Reported Violation:
Technical Specification 6.8.1.a and Regulatory Guide 1.33',
Appendix A, Paragraph 8 require implementation'of surveillance and test procedures.
Contrary to the 'above, test procedure 1-9102-S-02 and surveillance procedure'OST-1026 in use on December 29, 1986, did not adequately account for changes in the measured parameters leading to errors in the sign and magnitude of leakage from the Reactor Coolant System.
This is a Severity Level IV violation (Supplement I).
Denial or Admission and Reason for The Violation:

The violation is admitted. Three errors were found in the leak rate calculation for reactor coolant leakage. The calculations are shown on Attachment II of OST-1026 (Reactor Coolant System' Leakage Evaluation Daily Interval Modes 1-2-3-4) and Data Sheet 10.2 of.1-9102-S-02'(Reactor Coolant System Leak Rate Test). The identified errors were:
(1) The pressurizer volume calculation did not correctly account for density change of the coolant from pressurizer temperature / pressure conditions to atmospheric conditions, (2) The RCS volume changes due to T change calculation did not correctly account for densi$y change of the 8
coolant from RCS temperature / pressure conditions to atmospheric conditions, and (3) The identified leakage rate determined from EST-204 (Reactor Coolant System Isolation Valve Leak Test),
although conservative for the purpose of that surveillance test, was a non-conservative correction to OST-1026.
i Corrective Steps Taken and Results Achieved:
(1) The pressurizer volume change calculation (Step 4.5 of OST-1026, Attachment II) has been revised to include the appropriate density correction factor.
(2) The RCS volume change due to T,yg change calculation (Step 4.6 of OST-1026) has been revised to include a density correction factor obtained from a new 3
correction factor table and graph included in OST-1026 as Attachment III.
(3) The identified leakage rate determined from EST-204 (Step 3.2 of OST-1026) has been deleted from the calculation. These corrections were included in OST-1026, Rev. 2, by advance changes 2/1 and 2/2. OST-1026 is performed daily. The results obtained have been consistently within acceptance criteria and have shown " positive" out leakage which is the expected direction-of leakage.
