s HIGHLIGHTS OF WEEKLY INFORMATION REPORT WEEK ENDING JUNE 6, 1986 Diablo Canyon Units 1 and 2 On May 30, 1986 amendments were issued for the expansion of the spent fuel pools of Diablo Canyon Units 1 and 2.

The reracking will be perfonned with the pools in a dry and uncontaminated condition. The licensee expects to complete the effort in about 8 weeks. Unit 1 is scheduled to shut down for the first refueling at the end of August.

Should a trip occur after August I the unit will not be restarted.

A prehearing conference was held on May 13, 1986. We expect a hearing to be scheduled in about September.

NRC/ DOE Comparability Study Safeguards staff met with DOE representatives on May 6 to outline the conditions of the joint study on Category I fuel facilities.

On May 27, DOE transmitted to Safeguards the agreed Terms of Reference for the study and the names of their team members.

The DOE Offices of Naval Reactors and Safeguards and Security are represented.

NRC Safeguards will have two members on the study team.

The initial meeting of the joint study team is tentatively scheduled during the week of June 9,1986.

Operations Center Members of the Incident Response Branch, as part of the NRC Chernobyl Incident Tracking Team, are winding down their coordination of NRC's support to EPA through the Operations I

Center. Radiological measurements continue to be received, entered, and plotted by fractions of EPA Protective Action Guidelines (PAGs) by day.

Meeting with States on the Low-level Radioactive Waste Policy Amendment Acts (LLRWPAA) of 1985 OSP in coordination with NMSS is sponsoring a meeting on June 24-25, 1986 in Bethesda, Maryland on LLRWPAA of 1985 as part of NRC's continuing effort to foster the implementation of the Act's provisions. The purpose of this meeting will be to discuss with selected State officials NRC responsibilities under the Act, including the approach being taken and progress being made in fulfilling NRC responsibilities. The NRC staff wishes to obtain State views on technical and institutional issues associated with NRC and State implementation of LLRWPAA and to determine any additional areas in which the NRC can be of assistance in the develop-ment of disposal facilities. A Federal Register notice announcing the meeting with a preliminary agenda was published May 27, 1986. The OSP contact for the meeting is Kathleen N. Schneider.

Nine Mile Point 2 In a telephone call yesterday, May 28, 1986, Mr. Charles Mangan, Senior Vice President for Niagara Mohawk Power Corporation, indicated that in a Board of Directors meeting on May 27, 1986, the official fuel load date for Nine Mile Point 2 was slipped to July 21, 1986.

In a subsequent phone call from Mr. Anthony Zallnick, Licensing Manager for Nine Mile Point 2, it was also indicated that the schedule for criticality is August 18, 1986, and the schedule for commercial 4

i operation is March 28, 1987.

Emergency Technical Specification Change for St. Lucie Plant Unit No. 1 Florida Power and Light Company (FP&L), informed the staff that as a result of recent investigations of steam generator tube eddy current data, 17 steam generator tubes in Unit 1 have indications which exceed the 40% plugging limit. The 17 tubes are made up of 13 tubes in generator A and 4 tubes in generator B.

As a result of this new information, the licensee met with the staff on Thursday, May 29, 1986.

As a result of the meeting, the licensee and the staff agreed that an i

emergency technical specification change was needed in order for Unit 1 to continue operation.

The licensee submitted an emergency technical specification change request on May 30, 1986. The licensee proposed to operate the unit for up to 30 days on the basis that (1) the deterioration mechanism, i

intergranular attack (IGA), had not progressed significantly over the last cycle (cycle 6); (2) there is presently no primary to secondary leakage in the steam generators; (3) a tube would be expected to leak before it breaks; (4) an administrative leakage limit of 0.3 gpm for i

i both generators will be in effect; and (5) the tubes will be plugged when the unit shuts down.

The staff issued the emergency technical specification change on May 30, 1986. A final no significant hazards consideration determination was made. The State of Florida was consulted and had no comments on the determination.

River Bend Station During the week of April 28, 1986, a Region IV inspectcr identified a potential problem with the environmental qualification of a 2-inch jumper wire in one Limitorque M0V, located outside containment. After the NRC inspection, the licensee elected to audit Limitorque records at Limitorque.

On May 23, 1986, as a result of their audit of Limitorque, the licensee identified 78 MOVs, outside containment where the EQ documentation of certain wires within the MOVs were questionable. As of May 30, 1986, the licensee informed the staff, that the reactor would not be restarted until all 78 MOVs have been inspected and questionable wires replaced.

