This refers to the inspection conducted by Mr. S. A; Richards and Mr. G. 'C.

Kellund of this office on April 29 - June 16, 1986 of activities. authorized by'

NRC License No. NPF-1, and to the discussion of our findings held by Mr.

Richards and Mr. Kellund with Mr. Orser and other members of your staff

periodically during the inspection.

Areas examined during this inspection are described in the enclosed inspection

report. Within these areas, the inspection consisted of selective

examinations of procedures and representative records, interviews with

. personnel, and observations by the inspectors.

No violations of NRS requirements were identified within the. scope of this

,

inspection.

As discussed in paragraph 7 of this report, we considered your staff's initial

'

{

actions to resolve- the issues surrounding the:failu~re of: steam generator

-

~ hydraulic snubbers- and resulting restrained thermal growth' of the reactor.

_ coolant system to be inadequate. Substantial interaction between the NRC

staff.and your staff was necessary to resolve this issue. Recent deficiencies

[

identified with the control room emergency ventilation system and the

!

containment spray chemical addition system serve to further reinforce the

- necessity of aggressively pursuing apparent deficiencies .until a firm

understanding of the root cause of problems is-' gained. Your letter of June

i

'

20, 1986,- in which you describe' actions being implemented to improve

performance in this area, appears to be a step in the right direction. We

encourage you to ensure -that top levels of management become active

participants in the actions-you have stated in your letter. We anticipate

,

discussing these actions with you in the near future.

,

~In accordance with 10 CFR 2.790(a), a copy of this letter and the enclosures

will be placed in the NRC Public Document Room.
