, A reactive inspection was conducted on August 18-20 and September 8-11, 1986.
This inspection was in response to an occupational radiation exposure in
excess of your administrative limits. The exposure occurred during work on
< the reactor coolant recirculation pump seal in the period between July 25 and
August 7, 1986. A review of this job identified several concerns in planning
and control of this work. Also, one violation of 10 CFR Part 20 requirements
was identified. Details are provided in the enclosed Appendix A and the

attached inspection report. You are required to respond to this finding by
following the instructions included in Appendix A.