In addition, to ensure environmental qualification, the licensee is inspecting 15% of MOVs outside containment where the EQ documentation is adequate. The licensee had earlier inspected 100% of the MOVs within containment.

At 10:15 p.m. CDT on May 29, 1986, the licensee conducted a planned load rejection from full power as part of the last phases of their startup test program. The reactor is currently in hot standby.

The licensee expects River Bend Station to be in commercial operation by mid-June, 1986.

Comanche Peak, Units 1 and 2 SSER 13 was issued on Friday, May 30, 1986.

It. approved the utilities "Get Well Plan" for Comanche Peak - specifically the structure,,

framework, and scope of applicant's Comanche Peak Response Plan which was established to address and resolve all identified issues and conduct a reinspection of design adequacy and the quality of construction at Comanche Peak.

Certain issues will be addressed during the implementation phase of this program. Since new issues and associated corrective action may be identified during the reinspection, there is no definite completion date, but it is possible that it may be completed for Unit 1 by the end of 1986.

JUN 6 1936 ENCLOSURE B

Wolf Creek KG&E has voluntarily shutdown Wolf Creek to replace improperly crimped connections in safety related valve actuators supplied by Limitorque.

In a follow up to inspections required by IE Temporary Instructions 25.15/75 and Information Notice 86-03, KG&E has identified improperly crimped connections in approximately 156 safety related Limitorque valve operators. The improper connections appear to be caused by placing the terminal lug in the crimping tool backwards when assembling the valve and/or valve actuator.

KG&E plans to replace all crimped connectors in each of the 156 valves by June 9 and return to operation at tt:t time.

Region IV is contacting the Vendor Inspection Branch to determine what type of generic response is appropriate.

NRC TMI-2 CLEANUP PROJECT DIRECTORATE WEEKLY STATUS REPORT.

WEEK ENDING JUNE 6, 1986 1.

DEFUELING Core stratification sample acquisition (core boring) training is complete. The drilling rig will be disassembled this week then reassembled in the reactor building. Core boring is expected to start during the week of June 23, 1986.

Pick and place defueling activities continued. These activities were conducted principally to assure that the core bore drilling locations were ready to commence drilling. The number of canisters transferred to the fuel pool remains at 42.

The testing of the heavy duty spade bucket tool was inconclusive due to bending of the vertical jaw.

The Reactor Coolant System (RCS) turbidity was approximately 2 NTU (6

- 8 feet) as of Monday, June 2, 1986.

PLANT STATUS The reactor remains in long term cold shutdown, vented to the atmosphere. Core cooling is by natural heat loss to ambient building atmosphere. The average incore thermocouple reading is 80 F; reactor vessel water temperature as measured by resistance temperature detectors (RTD's) is 73 F.

The airborne radioactivity on the defueling platform is about 3 E-7 uCi/cc Tritium and 5 E-11 uCi/cc particulates, predominately Cesium-137 and Strontium-90. The platform is mounted above the modified internals indexing fixture which is mounted over the reactor vessel. These provide about 15 feet of water over the core region and 6 feet over the carousel holding the defueling canisters.

WASTE MANAGEMENT EPICOR II remains shutdown. The Submerged Demineralizer System (SDS) started processing the "B" reactor coolant bleed tank on Saturday, June 7, 1986.

ENVIRONMENTAL MONITORING US Environmental Protection Agency (EPA) sample analysis results show that TMI site liquid effluents are in accordance with regulatory limits, NRC requirements, and The City of Lancaster Agreement.

The Lancaster water sample taken at the water works river intake and analyzed by EPA consisted of a seven day composite sample taken May 18 - 24, 1986. A gamma scan detected no reactor related activity.

TMI water samples taken by EPA at the plant discharge (includes Units 1 and 2) to the river consisted of seven daily composite samples taken from May 18 - 24, 1986. A gamma scan detected a trace amount of Iodine-131 in the May 21 - 22, 1986 sample. While no reactor JUN 6 1986 ENCLOSURE B

i activity was measured, trace amounts of I-131 were detecte,d in water at both the Unit 1 intake and the site discharge, indicating that the activity is probably not TMI related.

The EPA analysis of the NRC outdoor air sample for the period May 26

- June 6,1986 showed that concentrations of Cs-137 and I-131 were below the lower limit of quantitative detectability for the system, i.e., less than 1.1 E-13 microcuries/cc. A trace of I-131 was detected in the sample.

The unquantifiably small traces of I-131 in both these samples are probably related to the Chernobyl accident.

