O Letter dated May 22,1996 from Blocontrol Technology, Inc. Informing Kennedy that under our license 'SNM-1690) there were two patients still listed as being implanted with one of their pacemakers.{

"(Model C 101-P Serial Number 1168) was on that list.

O September 23,1996 - stated in Radiation Safety Committee minutes that we were unable to locate [

~] nor her physician.

O December 12,1996 - Administrative Director of Performance improvement from Kennedy _ was able to locate 3 daughter in Laurel Springs, New Jersey. (

] daughter mentioned that her Mother was living in Florida and that she would have her Mother call Kennedy. We received no response.

O Degember 19, '996 - Jane Oyer, Chief Technologist of Nuclear Medicine, spoke to L

] daughter to explain the importance of following her Mother's medical care regarding her pacemaker.

Kennedy Memorial Hospital is asking for guidance frop the Nuclear Regulatory Commission as to the next step of responsibility regardingl
