SUBJECT:

ACTION PLAN TO ADDRESS ALLEGATIONS RAISE 0 BY MONROE COUNTY COMISSIONER (AMS N05. RIII 85-A-0165, RIII 86-A-0005, RIII 86-A-0009, AND RIII 85-A-0010) PERTAINING TO THE FERMI 2 SITE.

During the present Fermi 2 facility shutdown, a number of allegations have been received via telephone conversations, meetings, and written correspondence.

A majority of the allegations have been received from or through one of the Monroe County Commissioners.

The Region III Allegation board comprised of Messrs. E. Greenman, C. Weil, J. Harrison, and G. Wright met on two occasions and have developed the attached Action Plan for dealing with each allegation.

For ease of identification, each allegation has been addressed in a separate enclosure as identified in the composite index.

ENCLCSURE 1 AMS No. RIII 85-A-0165 Prime Responsibility: DRP Potential Followup: DRS Allegation:

Pipe fitter was concerned with a " block anchoring pipes" in the east end of the steam tunnel, and the block was cracking.

Pipefitter also stated the block had been " doctored up with stainless steel."

4 Action:

Mr. Kuron refused to identify pipefitter, but stated on September 30, 1985, he would attempt to obtain further information.

E. G. Greenman will contact Mr. Kuron to determine whether further information is available.

Decision on further action will be based on the results of E. Greenman's queries.

ENCLOSURE 2 AMS No. RIII 85-A-0165 AMS No. RIII 86-A-0005 Prime Responsibility:

DRP Allegation:

According to Kuron, he informed Detroit Edison of problems with the turbine about three years ago.

Allegation RIII 86-A-0005 also deals with Reactor Feedpump turbine failure and will be handled as part of Mr. Kuron's input.

Action:

Region III through IE Headquarters has arranged via Parameters Incorporated for an individual from NAEC to review and evaluate the entire history of the South Reactor Feedpump turbine.

The c

individual started the inspection on January 21,1986, and completed the inspection on February 5,1986.

Further Region,III action will be based on the findings of the special inspection.

AMS No. RIII 85-A-0165 Prime Responsibility:

DRS Allegation:

Fab shop was responsible for cutting a pipe at a 45' angle.

However, "the kid cutting the pipe was eating a pizza and let it go to...." Pipe repaired.

Installation was so bad that two, 25 ton bottle jacks had to be used to move the pipe into place.

(Previously identified to NRC).

Action:

With the exception of the fitup question, this item was addressed as Allegation 12 in Inspection Report No. 341/79-04 and Paragraph 3 of Inspection Report No. 341/79-26.

DRS to review the findings and conclusions presented in Inspection Reports 341/79-04 and 341/79-26.

After comparing the two reports, determine whether the allegation is adequately addressed.

If not adequately addressed, re-inspection may be required.

Allegation:

Action:

Specific issue appears to have been dealt with as Allegation "-

No. 9 in Inspection Report No. 341/79-04.

Results indicated ~

piping was nonsafety-related and as such "not a construction safety issue." DRS to review Inspection Reports dealing with weld rod control.

Determination of further action will be made after completion of above review.

ENCLOSURE 5 AMS No. RIII 85-A-0165 Prime Responsibility:

DRP/DRS Allegation:

Qualified welder at Fermi 2 failed welder qualification test of the Monroe Station fossil plant.

Kuron contends that nuclear qualified individual should have been able to pass qualification for fossil unit.

(According to Kuron, this item previously identified to NRC.)

Action:

E. G. Greenman to contact F. Kuron for name of individual alleged to have failed qualification test.

DRS to identify and review previous inspection reports dealing with welder qualification.

Further Region III action dependent on results of personal contact and report review.

ENCLOSURE 6 AMS No. RIII 85-A-0165 Prime Responsibility:

DRS/DRSS Allegation:

Fermi sacrificial shield was delaminating.

(Previously identified to NRC.)

Action:

E. G. Greenman contacted F. Kuron and determined that Kuron is simply restating a previous concern which was addressed as Allegation 16 in Inspection Report No. 341/79-04.

However Kuron's concern at this time is more related to potential radiation exposures.

DRS will review the closeout of violation in Inspection Report No. 341/79-04, which dealt with voids in the shield.

DRSS will determine whether the licensee has a drywell radiation monitoring program and provide a copy and staff evaluation thereof.

Further action by Region III will be based on results of above review.

Prime Responsibility:

DRS Allegation:

" Steam whips on the second floor at the headers had torn out of the embeds." Allegation No.15 in Inspection Report No. 341/79-04 addressed problems with installation of " Red Head" concrete anchors and assigned an unresolved item to Deco's action plan.

Unresolved item apparently never followed up.

Action:

DRS will review unresolved item 341/79-04-03 to deterzine if the item was closed.

If item is still open DRS will take action to resolve.

DRS will also review previous inspection reports dealing with expansion anchors to determine whether all previous concerns were prcperly evaluated.

Additionally, Region III will perform ultrasonic tests on a random sample of expansion anchors to determine embedment depth.

ENCLOSURE 8 AMS No. RIII 86-A-0010 Prime Responsibility:

DRP Allegation:

There is cracking in one primary containment hatch and in the torus shell.

Action:

DRP will review latest local leak rate tests on containment hatches to determine acceptability of tests.

Fermi 2 performed the containment integrated leak rate test in November / December 1984, which was reported in Inspection Report 341/84-53. Based on acceptability of both local and integrated tests, the allegation will be closed by documenting findings in a routine resident inspection report.

ENCLOSURE 9 AMS No. RIII 86-A-0009 Prime Responsibility:

DR55 Allegation:

N eerous Personnel / Management inadequacies.

Action:

DRSS is following up on the allegation which followed what they had already identified for the most part.

DRSS review of allegation includes an interview with the alleger.