AUXILIARY AND FUEL HANDLING BUILDING ACTIVITIES Water flushing continued in the Westinghouse Valve Room (281' Auxiliary Building) over the past week. This room abuts the annulus and is the location of the mini-decay heat cooling system.

Assembly of the Desludging system continued.

The testing program continues on the transfer shield.

On June 2, 1986, GPUN submitted a request to test three defueling tools over an approximate 2 week period. The tools were a heavy duty tong tool, heavy duty spade bucket, and hydraulic impact chisel. The use of this equipment was previously under NRC staff review as part of the May 15, 1986 defueling SER. The staff approved the use of the three tools on June 4, 1986.

PUBLIC MEETING l

The Advisory Panel for the Decontamination of TMI-2 will meet with the NRC l

Commissioners on June 11, 1986 at 11:00 AM. The meeting will be held at the NRC conference room at 1717 H Street, N.W., Washington, DC.

OFFICE OF NUCLEAR MATERIAL SAFETY AND SAFEGUARDS Items of Interest 4

Week Ending June 6, 1986 Kress Creek Representatives of NMSS and OELD met with Kerr-McGee Chemical Corporation representatives and their counsel on May 30, 1986, to discuss possible

. settlement conditions for the Kress Creek decontamination case.

The meeting was held at the request of Kerr-McGee.

Both parties concluded that further settlement discussions at this time would not be productive and so advised the Licensing Board Chairman. The Licensing Board will, therefore, proceed to issue a decision.

West Lake Landfill On June 2, 1986, a meeting was held at the West Lake Landfill.

Representatives of the West Lake Companies; University of Missouri, Columbia, (UMC); Oak Ridge Associated Universities; and NRC were in attendance.

The purpose of the meet-ing was to give the UMC professors the opportunity of seeing the present condi-tion of the landfill prior to completing their engineering report.

This was important as deposition of waste in one portion of the landfill has recently been completed and a new area is presently being utilized for waste deposition.

It was also important to determine if there was sufficient space on Area 2 of the landfill to redeposit the contaminated soil that has eroded from the west-ern berm.

B&W, Apollo, Pennsylvania B&W representatives made an oral presentation on license renewal and site decommissioning.

After the presentation, Mr. Cunningham, Director of Fuel Cycle and Material Safety, responded to questions by stating that residual problems from past operations needed to be closed.

Concerns were expressed about residual contamination leading to unnecessary personnel exposures during future operations and a future decontamination liability problem if decontami-nation could be effected now.

The staff would consider extending the expira-tion date on the license if B&W needed more time to prepare the renewal documents.

B&W was provided with copies of a May 28, 1986, letter report from Mr. David Styer, ORAU, on the subject, " Preliminary Survey - Babcock & Wilcox, Apollo, PA."

The report indicated that some radioactive material was found in outdoor areas of the Apollo site.

Mr. Coleman called the site to instruct personnel to establish the area as a " restricted area," pursuant to Part 20.

Prompt cleanup of the area was discussed.

NMSS representatives commented on the B&W survey plan for both the outdoor and under-the-facility areas.

Soil sanpling techniques for the crushed stone parking lot were also discussed.

B&W expects to resubmit the plan for approval and then promptly implement the plan.

The purpose of the meeting was improvement of the original system which is now over 10 years old.

Office of Inspection and Enforcement Items of Interest Week Ending June 6, 1986 1.

The following Significant Enforcement Actions were taken during the past week:

a.

A Notice of Violation and Proposed Imposition of Civil Penalties was issued to Progressive Engineering Consultants (Grand Rapids, MI) in the amount of $500. This action was based on multiple health physics violations (EN 86-34).

b.

0FCICE OF NUCLEAR REGULATORY RESEARCH Items of Interest Week Ending June 6, 1986 University of Maryland 2x4 B&W Simulation Loop The simulation of the initial blowdown phase for a small break LOCA has been completed successfully on the University of Maryland at College Park (UMCP) 2x4 Babcock and Wilcox (B&W) Simulation Loop. This loop is being used to perform integral effects tests on thermal hydraulics phenomena relevant to post-small break loss-of-coolant transients in B&W plant designs. The experi-mental data is intended to be used for comparison against small break loss-of-coolant models as required by TMI-2 Task Action Plan item II.K.3.30.

Current testing is developing detailed information on the operational perfor-mance characteristics of the loop and their applicability to small break LOCA Phenomena. After the completion of the characterization testing each experiment will be run to simulate a complete small break LOCA with all the phases (such as blowdown, boiler condenser mode) present.

