and prioritized sequence for corrective actions to correct or eliminate each potential cause.

5.3 Contact Hour of Instruction - A one hour period in which the course instructor is present or immediately available for instructing or assisting students: 1ectures, seminars, discussions, problem solving sessions, periods of assigned study and examinations are considered contact periods under this definition.

5.4 Licensed Staff Personnel - Any individual who possesses a current operators license or senior operators license pursuant to title 10, Code of Federal Regulations, part 55 " Operators' Licenses" who does not normally, but could be called upon to perform or supervise license activities.

5.5 Self-Study - Training during which a student or students are given specific assignments, supplied appropriate training materials and objectives and assigned study. Instructors are to be available during the assigned periods and attendance of the students will be verified on a periodic basis. This method of training is most

, appropriate for remedial or refresher training.

5.6 Control Room Personnel - Those individuals whose job duties include operations of plant controls from the control room,'or who directly supervise those individuals. Normally, this would include Control Room Operators, Shift Supervisors and Shift Superintendents.

5.7 Lecture - A presentation by an instructor whose contents pertain to one topic or subject. A presentation of the characteristics and design of the Primary Water System would be considered one lecture by this definition.

5.8 License Duties -Duties for which an individual is required to hold an NRC Operator's or Senior Operator's License pursuant to Title 10 CFR 55. This normally includes manipulation of controls affecting reactivity of the reactor or supervision of such duties.

6.1 General Requirements 6.1.1 Instructors who teach systems, integrated responses, transients or simulator courses shall hold or have held a Senior Operator's License on this or a similar facility (BWR) or hold an NRC instructor certification for GGNS and shall be enrolled in the continuing training program as described herein.

a. This requirement shall not prohibit the occasional use of experts on specific materials and guest lecturers.

6.1.2 Examinations given during this program shall conform to the requirements of Reference 3.6 and 3.7. They shall be safeguarded from compromise in accordance with Reference 3.8.

6.1.3 Records of the requalification program shall be maintained to document each licensed operator's and senior operator's participation and performance in the Continuing Training Program. These records will contain:

a. Copies of written examinations administered
b. Written answer sheets completed by examinees
c. Results of performance evaluations
d. Documentation of attendance at all training given
e. Documentation of licensed operator's and senior operator's cognizance of changes made to station design, appropriate procedures and the station license
f. Documentation of licensed operator's and senior operator's review of off-normal and emergency procedures
g. Documentation of licensed operator's and senior operator's participation in reactivity control manipulations.

6.1.4 The training program content and feedback must be reviewed by the Operations Training Review Group. This requirement is not effective until Jan 1, 1986.

The training sessions should include two types of lecture series as follows:

a. Fundamentals Review Lecture Series
b. Operational Proficiency Lecture Series 6.2.1 Fundamental Review Lecture Series - The fundamentals review training sessions cover areas in which the knowledge required of an individual is infrequently used but is important to safe and reliable operation.
a. Lecture Series Topics - The fundamentals review lecture topics should include topics from the below list as selected using paragraph 6.2.1.b below:

b. Lecture Series Topic Selection - Performance of the control room personnel, interviews with control room personnel and results of the annual examinations should provide the basis for the selection of lecture topics. Particular attention should be paid to the ability of personnel to apply fundamental knowledge to operational situations.

b. Lecture Series Topic Selection - The selection, scope and depth of the lecture series should be determined by the operating experience reports, feedback on licenses job performance, simulator and dr'ill evaluations, general results of annual examinations and other pertinent data requested by Operations Management. In addition, the Operations Training Supervisor may review the Job Analysis Reports for the plant to ensure that rarely used and difficult knowledge and skills important to safe, reliable operations are reinforced. Such knowledge and skills may be identified in job surveys as demanding very little time and infrequently performed but with extremely high importance or high difficulty. Examples might include the use of safety systems, establishing natural circulation, some aspects of refueling operations or steam condensing

. mode of RHR operations. The selection, scope and depth of each subject should be recommended by the Operations Training Supervisor and approved by the* Operations Training Review Group.

6.2.3 Planned Lecture Series Schedule

a. The planned lecture series will be scheduled on an annual basis. Lectures may be deferred due to unanticipated events, but should be conducted as soon as practicable thereafter and within the annual cycle. Scheduled plant activities should be considered in planning the lecture series. Efforts should be made to conduct lectures pertaining to scheduled plant activities, such as refueling during the training period immediately prior to such activities.

b. The lecture series should be conducted on a continuing basis. It should include separate training segments typically totaling one hundred (100) contact hours of instructions annually divided among the program topics and appropriately scheduled through the year.
c. The lecture series should be scheduled during the periods designated for formal training. No concurrent duties should be assigned to the students during these periods.

