i APPENDIX A NOTICE OF VIOLATION Maine Yankee Atomic Power Company Docket No. 50-309 Maine Yankee Nuclear Generating Station License No. DPR-36 As a result of the inspection conducted on April 6-10, 1987, and in accordance with the Enforcement Policy (10 CFR 2, Appendix C), the following violation was identified:

Technical Specification 5.12.1. requires, in part, that each high radiation area in which the intensity of radiation is at such levels that a major portion of the body could receive in any one hour a dose in excess of 100 mrem shall be barricaded and conspicuously posted as a high radiation area.

a. Contrary to the above, on April 7, 1987, whole body dose rates in excess of 100 mR/hr were present on the -2 foot elevation, adjacent to the access area to loop #1 in the Containment Building, and were inadequately posted and barricaded.
b. Contrary to the above, on April 8,1987, between 1230 and 1410, access to Reactor Coolant Pump #2, an area with whole body radiation intensities in excess of 100 mR/hr, was not barricaded nor conspicuously posted.
c. Contrary to the above, on April 10, 1987, at approximately 1005, access to the refueling cavity, an area with whole body radiation intensities in excess of 100 mR/hr, was not barricaded nor conspicuously posted.

The above multiple instances of failure to adequately control high radiation areas collectively constitute a Severity Level IV violation (Supplement IV).

Pursuant to the provisions of 10 CFR 2.201, Maine Yankee Atomic Power Company is hereby required to submit to this office within thirty days of the date of the letter which transmitted this Notice, a written statement or explanation in

. reply, including: (1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date.when full compliance will be achieved. Where good cause is shown,. consideration will be given to extending this response time.
