1.0 PURPOSE t-To identify classifications required to maintain daily journals, information that should be entered, and to 73 provide guidelines for quality of journals to be t maintained.

, 3.0 INSTRUCTIONS Ed 3.1 The following work stations / classifications shall maintain a daily journal on their assigned tours of 9 duty.

(a) Shift Engineer (SE) ,

the Senior Reactor Operator (SRO), Lead Operator, and BOP operator.

Action Log" (Al-6 Appendix A). The

Journals shall include date and name(s) of persons on duty.

,j Journals shall be made in duplicate and the original forwarded to supervisors each' day except for Sundays and holidays.
