NRC INSPECTION REPORT NO. 50-373/97015(DRP); 50-374/97015(DRP) AND
Dear Mr. Subalusky:
On September 12,1997, the NRC completed a specialinspection at the LaSalle facility. The
purpose of the inspection was to review the circtimstances related to the September 4,1997,
.
event involving the inoperability of required emergency core cooling systems during outage
maintenance activities on Unit 1. The inspectors examined the plant equipment and personnel
response to the event. During the event, no operational safety limits were approached or
exceeded.
The inspectors determined that the event's immediate safety consequence was minimal since
redundant sptems were available for decay hr.at removal and core coverage. However, the
team concluded that the matter was of regulatory significance since the focus on shutdown risk
appeared to come at the expense of a sharp focus on compliance with the Technical
Specification requirements.
Based on the results of this inspection, the NRC dettemined that a violation of NRC requirements
occurred. The violation is cited in the enclosed Notice of Violation (Notice) and the
circumstances surrounding it are described in detailla the inspection report. The violation is of
concem because it involved the technicalinoperability of all required emergency core cooling
system equipment and the subsequent inabirity of the operating staff to comply with the
Technical Specification required actions.
You are required to respond to this letter and should follow the nstructions specified in the
enciosed Notice when preparing your response. The NRC will use your response, in part, to
determine whether furter enforcement action is necessary to ensure compliance with regulatory

requirements. When you respond to this letter, the NRC requests that you include the results of
your root cause investigation documenting the reasons that contnbuted to the occurrence of this
situation,
