Inspection Report No. 50-293/86-21
This refers to the routine safety inspection (50-293/86-21) conducted by
Dr. M. McBride and others of this office during the period June 2, 1986 to
July 7, 1986 at the Pilgrim Nuclear Power Station, Plymouth, Massachusetts.
Areas examined during this inspection are described in the NRC Region I In-
spection Report which is enclosed with this letter. Within these areas, the
. inspection consisted of selective examinations of procedures and records, in-
terviews with personnel, and observations by the inspector.
We are concerned about several deficiencies that are identified in the enclosed
report. These include (1) the recurring failure of the recirculation motor
generator set field breaker used for anticipated transient without scram pro-
tection, (2) the apparent failure to properly schedule required surveillance
tests, (3) the completeness and technical adequacy of logic functional sur-
veillance tests, (4) excessive leakage pest primary containment isolation valves
in excess of established criteria, and (5) the extent of fire brigade training
at the station. We believe that these issues are important and we plan to re-
view them prior to startup.
Please submit, within thirty days, a schedule for
the completion of your evaluations and corrective actions with regard to these
five items so that we can complete our reviews in a timely manner.
Also, based on the results of this inspection, it appears that one of your
activities concerning safety related battery testing was not conducted in full
compliance with NRC requirements, as set forth in the Notice of Violation, en-
closed herewith as Appendix A.
This violation has been categorized by severity

level in accordance with the General Statement of Policy and Procedure for NRC
Enforcement Actions,10 CFR Part 2, Appendix C (Enforcement Policy 1986). You
are required to respond to this letter and in preparing your response, you
should follow the instructions in Appendix A.
The responses directed by this letter and the accompanying Notice are not
subject to the clearance procedures of the Office of Management and Budget as
required by the Paperwork Reduction Act of 1980, PL 96-511.
