Sequoyah Fuels Corporation On November 4,1986, Sequoyah Fuels Corporation (SFC) requested Region IV's approval to restart production.
Enclosed with their letter was the status of compliance with the conditions imposed by the October 2, 1986, Order, a sched-ule for starting up production and additional information on the Independent Oversite Team (IOT).
Current plans call for beginning limited cleanout opera-tions in the denitration, reduction, and hydrofluorination systems on November 5, 1986, in accordance with the limited authorization issued on September 20, 1986.
If Region IV withdraws the Confirmation of Action Letter (CAL) on November 7, 1986, SFC plans to start dissolving yellowcake on November 10, 1986, and production of UF between November 19 and 26, 1986, and be in full production by November 27,61986. The IOT is onsite, prepared for 24-hour surveillance, as is Region IV.

Mixed Waste A policy meeting on mixed low-level radioactive waste with EPA and the State of Washington is scheduled for November 14, 1986 in Scattle, Washington.
The purpose of the meeting is to examine technical, procedural, and administrative difficulties that may arise due to initiating the Resource Conservation and Recovery Act (RCRA) permitting process at the Hanford LLW Disposal Facility, Richland, Washington. The States of Nevada and South Carolina, which host the remaining two disposal facilities, have been invited to the meeting as observers.

A 1980 State law places a moratorium on the construction of new nuclear plants until a nuclear waste facility is in place. The Trojan Plant is the only nuclear power plant in the State.

Washington (Referendum #40)
Would require State officials to continue to challenge the-Federal selection process for high-level nuclear waste repositories and provide a means for voter disapproval of any Washington site. This would allow the populace to object to the selection of Hanford as the first HLW repository site.
Early indications are that this was APPROVED with a 82.5-17.5%
vote, with 92% of the precincts reporting.