, The schedule should include makeup sessions to account for 4

absences or exam failures by trainees.

6.2.4 Lecture Series Attendance

a. All Licensed Operators and Senior Licensed Operators actively performing licensed duties shall be required to participate in the planned lecture series.
b. A record of attendance shall be made and individuals with absences greater than two (2) days in any one week will not be allowed to take the examination for that week. These individuals must make up the absences within 14 weeks by rescheduling lecture attendance or by self-study and discussions with a knowledgeable person designated by the

, Operations Training Supervisor. The absentee is required to pass an examinat. ion covering the material presented during the missed lectures comparable to that completed by those who attended the lectures,

c. Self-study and discussion sessions cannot be used to make-up attendance on more than 40% of the total scheduled

, hours for lecture series topics.

d. The Operations Training Supervisor must promptly notify Operations Section Management of any person required to attend continuing training who fails to meet the requirements of this section recommending they be removed from license duties until such requirements are met.
e. An instructor who presents a lecture is considered to have

, attended the lecture.

6.2.5 Lecture Series Examinations

a. After each lecture or group of lectures all trainees must take a written examination covering the lecture topic (s).

. These examinations will normally be given on a weekly i basis. All lecture topics covered during the training session should be represented by questions in the examination. Instructors identified in 6.1.1 must take

. examinations over the portions they did not present.

b. Examinations will be evaluated and a grade determined. A performance standard of 80% is required for written examinations.
c. Trainees who do not meet the performance standard will not be credited with having completed the lecture (s) and must complete the remedial review process within 8 weeks.
d. The remedial review process will consist of trainee review of the training session material, trainee review of associated reference material identified by the instructor and administration of a second examination covering at least the identified knowledge deficiencies.
e. If the second exam is completed satisfactorily the trainee will receive credit for completion of the required lecture (s).
f. If the second exam is not satisfactorily completed the Operations Training Supervisor should make a dedicated effort to determine the root cause of the failure.

(1) The Operations Training Supervisor should promptly notify Operations Section Management of the evaluation results and provide a recommendation regarding the trainee's removal from license duties and entrance into accelerated training program consistent with the findings in f. above.

6.3 Simulator Training 6.3.1 General - Reactivity Manipulations and Plant Evaluations -

Annually each Licensed Operator and Senior Operator who is performing licensed duties shall participate in a variety of reactivity control manipulations. These should include normal, abnormal and emergency evolutions and other evolutions identified as needed to improve job performance. They are to include the following evolutions on a cyclic basis as noted.

Credit for completion of these evolutions is granted as follows:

Licensed Operator - Satisfactory performance of required actions as evaluated by, at least, a member of the Training Staff qualified in accordance with 6.1.1.

Senior Operator - Satisfactory performance of or direction of required actions as evaluated by, at least, a member of the Training Staff qualified in accordance with 6.1.1 or evaluating performance of required actions by Operations personnel,

a. The following items are to be performed on an annual basis:

(1) Plant or reactor startups to the point that reactivity feedback from nuclear heat addition is noticeable and a controlled heatup rate is established.

(2) Manual control of feedwater during startup and shutdown (3) Any significant ( > 10 percent) power changes due to

manual rod position or recirculation flow (4) Loss of coolant including

(a) Inside/outside primary containment (b) Large/small, including leak rate determination (5) Loss of coolant forced flow / natural recirculation (6) Loss of all feedwater (normal and emergency)

b. The following manipulations should be completed at least

6.4 Annual Requalification Examination l

6.4.1 General - In order to determine each Licensed Operator's and Senior Operator's knowledge of topics covered in the continuing training program and provide a basis for determining areas in which retraining is needed, an annual requalification examination j should be given.

a. The annual examination must be given to all Licensed Operators and Senior Operators actively performing license duties prior to completion of each annual continuing training cycle. Licensed Operators and Senior Operators who have held the license for less than 6 calendar months prior to the start of the exam cycle may be exempted from completion of the annual examinations.
b. The annual examination consists of a simulator performance evaluation, a written examination and an oral examination.
c. All personnel who may function as a Control Room Operator, Shif t Supervisor or Shift Superintendent should complete the annual examination.
d. Up to four licensed individuals may be exempted from the examination by virtue of preparing and grading the examination. These individuals should be fully qualified instructors but should not be the same instructors who have conducted the majority of continuing traiuing during the past year or the review craining prior to the exam in consecutive years, if possible.

6.4.2 Annual Requalification Examination Schedule - Annual requalification examinations may be administered in sections.

i.e. written, oral, simulator. The schedule must ensure, however, that all examinations are completed within the annual cycle.

a. Consideration should be given in establishing the schedule to the possibility of individuals having to complete accelerated retraining prior to passing the annual examination.

