't SQNP AI-5 Page 1 Revision 36 SHIFT AND RELIEF TURNOVER 1.0 PtTRPOSE (Ref. NUREG-0578, IE Notice 83-027)
To provide instructions for distribution of shift employee schedules (bulletined hours of work) and requirements of personnel during periods of relief or shift turnover.
To provide critical valve checklist for indication that some improper valve positioning and instrumentation irregularities may have involved deliberate acts directed against plant equipment in vital areas.
Routine shift work will be performed by persons such as op-erators, Shift Technical Advisors (STA), Health Physics (HP)
Technicians, Chemical Laboratory Analysts and Public Safety Officers.
Non-routine shift work may be required for some maintenance activities or testing. These activities may require shift work by personnel from onsite maintenance, TVA test engineers, off-site maintenance, or equipment vendor representatives.
Foremen of crafts on back-to-back shifts shall maintain a checklist on activities associated with safety-related eq-uipment on which they are involved.
The primary purpose of this checklist is to ensure that the oncoming shift is know-ledgeable of the activity in progress during the preceding shift.
To provide instructions to the Senior Reactor Operator (SRO) when deliberate acts directed against plant equipment may have taken place.
2.0 ROUTINE SHIFT WORK 2.1 Schedule distribution and changes:
Copies of routine employee shift work schedules shall be distributed to the Shift Engineer (SE) and Public Safety Services (PSS). These may be used to identify duty personnel for the SE and provide information to PSS as to who is scheduled to be on site on back shifts and holidays.
2.2 Shift Relief 2.2.1 The individual being relieved is responsible for passing on all pertinent information concerning buldA/vlf

He shall bring particular attention to ab-normalities which have occured or exist and significant journal entries.
He shall retain the responsibilities of the job until he has fully informed his relief of the status of all equipment under his jurisdiction.
2.2.2 persons assigned to jobs shall remain on them until properly relieved by someone of equal or higher classification or released by his sup-ervisor.
2.2.3 Temporary relief by operations personnel or STAS during an assigned shift may be required.
However, these reliefs shall be limited in duration and frequency to that of absolute necessity.
When temporary relief is necessary, the person being relieved shall acquaint the oncoming person with job information such as any abnormal or unusual conditions existing, any actions anticipated during his absence, and where he may be reached in the plant during his absence.
Temporary relief as described here does not require' completions of Appendix A and B.1, B.l.A, B.2, or B.2.A.
Addit.ionally, personnel shall log in the journal the time that he was relieved and the person's name that relieved him.
The person relieving shall enter his initials in the journal upon assuming responsibility.
For personnel who have reviewed the system status checklists for that unit need only verbal exchange for temporary shift relief.
DEFINITION OF TEMPORARY SHIFT RELIEF Temporary shift relief is required for following examples:
A.
Unit Operator (UO) who performed shift relief per AI-5 at the beginning of a shift on Unit 1 is requested to relieve a UO on Unit 2 during the assigned shift.

The Assistant Shift Engineer (ASE) (SRO) who performed shif t relief per AI-5 at the beginning of the shift on Unit 1 is requested to assume lead operator or ba'ance of plant (BOP) position t

Temporary shift relief is not required for the following examples:
NOTE:
Unit 1 is used as example - same instructions apply to Unit 2.
A.
A UO who performed shift relief per AI-5 at the beginning of the shift on Unit 1 as BOP is requested to assume lead operator position on Unit 1 during the assigned shift.
B.
A UO who performed shift relief per AI-5 at the beginning of the shift on Unit 1 as lead operator is requested to assume BOP position on Unit 1 during the assigned shift.
C.
The licensed BOP UO on Unit 1 that reviewed the Unit 2 BOP Appendix B2 is requested to assume the Unit 2 BOP position for an emergency during the assigned shift.
D.
The ASE- (SRO) who performed shif t relief per AI-5 at the beginning of the shift on Unit 1 is requested to assume lead operator or BOP position on Unit 1 during the assigned shift.
2.2.4 Operators (all classifications) and STA's--Transfer of Authority and Responsibility:
Oncoming operating personnel shall be res-ponsible to acquaint themselves with the eq-(
uipment status and any activities under their jurisdiction before assuming the duty for the shift.
As a minimum it shall include reviewing the journal entries back to his last shift worked or back five (5) calendar days (7 days for STA's), whichever is less; observance of control boards, alarm panels, etc.;
determination of plant status as related to Technical Specifications (TS) as applicable 0018A/vlf

Information to be passed to the oncoming shift should be prepared beforehand to assist them in familarization.
Documentation of transfer of authority and responsibility shall be by completing a form similar to Appendix A1, A2, or A3.
The oncoming lead UO shall complere Appendix B1 or BlA prior to relieving the off-going lead UO.
The oncoming BOP UO shall complete Appendix B2 or 82A prior to relieving the off-going BOP UO.
If the unit is staffed with an unlicensed BOP UO, the other unit's licensed BOP Reactor Operator (RO) will review and sign the unlicensed BOP UO's Appendix B-2.
(This will allow the BOP RO to assume duties during an emergency without having to do a temporary shift relief and will ensure that both units will have two RO's with knowledge of the unit's status when in modes 1 through 4.
The oncoming lead operator shall review and sign Appendix B2 or B2A.
The oncoming BOP UO shall review and sign Appendix B1 or BlA.
The on-coming ASE.shall verify completion by his signature on Appendix B1 or BlA and Appendix B2 or B2A. Appendix B1 and B2 are to be used for modes 1-4, and Appendix BlA and B2A are to be used for modes 5-6.
Should suspected tampering discrepancies be found, follow guidelines in Appendix D.,
Appendix C, Critical Valve Summary Checklist, shall be completed during each shift to verify proper valve alignment on each safety related system applicable to the unit's mode.
Valve position should be verified on the (11-7) shift by visual or hands on inspection, however, valve position may be verified on the (7-3) and (3-11) shifts by status file review, configuration log review and/or visual check and documented on the Critical Valve Summary Checklist.
(Visual check is physically verifying valve position at the valve locally or by main control room indicating lights.)

