Division of Waste Management staff met on October 23, 1986 with representatives from the Environmental frotection Agency (EPA) Department of Health and Human Services (HHS), including National Institutes of Health (NIH) and Health Care i"

Financing Administration (HCFA), Joint Commission on Accreditation of Hospitals (JCAH), and the State of Maryland to discuss the current status of the regula-tion of Radioactive 81ohazardous Waste (R8W).

Based on these discussions, the staff understands that there are no federal regulations governing R8W, and that these wastes are being regulated either by respective states or through guidance developed by HHS. This guidance is mostly general, rather than specific.

EPA does not have specific regulations on R8W, although the agency has the authority j

to regulate such wastes under the Resources Conservation Recovery Act (RCRA).

The staff continues to gather more information and plans-to develop a regulatory position for NRC on this issue.

Beatty License Renewal l

On September 25, 1986, the Director of the Nevada Department of Human Resources i

informed U.S. Ecology Inc. that the company could continue to operate the Beatty LLWDF during the formal license renewal process.

Nevada is an Agreement State and has established its own administrative procedures for ifcensing. Ac-1 cording to NRC procedures, the license would have been in timely renewal since 1980. Much of the information contained in the licensee's 1980 renewal applica-j tion is, by now, out of date.

The Itcensee was instructed in a July 1986 i

meettag, and follow-up letter from the supervisor of the Nevada Radiological l

Health section, of the minimum technical content of the amended license appli-l cation.

Since the license renewal process is likely to take a year or more, i

some interim licensing action may be warranted.

NRC staff is scheduled to meet with Nevada officials on November 20, 1986 to discuss the situation.

Alleged Falsification Concerning Quality Assurance Program i

There has been considerable publicity concerning a wanted felon (Gary McChain) l who has alleged that he has worked at five nuclear facilities under an alias.

In addition to reportedly being offered bribes to falsify welds, he reportedly

was aware of other incidents similar in nature.

It has been established that l

he has worked at fise facilities; four under construction and one operating reactor.

It has also been determined that he had quality assurance responsi-bilities and had unescorted access to vital areas.

The FBI has assumed investigative jurisdiction and 0I is also investigating.

OFFICE OF NUCLEAR REACTOR REGULATION ITEMS OF INTEREST Week Ending October 31, 1986 Seabrook, Unit 1 On October 22, 1986, at about 6 p.m. Public Service of New Hampshire began loading fuel into the Seabrook. Unit I reactor vessel. A license authorizing fuel load and precritical testing was issued on October 17, 1986.

Perry Unit No. 1 Low power testing of Perry Unit No. I was completed on October 22, 1986 and a controlled shutdown ~ commenced at 2:40 p.m. on-that date. A 2E-day maintenance outage has commenced. The licensee currently expects the unit to be ready for operation above 5 percent power by November 15, 1986.

Shearon Harris On October 24, 1986, Facility Operating (License No. NPF-53 was issued authoriz-CP&L) to conduct low power testing ing the Carolina Power & Light Company and operation of the Shearon Harris Nuclear Power Plant, Unit 1, at up to but not to exceed 5 percent of rated core power. CP&L expects to complete low power testing and be ready for full power operation in January 1987.

Braidwood Station, Unit 1 On October 17, 1986, the NRC issued a license for Braidwood Station, Unit 1 authorizing fuel loading and precritical testing. The first fuel assembly was loaded at 2022 on October 25, 1986. As of 0930 on October 27, 1986, 36 of the 193 total assemblies were loaded.

Fuel loading activities are proceeding in an orderly manner; the Resident Inspectors are witnessing the activities.