6.5 Simulator Performance Evaluations - The annual evaluation of performance will be conducted on the GGNS simulator.

  • 6.5.1 Scenarios for evaluation exercises should contain specific performance standards for key actions by the operating team.

6.5.2 Questions for the evaluators to ask each team member and suggested acceptable response should be included in evaluation scenarios. These questions should be consistent with the individuals job requirements.

a. The questions should be consistent with continuing training program learning objectives and verify the retention of job-related theory and principles.

6.5.4 Simulator evaluation scenarios are to be developed each annual cycle so that changes in learning objectives, changing in training emphasis, operating experiences and changes to plant equipment or procedures may be considered.

6.5.5 Evaluations scenarios should be prepared by and conducted by qualified instructors other than those conducting the majority of simulator training of the team being evaluated, if possible.

6.5.6 Evaluations and evaluation scenarios should be given and/or controlled so as to avoid the potential for compromising their validity as evaluations tools.

6.5.7 NUREG 1021, ES-302 may be used for guidelines for conducting the simulator examination where applicable and where the guidelines do not conflict with other requirements of this procedure.

6.5.8 Determination of acceptable overall performance is made on a pass-fail basis. A determination of fail must be made for those personnel responsible if:

a. During the conduct of the examination Safety Limits are exceeded where exceeding thereof could have been prevented.

i b. During the conduct of the evaluation Technical Specification violations occurred without proper justification.

c. During the conduct of the evaluation conditions which could potentially affect the health and safety of the public were not detected and/or responded to in a timely manner,
d. During the conduct of the evaluation a procedure violation occurred that could result in adverse
  • effects on safety or health of the public or in damage to safety related
equipment of such an extent as to keep the equipment from fulfilling its safety related function.
e. Knowingly violating any procedure without proper justification.
f. If, during the course of the evaluation, an individual displays less than about 70% attainment of the specific performance measures outlined per 6.5.1 and/or fails to respond properly to about 30% of the questions presented to them in accordance with 6.5.2.

g. If, in the individual evaluators judgement, an examinee displays a significant deficiency in a subject vital to plant safety such that the examinee's ability to safely operate the reactor is in doubt.

Otherwise a determination of pass should be made.

6.5.9 Should the NRC choose to give the annual simula~ tor examination, it shall be considered the examination required by this procedure and need not be duplicated for those individuals examined to meet the requirements of this procedure.

6.6 Annual Written Examinations 6.6.1 Written Examination Content

a. The written examination should contain examination questions covering the following topics:

d. The examination should be structured so that the level of questioning is consistent with the individuals job requirements.

6.6.2 Written Examination Administration

a. The annual written requalification exam shall be prepared and administered in accordance with 14-S-01-4, 14-S-02-4 and I

14-S-02-14 and the following guidelines.

(1) Prepared exams and answer keys shall be reviewed and

approved by the Operations Training Supervisor or designee prior to administration.

1 (2) Examination answer keys must reflect the expected answers and in the case of multiple part and/or answer questions the point value to be allowed for each response must be indicated. Deviations from the guidelines of the answer key are permissible when an unexpected response is given which can be considered

correct or partially correct. However, justification for these deviations must be documented either on the answer sheet or key or on a separate sheet. The justifications must be initialed by the instructor grading the exam.

(3) Following grading of the examination, the grading is to be reviewed by a second instructor for consistency of

grading and scoring accuracy.

(4) Final review of the examinations and approval of the scores will be done by the Operations Training Supervisor or designee. '

b. Should the annual examination administration be contracted the contractor should be requested to conform to these

, guidelines as much as possible,

c. Should the NRC participate in or give the annual written examination, that examination shall be considered to be the examination required by this procedure for that portion they j

give and need not be duplicated to meet the requirements of this procedure.

6.6.3 Written Examination Performance Criteria

a. Examinations are to be scored on a percentage correct basis.
b. A determination of pass / fail will be made in accordance with the current criteria used by the NRC on the most recent NRC administered license examinations.

(1) At the time of writing this procedure, the NRC Criteria to be used is no less than 80% overall grade and no less than 70% on any one section as currently formatted by the NRC.

(2) This criteria will be used for determination of pass / fail until such time as the NRC format or standards are revised.

6.7 Annual Oral Examination 6.7.1 Oral Examination Content

a. The oral examination should contain questions covering the following areas.

(1) Duties and responsibilities of the position corresponding to the individuals position.

a. The oral examination.should be conducted under a structure enabling consistency of questioning and evaluation. The following guidelines should be considered.

(1) A checklist identifying the areas to be covered should be prepared prior to the examination.