OFFICE OF NUCLEAR REACTOR REGULATION ITEMS OF INTEREST Week Ending November 7, 1986 Nine Mile Point Nuclear Station Unit 2 A license was issued today to Niagara Mohawk Power Corporation (on its own behalf as co-owner and as agent for the other co-owners, Rochester Gas and Electric Corporation, Central Hudson Gas and Electric Corporation, New York State Electric and Gas Corporation, Long Island Lighting Company) to operate the Nine Mile Point Nuclear Station Unit 2 at power levels not in excess of 5 percent of rated thermal power.
Nine Mile Point 2 is a General Electric Boiling Water Reactor (BWR-5) with a Mark Il containment. The plant-rated core thermal power level is 3323 MWt corresponding to a net electrical output of 1,080 MWe.
Nine Mile Point 2 is located on the Southeast Shore of Lake Ontario, approxi-mately 6 miles Northeast of Oswego, New York, Nine Mile Point 2 is situated between Nine Mile Point 1, which is owned and operated by Niagara Mohawk, and the James A. Fitzpatrick Nuclear Power Plant, which was constructed and operated by Niagara Mohawk until its transfer to the New York Power Authority.
Initial criticality of Nine Mile Point 2 is scheduled to occur in early December.
McGuire Units 1 and 2 At about 9:00 pm on October 28, 1986, McGuire Units 1 and 2, initiated cold shutdown due to questionable torque settings for numerous (about 500 for both units, of which 251 are safety related) valve operators supplied by Rotork.
The setpoints for the Rotork valve operators had been established based upon the erroneous assumption of a linear relationship between the torque switch setting and the actual torque applied by the valve operator.
As a result of IE Bulletin 85-03 and related reviews at Oconee, Duke engineers, realizing that the torque function is non-linear, estimated that some settings may be off as much as 30%. Shutdown was initiated in accordance with TS action statement 3.0.3.
Duke estimates that the McGuire units will be down for about two weeks.
Catawba 1 and 2, which are down for other reasons, has 51 such valves which will be corrected during the current outages and in Mode 4 on Sunday. Valve settings for the total of 30 safety related valve operators have been corrected at Oconee.
An IE information notice will be issued to all licensees and applicants.
ENCLOSURE B NOVEMBER 7, 1986
Arkansas Nuclear One, Unit 1 During the current refueling outage for ANO-1, Arkansas Power and Light Company performed the 10-year inservice inspection of the "A" Reactor Coolant Pump casing. Radiography of one vertical weld in the pump casing showed a weld defect about six inches long. On comparing the radiograph with a computer enhancement of the original radiograph of this weld, the licensee determined that the weld defect has been in the pump casing since the initial operation and has not changed since that time. The licensee has also reviewed a computer enhancement of the original radiograph of the welds of RCP "B" and discovered a defect. These same defects in RCPs "A" and "B" were subsequently identified by an enhancement UT technique and were confirmed by this technique. The licensee concludes that the weld defect in RCP "B" casing has not grown or changed since the inservice date. The licensee has performed analyses on the pump casings which they maintain support the structural integrity of the pump casing. The licensee is planning on requesting relief from the ASME Code requirements to perform inspections on more pump casings.
Additionally, during an inspection of the High Pressure Injection nozzles of ANO-1, severe corrosion or wastage on the external surface of the "A" HPI nozzle due to boric acid was discovered. The inspection of the HPI nozzles was being performed in response to a comitment to perform ISI of those nozzles which had loose or damaged sleeves in 1982. The area of nozzle where the corrosion was found is in the lower quadrant of the nozzle exterior. The inside diameter of the nozzle at the corrosion is 2".
The corrosion is as much as one-inch deep and is the result of borated water which leaked from
,i the bonnet of a valve in the HPI line into the insulation and collected at the lower quadrant of the HPI nozzle area.
It has been postulated that when the borated water came in contact with the nozzle, it evaporated, leaving boron crystals which melted from the heat of the pipe and attacked the carbon steel. No other nozzles were affected. Present plans are to bring out the affected area and fill it up with weld material.
ANO-2 At 2:15 am CST on November 4,1986, the licensee began reducing power on ANO-2 to bring it to cold shutdown in order to repair a check valve in the main feedwater line to the "B" steam generator. During the evening on November 3,1986, the operators noticed an increase in sump leak rate. An entry was made into containment, the leaking valve identified, and shutdown was begun.
After the valve has been repaired, the licensee expects to begin heatup on November 8 and achieve full power by November 10, 1986.
ENCLOSURE B NOVEMBER 7, 1986
NRC TMI-2 CLEANUP PROJECT DIRECTORATE WEEKLY STATUS REPORT FOR NOVEMBER 3 - 9, 1986 1.
DEFUELING Approximately 200 lbs. of material was removed from the test pattern area following partial drilling of fuel pins. Drilling of the hard crust at the top of the remaining core material continued. Thus far 245 holes have been drilled in the crust out of a total of 421 planned for the current drilling evolution.
Preparation of canisters is in progress for the next shipment of fuel to the storage facility at the Idaho National Engineering Laboratory.

ENVIRONMENTAL MONITORING US Environmental Protection Agency (EPA) sample analysis results show that TMI site liquid effluents are in accordance with regulatory limits, NRC requirements, and The City of Lancaster Agreement.
The Lancaster water sample taken at the water works river intake and analyzed by EPA consisted of a seven day composite sample taken October 19 - 25, 1986. A gamma scan detected no reactor related activity.
TMI water samples taken by EPA at the plant discharge (includes Units 1 and 2) to the river consisted of seven daily composite samples taken from October 18 - 29, 1986. A gamma scan detected no reactor related activity.
The EPA analyses of the NRC outdoor air sample for the period October 31 - November 6,1986 showed that concentrations of Cs-137 and I-131 were below the lower limit of quantitative detection (LLD) for the system. The LLD varies around a value of IE-13 uCi/cc.

i OFFICE OF NUCLEAR MATERIAL SAFETY AND SAFEGUARDS Items of Interest Week Ending November 7, 1986 Allied Chemical Two members of the Division of Fuel Cycle and Material Safety staff are participating with members of the Region III staff in a team inspection of the Allied Facility. The purpose of the inspection is (1) verify the status of open items identified in previous inspections, (2) compare actions taken at Allied with those taken at Sequoyah, and (3) compare actions taken at Allied vis-a-vis the Lessons Learned Recommendations.
R. E. Cunningham, NMSS/FC, is scheduled to attend the close-out meeting with Allied management on November 7, 1986.

