Request supplementation of Dr. James A. Wilson to participate in the implantation of Nuclear powered Pacemakers

Dear Sir:

Since John F. Kennedy Memorial Hospital is licensed to implant Nuclear Pacemakers under the License No SNM-1690, dated Dec-ember 1,1976, we request the addition of Dr. James A. Wilson to the list of physicians responsible for study. Dr. Wilson's qualifications are as follows:

(609) 667-7609 States Licensed in which to practice: New Jersey, Pennsylvania, Florida, Kentucky Specialty Board of Certification: Surgery, 1971 Position with Applicant: Hospital Surgical Staff Previous Experience: Dr. Wilson has been implanting Pacemakers for approximately nine (9) years in which time he has implanted over 400 Pace-makers. Dr. Wilson follows all Pacer patients through a transtelephonic follow-up system located in his office. The system utilized by Dr. Wilson is the Cordis Corporation Commucor System.