. During the current refueling outage for ANO-1. Arkansas Power and Light Company performed the 10-year inservice inspection of the "A" Reactor Coolant Pump casing. Radiography of one vertical weld in the pump casing showed a weld defect about.six inches long. On comparing the radiograph with a computer enhancement of the original radiograph of this weld, the licensee detemined that the weld defect has been in the pr.ap casing since the initial operation and has not changed since that time. The licensee has also reviewed a computer.

enhancement of the original radiograph of the welds of RCP "B" and discovered a defect. These same defects in RCPs "A" and "B" were subsequently identified by an enhancement UT technique and were confimed by this technique. The licensee concludes that the weld defect in RCP "B" casing has not grown or changed since the inservice date. The licensee has performed analyses on the 3

l pump casings which they maintain support the structural integrity of the pump casing. The licensee is planning on requesting relief from the ASME Code requirements to perform inspections on more pump casings.

Additionally, during an inspection of the High Pressure Injection nozzles of ANO-1, severe corrosion or wastage on the external surface of the "A" HPI nozzle due to boric acid was discovered. The inspection of the HPI nozzles was being performed in response to a comitment to perfom ISI of those nozzles which had loose or damaged sleeves in 1982. The area of nozzle where the corrosion was found is in the lower quadrant of the nozzle exterior. The inside diameter of the nozzle at the corrosion is.2".

The corrosion is as much as one-inch deep and is the result of borated water which leaked from

the bonnet of a valve in the HPI line into the insulation and collected at the lower quadrant of the HPI nozzle area.

It has been' postulated that when i

the borated water came in contact with the nozzle, it evaporated, leaving boron crystals which melted from the heat of the pipe and attacked the carbon

  • steel. No other nozzles were affected. Present plans are to bring out the affected area and fill it up with weld material.

NRC TMI-2 CLEANUP PROJECT DIRECTORATE WEEKLY STATUS REPORT FOR OCTOBER 27 - NOVEMBER 2, 1986 1.

DEFUELING The core drilling rig has drilled 101 holes through the hard crust at the top of the remaining core material. The test pattern area (19 holes) has been drilled down through_part of the region containing fuel pins. These-holes'end approximately 2 inches above the bottom spacer grid on the fuel assembly. Removal of the drilled fuel pin material from the test pattern area is scheduled for Monday, November 3,1986. This will check the effectiveness of drilling fuel pins as a defueling technique.

ENVIRONMENTAL MONITORING US Environmental Protection Agency (EPA) sample analysis results show that TMI site liquid effluents are in accordance with regulatory limits, NRC requirements, and The City of Lancaster Agreement.

The Lancaster water sample taken at the water works river intake and analyzed by EPA consisted of a seven day composite sample taken October 12 - 18, 1986. A gamma scan detected no reactor related activity.

TMI water samples taken by EPA at the plant discharge (includes Units 1 and 2) to the river consisted of seven daily composite samples taken from October 11_- 18, 1986.

A~ gamma scan detected no reactor related activity.

The EPA analyses of the NRC outdoor air sample for the period October 23 -'31, 1986 showed that concentrations of Cs-137 and I-131 were below the lower limit of quantitative detection (LLD) for the system. The LLD varies around a value of IE-13 uCi/cc.

AUXILIARY AND FUEL HANDLING BUILDING ACTIVITIES Decontamination of the seal injection cubicles continues.

Reclamation of clogged Defueling Water Cleanup System-(DWCS) filter canisters by flushing with hot boric acid is in progress.

Modifications are in progress to the fuel handling building (FHB) ventilation system. These modifications are in support of the installation of an engineered safety feature grade FHB ventilation system for TMI-1 as mandated by the TMI-1 restart license conditions.

j OFFICE OF NUCLEAR MATERIAL SAFETY AND SAFEGUARDS Items of Interest Week Ending October 31, 1986 Alleged Falsification Concerning Quality Assurance Program There has been considerable publicity concerning a wanted felon (Gary McChain) who has alleged that he has worked at five nuclear facilities under an alias.

In addition to reportedly being offered bribes to falsify welds, he reportedly was aware of other incidents similar in nature.

It has been established that he has worked at five facilities; four under construction and one operating reactor.