ENCLOSURE C NOVE!1BER 7, 1986
2 NMSS/IE Meeting with DOE NMSS and IE staffs will brief senior management on the DOE high level waste repository program on November 24, 1986 regarding lessons-learned from the power reactor program.
The briefing will also cover improved NRC inspection programs such as Integration Design Inspections and Construction Appraisal Teams, and possible adoption of similar programs by the DOE.
Similar briefings have been held in the past on Ford Study findings and will continue in the future to help effect transfer of reactor lessons learned to the DOE repository program.
Meeting on Borosilicate Glass Program Division of Waste Management staff has scheduled a meeting on December 9-10, 1985 with DOE and Savannah River Laboratory staff to discuss the borosilicate glass testing programs for the Defense Waste Processing Facility (DWPF) waste.
The DWPF waste processing systems, process control programs and glass leach testing and modeling programs will be discussed.
Meeting in South Carolina A November 13, 1986 meeting is being arranged in Columbia, South Carolina between representatives of NRC's Division of Waste Management, DOE, and the two operators of existing low-level waste disposal facilities:
U.S. Ecology and Chem-Nuclear Systems, Inc.
The purpose of the meeting is to discuss NRC and DOE acquisition of low-level waste shipment manifest information in an electronic format.
(NRC's efforts in this regard are being coordinated with DOE's development of a national information system pursuant to the Low-Level Radioactive Waste Policy Amendments Act.) The principal topics to be addressed at the meeting include (1) arrangements to protect proprietary portions of the data, (2) NRC and DOE data requirements, and (3) the potential l
for development of a common waste shipment manifest.
Sheffield Closure U.S. Ecology, the site operator for the Sheffield low-level waste burial facility, has submitted to the ASLB its legal arguments for summary disposition.

3 Mixed Waste A policy meeting on mixed low-level radioactive waste with EPA and the State of Washington is scheduled for November 14, 1986 in Seattle, Washington. The purpose of the meeting is to examine technical, procedural, and administrative difficulties that may arise due to initiating the Resource Conservation and Recovery Act (RCRA) permitting process at the Hanford LLW Disposal Facility, Richland, Washington.
The States of Nevada and South Carolina, which host the remaining two disposal facilities, have been invited to the meeting as observers.
Proposed FBI Criminal History Rule In response to the Staff Requirements Memorandum of October 31, 1986, staff forwarded the FBI Criminal History Rule package to SECY on November 5, 1986 for publication in the Federal Register.
Publication is expected the week of November 10-14, 1986.
Public comment period for the rule is 30 days.
Policy Statement and Guidelines on Access Authorization Final office concurrences were received on the Access Authorization Policy Statement and Guidelines on October 31, 1986.