(2) Comments on individual strengths and weaknesses should be made,

b. Personnel assigned to conduct oral evaluations will be assigned by the Operations Training Supervisor.
c. Oral examinations should be conducted by a fully qualified operation instructor.
d. Each oral examination should be structured so that an examination time of two or more hours is appropriate.
e. The oral examination should involve seasions conducted in the plant control room and while walkirg through plant areas that are under the cognizance of the olerations department.

6.7.3 Examination Performance Criteria

a. Oral examinations are to be assigned a 1 ass / fail grade
b. The examiner must make every effort to be as objective as possible and assign a pass / fail grade based on his/her best professional judgement and considering the following guidelines for a pass determination.

(1) An examinee should correctly answer approximately 80%

or more of the questions asked or answer questions with 80% or greater accuracy or a combination of the two.

(2) The examinee does not display significant weaknesses in any one area to such an extent that there is a doubt as to his/her ability to safely operate the reactor.

(3) Should an examinee be marginal, further questioning should be conducted until a determination of pass / fail can be justified. Continued marginal performance would in most cases be justification for failure.

(4) All failures must be documented as to specific weak areas and the reason for failure if not obvious.

6.7.4 NRC Participation - Should the NRC participate in the annual oral examinations, they shall be considered to be the examination required by this procedure for those individuals examined and the examination need not be duplicated to meet the requirements of this procedure.

6.8 Notification of Annual Examination Failure 6.8.1 The Operations Training Supervisor shall, as soon as unsatisfactory results of any segment of the examination, i.e. ,

written, oral, or performance are determined and verified, notify Operations Management of the unsatisfactory results and recommend removal of the individual from license duties.

6.9 Special Retraining Programs 6.9.1 Accelerated Retraining Program - The accelerated retraining program is for individuals having identified deficiencies requiring assignment to a special training program.

a. Required attendance - Licensed individuals meeting one or more of the following criteria shall be assigned to an accelerated retraining effort prior to resuming license duties.

b. Program Content - The accelerated retraining program content should be specifically structured to upgrade knowledges and skills identified as deficient.

(1) The Operations Training Supervisor shall be responsible for the formulation and approval of the individual accelerated training program contents. The Operations Superintendent should, but is not required to, provide recommendations regarding the program content at his discretion.

c. Program Administration - The accelerated retraining program may involve a variety of training methods including:

(1) Directed Self Study (2) Oral interviews and discussion sessions (3) Planned lectures (4) Skills training exercises on the simulator

d. Program Duration - Program duration should be as dictated by the extent of the training required and trainee performance.
e. Performance Standards - Successful completion of the accelerated training program should be determined by administering an examination. The examination should be of the same type, scope and depth as the examination originally failed. It must, however, be of substantially different questions / content than the examination previously failed except that the major subject areas may be the same, i.e. ,

. Theory, Procedures, Emergency Procedures, etc.

(1) The examination will be prepared and administered by individuals designated by the Operations Training Supervisor.

(2) Acceptable Performance will be demonstrated as applicable by:

(a) A score of at least 80% on each accelerated written category necessary.

(b) A passing evaluation on accelerated retraining oral examination.

(c) A passing evaluation on an accelerated retraining simulator examination.

(3) In the event that these standards are not met, the individual's suitability for resuming control room duties will be reviewed by the Operations Training Review Group. The Operations Training Review Group should provide a recommendation regarding the individual's permanent removal from license duties or additional upgrading efforts to be considered. If appropriate, another accelerated retraining program should be structured to correct deficiencies.

6.10 Inactive Status Retraining 6.10.1 General - If a licensed individual has not actively carried out license duties for a period in excess of four months, a special retraining program and/or evaluation shall be administered prior to the individual resuming license duties.

6.10.2 Requirements - In the event that an individual does not maintain an active status, the Operations Training Supervisor should designate a senice operator to conduct an oral examination similar in scope and format to an annual oral examination prior to resuming license duties. In addition, evaluation of performance in the current lecture series should be conducted.

If performance in the lecture series is not to the standards described, a written examination similar in scope and format to the annual written examination should be administered to the individual prior to resuming license duties. The individual should be assigned to a training status on an' operating shift prior to resuming license duties.

a. The performance standards applied to the annual requalification examination should be used in evaluating the results of the oral and/or written examination. If the performance standards are not met, the individual should complete an accelerated retraining program prior to resuming license duties.

6.11 CONTINUING TRAINING PROGRAM EVALUATIC:

The continuing training program should be evaluated on a periodic basis to determine the effectiveness of the t c aram in meeting designated requirements and to identify the job parformance results attributable to training. Changes to the continuing training program should be considered ,