It has also been determined that he had quality assurance responsi-bilities and had unescorted access to vital areas.

The FBI has assumed investigative jurisdiction and OI is also investigating.

NMSS and IE are actively assisting in this investigation.

Spent Fuel Shipment Delay A spent fuel shipment from Monticello, Minnesota to Morris, Illinois was de-layed approximately two days as a result of tampering with the railway track between Monticello and St. Paul, Minnesota. The incident began on October 27, 1986, with an early morning call to a local television station, alleging that a portion of track on a 50 mile spur connecting Monticello and St. Paul had been tampered with.

A similar call was made to the local police at approxi-mately the same time. At 9:15 am, the crew of a train observed and removed from the track a barricade described as a "saw-horse" covered with a piece of cloth containing the words "no more rad waste." The train proceeded slowly.

A short time later, the engine was derailed as a result of a section g

of the track having been removed.

The scheduled shipment was-put on hold until assurances were received that it was safe to proceed. ~Burlington Northern provided a lead engine to precede the spent fuel shipment train to f

assure all was in order. The shipment was further delayed due to the Governor of Minnesota requesting more information on the incident.

The matter was resolved between Northern State Power and the railroad authority. The ship-ment began at 8:40 am CST on October 29th and arrived at approximately 3:00 am, October 30th.

The FBI is investigating this incident.

NRC Region I Physical Protection Seminar Members of the Division of Safeguards participcted on October 29, 1986 in Region I's licensee seminar on physical protection.

Mr. Robert Burnett presented an overview talk on NRC's physical protection program for reactors.

Other subjects covered by Division personnel were RER program; NRC/ FBI / DOE I

support for contingency threats, safety / safeguards interface and backfit program, and power reactor regulatory initiatives.

Attendees (approximately 130) at the meeting included representatives from NRC Headquarters, Region I, Region I licensees, and licensee contractors.

2 Babcock & Wilcox, Commercial Nuclear Fuel Plant (CNFP)

On October 23, 1986, the CNFP Plant Manager, and members of his staff, met with members of the NMSS staff to discuss their new organization and the mini-mum qualifications of key safety-related personnel.

The CNFP will prepare an amendment application incorporating the new organization and the qualifications of its personnel.

EXXON Nuclear Company, Inc.

On October 28, 1986, the Office of Nuclear Material Safety and Safeguards gave consent to the pending transfer of controlling interest in Exxon Nuclear, a Delaware Corporation, to Kraftwerk Union AG (KWU).

KWU is incorporated in the Federal Republic of Germany and is a wholly-owned subsidiary of Siemans AG.

MOLYCORP On October 23, 1986, members of the Division of Fuel Cycle and Material Safety (FC) staff met with Molycorp' representatives to discuss the possible reworking of residues generated at the York, Pennsylvania site.

These residues were generated processing licensable ores to recover rare earths and other non-radioactive metallic compounds.

The processing will recover usable rare earth products to be shipped to a Molycorp mill in California for further processing.

The processing will concentrate the thorium and uranium thereby reducing the volume of low level waste for burial.

GA Technologies, Inc.

A letter dated October 2, 1986, informed th'e NRC that on September 30, 1986, all the issued and outstanding shares of GA Technologies, Inc., (GA) were sold by Chevron U.S.A., Inc., and transferred to a new holding company, General Atomic Technologies Corporation.

Since the name of the licensee remains GA Technologies,-Inc., all NRC licenses currently held or applied for by GA (reactors, special nuclear materials, byproduct devices, certiti-cates of compliance, export and import), remain unchanged.

Dry Spent Fuel Storage Staff members from NMSS and personnel from Lawrence Livermore National Laboratory (LLNL), which is providing technical assistance on safety reviews of dry spent fuel storage cask designs, met with representatives of Combustion Engineering and Westinghouse on October 23 and 24, respectively, to discuss the firms' responses to staff comments and questions on topical reports sub-mitted for review.