OFFICE OF NUCLEAR REGULATORY RESEARCH Items of Interest Week Ending November 7, 1986 Revision to ECCS Rule contained in Appendix K and 10 CFR 50.46 of Part 50 The proposed revision to the ECCS Rule contained in Appendix K and 10 CFR 50.46 of Part 50 is now before the EDO for his signature. The rule revision package, which is in the form of a Commission Paper, includes the proposed rule wording, a regulatory analysis, an environmental impact statement and a regulatory guide. Upon EDO and Comission approval, the proposed rule and regulatory guide would be issued for public coment.
l Systems Analysis and Risk Assessment (SARA) System The Systems Analysis and Risk Assessment (SARA) System is under development at INEL for the purpose of developing a capability to analyze risk characterization of nuclear power plants using state-of-the-art, user friendly, and modularized computer software. The SARA system is intended to be a flexible tool to support different levels of users requiring risk and reliability information for decisionmaking and regulatory analysis.
The SARA system will be expanded to include the capability to perform sensitivity studies on the consequence models for NUREG-1150, and to assess the expected risk impact from resolving generic issues.
Demonstrations of using the SARA and Integrated Reliability and Risk Analysis System (IRRAS) were given to NRC staff, and to the EDO (and staff) and CRGR members on 28 October 1986. Reception was favorable with an emphasis on the l
need for verified NPP data. On 15 October 1986, the EDO issued a memorandum to the office Directors outlining office task assignments for implementation of the Comission Policy on Safety Goals. That memorandum identified SARA and IRRAS as key tools to enable the staff to have a direct, hands-on capability to use the NUREG-1150 risk baseline for reference analysis.
It was pointed out that these tools are to assist staff analyses of the probable effects of a proposed backfit or safety issue resolution (both individually and collectively. This is regardless of whether these actions would involve plant changes at the component, system, or severe accident sequence level. This will provide the staff with a means of measuring the effects of proposed hackfits or generic issue resolutions aaainst the Comission's safety goals.

2 Other uses of SARA and IRRAS that have been identified are: prioritization of generic issues review of PRAs and other reliability studies evaluation of plant-specific issues (e.g., operational problems, ISAP, and tech spec and procedural changes), assessment of operational experience and prioritization of safety research. With proper design, and with reliable data, these analytical tools should be able to provide assistance in all of these areas.
IPIRG Technical Advisory Meeting The first meeting of the Technical Advisory Group (TAG) of the International Piping Integrity Research Group (IPIRG) was held on November 3-4, 1986 at Battelle Columbus Laboratories. The IPIRG is a consortium of nations and organizations cooperating to conduct a series of large scale piping experi-ments that will provide a common basis for the countries to set their regulations on leak before break and a replacement criterion to the double ended guillotine break. Membership in iPIRG is set at $200K per year for three years; this entitles the country to full access to the experimental data and a vote at the TAG meetings. Japan has just returned its signed contract, and we expect similar signed contracts by the first of the year from the UK, France, and EPRI, and half-share half-vote contracts also from Taiwan, Switzerland and Sweden. Canada, Spain and Argentina have expressed strong interest, and we are in various stages of negotiations with them for member-ship.
At the organization meeting, Battelle presented detailed technical review of the proposed research program, which concentrates on seismic and dynamic loadings on large diameter cracked pipe under operating temperature and pressure. The very animated discussion revealed general acceptance with the plans, as well as some excellent suggestions for alterations in the program which all agreed would be quite beneficial. The technical program is now set, and only awaits formal signatures of five full-share members for the program to begin experimental work.
Joint EPRI/NUPEC/NRC Program on Primary Coolant Loop A meeting was held among representatives from EPRI, Brookhaven National Laboratory (BNL), Japanese Nuclear Power Engineering Test Center (NUPEC),
Mitsubishi, Hanford Engineering Development Laboratory (HEDL), and NRC at Brookhaven on November 3 and 4, 1986. The purpcse of the meeting was to discuss a joint program sponsored by EPRI, NUPEC, and NRC in which a PWR primary coolant loop will be seismically tested on the Tadotsu shaker table in Japan now scheduled for April 1988. As a result of the meeting, a modified test specimen was agreed upon that will enable us to accomplish our objective for the test which is to strain the specimen into the inelastic range so that our code methodology can be challenged. In addition, it was agreed that the piping would be pressurized during the test to 2200 psi; this pressure represents PWR primary system condition. A test under the combined seismic-pressure loading will give us a good insight into system behavior under these conditions and validate or point out deficiencies in our methodology.