During each shift, the status of the safety related systems in the status file shall be reviewed as follows:

Review configuration log and complete Appendix C of AI-5.

Review status file if items have been transferred from the configuration log to the status file.

Review only the deviation sheet (s) for those identified in (a) above.
In order to ensure that Operations personnel and STA's are cognizant of new abnormal and em-ergency in~structions and changes to existing abnormal and emergency instructions prior to assuming shift responsibilities, the in-structions of Operations Section Letter OSL T1 (section 11) shall be followed and documented by the oncoming persons signature on Appendix A.
The completed Appendixes A, B1, BIA, B2, B2A, and C forms will be forwarded to the Operations Supervisor's office where they shall be reviewed by the power plant Operations Supervisor or his assistant.
They will be held in the Operations Supervisor's office files for one month fol-lowing review.
Appendixes A, B 1, BlA, B2, B2A, and C will be initialed by the Operations Supervisor or his assistant at the top of the first page following review.

During his absense while the unit is in mode 1, 2, 3, or 4 an individual (other than the STA) with a valid SRO license shall be designated to assume the control room command function.
During any absence of the shift supervisor from the control room while both units are in mode 5 or 6 an individual with a valid SRO or RO license (other than the STA) shall be designated to assume the control room command function.
Any time either unit is in modes 1-4 only one SRO is required in the control room and he must be the shift supervisor or the designated shift sup-ervisor.
2.2.6 Chemical Laboratory Analyst - Oncoming persons shall acquaint themselves with the operating status of the plant; any abnormal or unusual conditions existing; and any special in-structions from the Engineering Section Supervisor or the SE.
2.2.7 Assistant Unit Operators (AUO) shall maintain logs on any equipment under maintenance or test that could degrade a system critical to plant safety.

2.3 Distinctive Identificati'on of On-Shift Operations Personnel A shift manning chart will be displayed in each control room.
The chart will identify the SS, ASE and UO assignments by unit and will be updated each shift.
Identification badges shall be-provided to'oe worn by the on-shift SE, ASE, and UO.
These badges will be of distinctive colors and of. sufficient size to facilitate ready identification of these individuals.
The SE clerk is responsible for updating the manning chart and providing the ID badges each shift.
3.0 NONROUTINE SHIFT WORK All nonroutine shift work shall be scheduled on the daily schedule.
The nature of the work and estimated duration will be thoroughly discussed.
The Operations Supervisor is respon-sible to familiarize the SE with the planned activities.
The oncoming supervisor of work to be done will verify crew relief and acquaint himself with preceding ' shift activities.
Before commencing work, unless work or testing is in progress, he shall report to the SE to discuss the planned activities for the shift and report and schedule changes.
He will maintain close communication with the SE during the shift and report any abnormalities, or changes in the planned shift activities.
Particular attention will be directed to any activities that could affect an operating unit.
4.0 ANNUAL REVIEW OF SHIFT AND RELIEF TURNOVER Shift and relief turnover (AI-5) fulfills NUREG-0578 and IE Notice 83-027 and requires that an annual review of its effectiveness be made by an independent review group at l

II. The 2300-0700 shift should verify proper valve position per visual or hand.s on inspection.

The 0700-1500 shift and 1500-2300 shift operators may use the unit i

One shift per day shall perform a visual or hands on verification of position.
NOTE:
If a locking device was unlocked to verify position a double person verification of valve position and reinstallation of locking devices is required. This double verification satisfies the requirements of SI-186, therefore a conditional performance of SI-186 is not required.
NOTE:

SQNP AI-5 Page 72 Revision 36 APPENDIX D SUSPECTED TAMPERING OR DELIBERATE ACTS GUIDELINES FOR OPERATIONS PERSONNEL In the event that tampering or deliberate acts are suspected against plant equipment or plant safety, the SE and/or unit SRO should follow the guidelines listed below to make a decision on whether or not continued operation is justified and whether or not systems necessary for safe operation and shutdown are operable.
Examples of tampering are chains and/or locks cut and/or valves mispositioned, power removed from vital equipment or any deliberate acts performed by plant personnel against plant equipment or plant safety.
GUIDELINES TO FOLLOW NOTE:
SE/SRO to determine which guidelines apply.
1.

2.
Verify affected systems alignment (may use SOI's, SI-186 or AI-5

Appendix C).
3.
Verify affected systems power supplies.
4.
Verify affected systems control logic (instrument and electrical shops may be called upon to aide in determining systems operating logic has not been affected).
5.
Interrelated systems and selected safety-related electrical panels and cabinets, both in the plant and in the control room, may require a detailed inspection.
6.
Make a decision on whether or not continued operation is justified and whether or not systems necessary for safe operation and/or shutdown are operable.
7.
Notify plant duty supervisor.
8.
Notify PSS (PHYSI-26).
9.
Follow guideline in AI-18 on Reportability to NRC.
10.
Follow guidelines in SQNP-REP-IP-1 on implementing the REP and reportability to duty specialist.
11.
Obtain written statements from involved personnel and attach to Data Sheet Appendix D.1.
