Dear Mr. Manning:
This letter is written to respond to the three issues regarding the lost Coratomic pacemaker
- patient,
- l. When were we informed of the additional patient? What have vie done to locate the patient?

ISSUE #I l
The following data is the chronological attempts to locate the Coratomic pacemaker patient:
- 1. Received a letter from Biocontrol Technology daled May 1990 informing us of an additional patient implanted with a Coratomic pacemak~.l_

- 2. In June 1996 v>e found through Medica! Records that._
jr received a Coratomic pacemaker.
- 3. September 23,1996 it was stated in the Radiation Safety Committee minutes that we were unable to locate the patient or her physician.
- 4. Between September 1996 and December 1996 Jane Oyer, Chief Technologist, made several attempts to find information regarding the patient's and physician's address or phone number.

8 6. Between December 19,1996 and J c.uary 20,1997 several attempts were made thrpugh phong and a re istered letter was sent to the daughter requesting information regardin8L J

7, April 21,1997 - Administration sent a letter to the NRC documenting the attempts to locate
]and asking for guidance.
- 8. In October 1997 another daughter of[
] waslocated and we received the patient's physician name and address. Letters to both physicians were sent registered mail requesting information regardmg the pctient status and explaining the importance of follow up.
We are presently waiting for a response.
- 9. We are tsking for your guidance if no response from either physician or patient.
ISSUH H2 In June 1983 Kennedy Memorial Hospital Chief Technologist sent a letter to the surgeon who was responsible for pacemaker implantations explaining the importance of patient six month follow-up and the NRC reg lation. We received a listing of three (3) patients w;.o had implantations. The patient,I
]had her implantation January 1984. The physician did not inform the licensee of the implantation. The RSO, surgeon, or technologia from 1983 is no longer a Kennedy employee and is net available to question.
ISSUE #3 A letter has been sent to Biocontrol Technology requesting another check of records to assure another patient has not been implanted without our knowledge.