4 14TH WATER PEACTOR SAFETY INFORMATION MEETING The 14th Water Reactor Safety Information Meeting was held at the National Bureau of Standards on October 27 through 31, 1986. The meeting was sponsored by the Office of Nuclear Regulatory Research, and attended by over 600 researchers from the nuclesr industry, government, and 17 foreign countries.
In addition to speeches by Chairman Zech and Victor Stello of NRC and Jean Bussac of CEA, some of the issues that were discussed during the different sessions were:
TMI-2 ANALYSES This session covered the recent results from core-drilling operations, and examinations of the lower plenum core debris, an update of in-core accident progression understandings and a review of the data bases and parameters for the TMI-2 Accident Standard Problem. Perhaps the most notable finding was the apparent fact as shown from video examinations of the holes drilled through the core that there is a solid mass in the center of the core with no void space and no central pathway at the bottom for material to pass into the lower plenum.
SEVERE ACCIDENT SEQUENCE ANALYSIS A variety of papers were presented ranging from an investigation of BWR Mark I containments for their effectiveness in retaining fission products after a severe accident, to a study of lateral separation of oxide and metal components of core debris on the drywell floor, and a study of an ATWS sequence for high pressure boiloff without operator action.

Seven papers were presented at this session on such subjects as steam separator modeling for NPPs, dryout in rod bundles, two-phase flow measurements in the upper plenum, phase separation in a hot-leg u-bend and fluid-fluid thermal mixing.
ECCS RULE REVISION The revised ECCS rule would eliminate the requirement to use Appendix K methods when performing ECCS performance calculations and allow realistic calculations, l
combined with estimates of calculational uncertainty to be used to demonstrate t
ECCS performance. The proposed rule revision and regulatory guide that would describe the rule were discussed along with discussions of experience using realistic calculations in the approval of a new GE evaluation model.

5 INTEGRAL SYSTEMS TESTING This session discussed the status of Integral Test Facilities and Future Plans for Integral Testing. A representative from JAERI presented results from six 5% SBLOCA experiments performed at ROSA IV and a representative from B&W presented results from the MIST experimental program. A representative from INEL described the approach and principal results of a scaling study which would form a basis for design of future integral facilities. Other papers included results of analyses that indicated for a station blackout that coolant sources for emergency feedwater could be exhausted.
MECHANICAL AND STRUCTURAL RESEARCH Papers presented at this session were primarily concerned with presenting the results of research related to piping system dynamic reliability analyses and failure. Detailed results of pipe seismic fragility tests and pipe damping tests were also presented.
INTERNATIONAL CODE ASSESSMENT PROGRAMS Papers presented discussed: The development of a two-phase pump model for system codes; the status of TRAC and RELAP assessment in FRG; the CSNI validation matrix; and TRAC-BWR.
NDE This session provided papers on NDE reliability and qualification, Real-time 3-D SAFT-UT, and On-Line Acoustic Emission Monitoring. Also an invited paper l
was presented on the PISC II and III programs.
7 SEVERE ACCIDENT SOURCE TERM Papers presented at this session ranged from generic implications of Chernobyl to implementation of NRC's severe accident policy. A panel discussion was held on the Source Term Code Package.
POSTER SESSION One of the highlights of the poster session held on Wedneaday, October 29, 1986 was a demonstration of the NRC Operations Center Expert System.
CONTAINMENT SYSTEMS Topics of presentations during this session included direct containment heating, hydrogen burning, steam explosions, containment integrity and research activities in Japan.