Both topical reports are for cask designs using thick-walled ferritic steel with external solid resin neutron shields.

Both casks are designed to store 24 PWR assemblies, and the Combustion Engineering design includes a proposed fuel basket design that would provide for storage of 60 BWR assemblies.

3 Reviews of the topical reports are at the same stage. The applicants have pro-posed responses to the staff's questions and comments following initial review of the topical reports.

These proposed responses were discussed in detail at the meetings and, following resolution of all issues, the topical reports will be revised accordingly.

In particular, issues related to fuel basket designs require further consideration by the firms for further staff review.

Proposed Meeting with State of Washington NRC and EPA representatives will meet with Washington State representatives on the application of the Resource Conservation Recovery Act standards and how to develop procedures that will best apply these standards to low-level radio-active waste disposal facilities.

Beatty License Renewal O

On September 25, 1986, the Director of the Nevada Department of Human Resources informed U.S. Ecology, Inc. that the company could continue to operate the Beatty LLWDF during the. formal license renewal process.

Nevada is an Agreement State and has established its own administrative procedures for licensing.

Ac-cording to NRC procedures, the license would have been in timely renewal since 1980. Much of the information contained in the licensee's 1980 renewal applica-tion is, by now, out of date.

The licensee was instructed in a July 1986 meeting, and follow-up letter from the supervisor of the Nevada Radiological Health section, of the minimum technical content of the amended license appli-cation.

Since the license renewal process is likely to take a year or more, some interim licensing action may be warranted.

NRC staff is scheduled to meet with Nevada officials on November 20, 1986 to discuss the situation.

Coordination with State of South Carolina On October 24, 1986, Waste Management personnel met with a representative of South Carolina's Department of Health and Environmental Control (DHEC) in Columbia, SC.

The primary purpose of the meeting was to coordinate with the State on renewal of NRC's Special Nuclear Material license for the Barnwell OCTOBER 31, 1986 ENCLOSURE C

4 Low-Level Waste Disposal Facility. Also discussed.were the objectives of the renewal process, environmental monitoring and the need for additional NRC-DEHC coordination on LLW matters. Although the State had no immediate need for technical assistance from NRC, the attendees agreed to meet again after the first of the year to discuss in more detail activities of the two agencies.

EPA Location Standards On October 24,1986,'the WM staff participated in the first meeting of EPA's RCRA Location Standards Work Group.

EPA convened this group of EPA, States, NRC and USGS representatives to identify and recommend policy options for the development of " acceptable location standards" under the Resource Conser-vation and Recovery Act, as amended.

NRC's interest in the development of these standards resulted from interactions with EPA about location standards for the disposal of Mixed LLW.

The WM staff recognizes that EPA's development of the location standards may have programmatic implications beyond Mixed LLW disposal, including waste treatment, management, storage, and disposal at fuel cycle facilities and nuclear power reactors. WM plans to coordinate agency, comments by using the " Groundwater Protection Working Group."

Week Ending October 31, 1986 Direct Containment Heating (DCH)

The DCH program at Sandia National Laboratory includes a series of tests in a large steel vessel named SURTSEY. This vessel simulates a reactor containment volume and is used to investigate containment heating and pressurization that would result from the high pressure ejection of core melt from the reactor pressure vessel in a severe accident. The second test in the series, DCH-2, was conducted on October 22, 1986. The test involved 77 kg of a thermitically prepared iron and alumina melt plus 3 kg of fission product simulants. The melt generator was charged with nitrogen at an initial pressure of 500 psig (3.4MPa). The concrete test cavity was a 1:10 scale model of a portion of the Zion plant.

atmosphere occurred and that' aerosols were generated.

Inspection of the chamber interior showed aerosol covering virtually all exposed surfaces.

Solidified melt'in the form of a thin crust (less than-one cm) was detected on several areas of the chamber wall. Following retrieval of the in-chamber instrumentation devices, the debris will be removed and weighed. Processing of the test data is in progress. Preliminary results should be available in November 1986.

