We hereby request renewal of the above referenced license which covers the implantation of nuclear powered pacemakers in humans.
Please note the change in the name of our hospital.
We request approval to continue presently authorized activities (i.e.,
receive and implant pacemakers, follow patients with im-planted devices, return explanted pacemakers to manufacturer for proper disposal).
We request the same possession limit.
There have been no changes in personnel from those presently licensed.
This application is intended to supplement previous correspondence with your office as outlined in Condition #16 of the present license.
It is our understanding that there is no fee for this renewal since we hold another NRC license (No. 29-12167-01).