RISK ANALYSIS /PRA APPLICATIONS This session included papers on use of PRAs in regulation, risk-based performance indicators, management of generic safety issues and a status report on Safety Goal implementation.
PRESSUPE VESSEL RESEARCH Information presented at this session pointed out significant progress in the understanding of fracture initiation and crack arrest phenomena. Presentations also discussed advances made in the separate effects cladding study, i
INDUSTRY SAFETY RESEARCH A number of papers were presented summarizing industry sponsored safety research by EPRI.
In addition, an NRC paper provided a user challenge to the research community (both government and industry) to provide " complete" data or data with clear delineation of limitations.
The technical papers provided details of work culmination on specific containment and piping research, seismic hazard evaluation and probabilistic methodology for enhancing plant safety and productivity.
Status reports were given on Source Term Research, hydrogen burning and other safety and system investigation, including the impact of the unfortunate accident in the Soviet Union.
ADVANCED REACTORS The session on Innovative Concepts for Increased Safety of Advanced Reactors included presentations on containment modifications to assure low or zero releases and on the principles of using passive and inherent methods for assuring shutdown and shutdown heat removal. There was significant consensus achieved on the use of natural convection to remove shutdown heat removal, and on the need for a large scale demonstration of the capability of an advanced plant to be safely shutdown in the absence of offsite power and a number of other key initiators.
2D/30 RESEARCH At this session participants from JAERI provided a summary of CCTF and SCTFIII test results and discussed development of a reflood model.

8 RES RULEMAKING ACTIVITIES Primary Reactor Containment Leakage Testing for Water-Cooled Power Reactors (Part 50) (Appendix J)
The proposed rule updates and revises the 1973 criteria for preoperational and periodic pressure testing for leakage of primary and secondary containment boundaries of water-cooled power reactors. The revision was needed to reflect current regulatory practice and current technology which were no longer being reflected accurately by the existing rule, and endorsement in the existing regulation of an obsolete national standard that was replaced in 1981.
The Commission-approved proposed revision to Appendix J was published in the Federal Register on October 29, 1986, allowing 90 days for public comment.
Emergency Preparedness for Fuel Cycle and Other Radioactive Materials Licensees (Parts 30, 40, 70)
In 1981 the NRC issued orders to certain fuel cycle and other radioactive material licensees requiring them to submit comprehensive onsite radiological energency plans. Subsequently, rulemaking was initiated to place a requirement for emergency plans in the regulations.
The Commission has disapproved publication of the rule as proposed. A staff requirements memorandum, dated October 28, 1986, directs the staff to address more directly the need to consider and plan for any serious non-radiological hazards associated with licensed facilities and also to reflect the lessons learned from the Sequoyah accident and submit a revised proposed rule by January 30, 1986.

l ITEMS OF INTEREST OFFICE OF INTERNATIONAL PROGRAMS WEEK ENDING NOVEMBER 7, 1986 First IPIRG Organizational Meeting On Monday and Tuesday a meeting of the International Piping Integrity Research Group (IPIRG) was held at Battelle in Columbus, Ohio.
Representatives from Canada, France, Japan, Spain, Sweden, Taiwan, the United Kingdom, and EPRI participated in discussions with NRC and the Battelle contractor concerning operation of the IPIRG program and its five specific tasks. The representatives from Japan brought with them their newly-signed copy of the agreement. This is the first signed agreement that has been received by NRC.
Four more binding commitments are needed before the IPIRG can begin. These are expected around the first of the year.
Foreign Visitors On Wednesday Mr. C. C. Tung, Director of Planning for the Taiwan Atomic Energy Council, visited the Incident Response Center for a briefing and tour of the facility.

Would prohibit nuclear power plant operations until the Federal government licenses a perinanent high-level radioactive waste disposal site. A 1980 State law places a moratorium on the construction of new nuclear plants until a nuclear waste facility is in place. The Trojan Plant is the only nuclear power plant in the State.
Early indications are that this was DEFEATED with a 65-35% vote, with 57% of the precincts reporting.
Washington (Referendum #40)
Would require State officials to continue to challenge the Federal selection process for high-level nuclear waste repositories and provide a means for voter disapproval of any Washington site. This would allow the populace to object to the selection of Hanford as the first HLW repository site.
Early indications are that this was APPROVED with a 82.5-17.5%
vote, with 92% of the precincts reporting.