Hydrogeologic Performance Program for Reactor Siting On October 28, 1986, Tom Nicholson, WMB, DES, RES, met with Antonio Vela, Spanish Nuclear Regulatory Commission, Madrid, to discuss setting up a hydroceologic performance program for reactor siting as part of the USNRC/Spanish Governments' cooperative agreement. The discussions included

'both licensing guidance and accomplished research studies (PNL's study on ground-water interdictive strategy) as the technical basis for the discussion.

Mr. Vela requested a list of NUREG reports and technical references used in the discussion which was provided. He also expressed an interest in continuing the l

cooperative exchange which may involve sending NRC staff or other Federal agencies' experts to Spain 'to aid Mr. Vela in establishing the hydrogeologic siting review program.

2 Fourteenth Water Reactor Safety Information Meeting The Fourteenth Water Reactor Safety Information Meeting was held October 27-31, 1986 at the National Bureau of Standards. This ye'ar's meeting was kicked off by speeches by NRC's Chairman Zech, EDO Victor Stello, Research Director Eric Beckjord and the Research Director of France's CEA, Jean Bussac.

Over 500 persons attended the meeting. Attendees were from NRC, the DOE Labs, contractors, utilities, industry groups and 19 foreign countries. Over 150 papers were presented at 27 distinct sessions covering various reactor safety issues. A poster session was also presented by the Office of Inspection and Enforcenent.

The meeting is held annually to exchange reactor safety information and research and was broadened ins cope this year to embrace other than research issues. The meeting was culminated by a panel discussion on regulatory issues by the Deputy Directors of NRR, NMSS and IE, Richard Vollmer, Don Mausshardt, and Richard Starostecki.

Proceedings will be published by January 1987.

ITEMS OF INTEREST OFFICE OF INTERNATIONAL PROGRAMS WEEK ENDING OCTOBER 31, 1986 Meeting of Interagency Task Force for Nuclear Exports The Interagency Task Force for Nuclear Exports met on Tuesday to discuss the nuclear industry post-Chernobyl and to receive an update on a proposed nuclear technical sales seminar to be held in the USSR and Eastern Europe sometime in 1987. The post-Chernobyl briefing was given by Arrbassador Richard T. Kennedy.

Foreign Visitors On Wednesday representatives of three Dutch nuclear organizations visited NRC. George Vayssier of the Ministry of Social Affairs and Employment (the Dutch nuclear licensing authority), Jan Speelman of the research establishment ECN-Petten, and Paul Kloeg of KEMA (a utility organization equivalent to EPRI) met with NRR personnel to discuss safety related evaluations and possible plant modifications which the Dutch have decided to undertake in the aftermath of the Chernobyl accident. The areas under investigation include:

1) reactivity induced accidents, 2) fire protection issues, 3) human factors aspects, and
4) containment performance and possible modifications. The discussions with NRR focused on the last topic. The visitors noted that an earlier decision by the Dutch government to expand nuclear capacity by at least two, and possibly as many as four, nuclear plants has been put on indefinite hold as a result of the Chernobyl accident.

This week Mr. Luis Echavarri, Technical Director of the Spanish Nuclear Safety Council (CSN), and Mr. Jose Villadoniga, Director of the CSN's Division of Operating PWRs, met with the EDO and staff representatives of NRR, IE, RES, and IP to discuss a variety of regulatory / safety issues and the status of NRC/CSN bilateral activities. Prior to these discussions Srs.- Echavarri and Villadoniga met with senior staff representatives of NRC's Region II office in Atlanta and toured the Technical Training Center in Chattanooga.

On Friday Mr. K. N. Tusch, President of Colebrand, Ltd., a British firm which has developed a microprocessor-based inspection system for examining the degree of corrosion in steel reinforcing bars embedded in concrete, outlined the functienal capabilities of his company's system
