I ENCLOSURE 1 NOTICE OF VIOLATION Florida Power Corporation Docket No. 50-302 Crystal River Unit 3 License No. DPR-72 During the Nuclear Regulatory Commission (NRC) inspection conducted on August 2 -
September 5, 1986, a violation of NRC requirements was identified. The violation involved the adequacy of the equipment control procedure. In accordance with the
" General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1986), the violation is listed below:
Technical Specification 6.8.1.a requires that procedures be established as recommended in Appendix "A"
of Regulatory Guide 1.33, November 1972.
Regulatory Guide 1.33, Section A.3, recommends the establishment of a procedure for equipment control.
Section A.4, recommends a procedure that covers procedure adherence.
Administrative Instruction AI-400, Plant Operating Quality Assurance Manual Control Document (P0QAM), was written to meet the requirements of Regulatory Guide 1.33, Section A.4, and specifies in section 4.1.3 that the word "shall" denotes a requirement and that the word "should" denotes a recommendation.
Compliance procedure (CP)-115, In-Plant Equipment Clearance and Switching Orders, was written to meet the requirements of Regulatory Guide 1.33 and specifies in step 5.3.5.h that the return-to-normal position of the valve, switch, or breaker after removal of a tag should be obtained from the applicable operating procedure (DP).
Contrary to the above, on August 20, 1986, procedure CP-115 was found to be inadequate in that the procedure did not require the use of an OP for a system's return to normal and did r.ot specify that system lineups may also be included in Surveillance Procedures (SP's) in lieu of OP's. As a result of this inadequacy, containment isolation valves were improperly restored to service following a maintenance evolution.
This is a Severity Level IV violation (Supplement I).
Pursuant to the provisions of 10 CFR 2.201, Florida Power Corporation is hereby required to submit to this Office within 30 days of the date of the letter transmitting the Notice a written statement or explanation in reply including:
(1) admission or denial of the alleged violation; (2) the reasons for the violation if admitted; (3) the corrective steps which have been taken and the 8610310140 861017 PDR ADOCK 05000302 G

Docket No. 50-302 Crystal River Unit 3 License No. DPR-72 results achieved; (4) corrective steps which will be taken to avoid further violations; and (5) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