Status of HDR Experimental Facility The NRC Office of Nuclear Regulatory Research and the Karlsruhe Nuclear Research Center (KfK) have entered into a cooperative program using the Heissdampfreaktor facility (an out-of-service nuclear power plant located in Kahl, West Gennany). The program consists of a series of experiments employing a large shaker (that was specifically designed for this program) to excite the internal structure and piping at frequencies typical of seismic events. The Electric Power Research Institute (EPRI) is also participating ir this experimental program.

The main objective of the NRC effort is to use the measured piping responses to validate complex computer codes that are used to predict the behavior of piping into the inelastic range. Another objective is to determine the effects of various piping responses and flow forces on the operability of a gate valve typical of U.S. designs. Valve operability; that is, the capability of the valve to open or close against fluid under pressure at the same time it is subjected to motions similar to those expected in seismic events, will be included in the experimental program.

To accomplish the NRC objectives, it has been necessary to design and install additional piping supports to replicate the stiffness of a typical U.S. plant piping system. The gate valve was procured from the Shippingport facility, refurbished, qualified, and installed for use in the experiment.

The experiments began on' June 2 (as scheduled) and will continue through mid-July 1986. A total of 27 experiments will be conducted during this period.

The effects of parameter variations such as temperature, excitation amplitude and frequency, and piping flexibility will be investigated. The later experiments will subject the facility to severe seismic challenges which are of particular interest to the NRC. A cognizant NRC staff member will be present during these latter experiments.

Preliminary results from the first two experiments showed that the piping response was strong at expected frequencies and that these responses had no measurable effect on valve operability.

l RES RULEMAKING ACTIVITIES Emergency Core Cooling Systems; Revisions to Acceptance Criteria (Part 50)

The proposed rule would amend regulations concerning acceptance criteria for emergency core cooling systems (ECCS) by changing the methods used to demon-strate that an ECCS would protect the nuclear reactor core during a loss-of-coolant accident.

Because the proposed rule represents a significant change in a regulatory requirement, the staff is currently preparing a summary of ECCS research performed over the last 10 years which will serve as the technical basis for the proposed rule and a regulatory guide which will provide definition of what constitutes an acceptable best estimate model and acceptable methods of performing the uncertainty evaluation.

A meeting was held on April 30, 1986, with the Thermal Hydraulic Phenomena Subcomittee of the ACRS to review activities associated with revision of the ECCS rule. Two peer review panels have been formed to review and recommend correlations for thermo-hydraulics and fuel rod behavior. These recommenda-tions will be included in the regulatory guide to be issued with the ECCS rule.

Preliminary results of the study of the risk implications of the ECCS rule revision indicate that, while there may be some impact on individual accident sequence emergencies, there should not be a significant impact on overall risk.

The draft Comission paper was sent out for office concurYence on May 7,1986, with responses requested by May 16. To date, comments have been received from NRR, ELD, and ADM; and the staff has met with these offices to resolve comments. No major problems have surfaced and concurrence has been received from ADM and NRR, while ELD's is expected this week. The paper will be sent to the CRGR for review by the end of this week and will be forwarded to the ED0 for submission to the Comission in July.

PRM-50-40--Requires Mandatory Reactor Shutdown if Cause of Reactor Trip Undetermined in 8-Hour Period In a petition for rulemaking filed on November 26, 1986, J. F. Doherty requested the subject action. The petitioner's concern stemed from the 1983 Salem anticipated transient without scram event.

Numerous comments on the PRM were received on the last day of the comment period. Most of the comment letters were sent by individual utility companies or their representatives. These utility comments emphasized the applicability of actions taken in response to Generic Letter 83-28 in lieu of the petitioner's recommendation. Safety and economic reasons were presented arguing against the petitioner's recommendation that the reactor be placed in cold shutdown condition if the cause cf a trip is not identified in 8 hours9.259259e-5 days <br />0.00222 hours <br />1.322751e-5 weeks <br />3.044e-6 months <br />.

Additional comment letters were received shortly after the close of the comment period on the PRM. These letters were trom utility companies and repeated arguments for denial made in previous utiiity letters.

The purpose of this meeting will be to discuss with selected State officials NRC responsibilities under the Act, including the approach being taken and progress being made in fulfilling NRC responsibilities. The NRC staff wishes to obtain State views on technical and institutional issues associated with NRC and State implementation of LLRWPAA and to determine any additional areas in which the NRC can be of assistance in the develop-ment of disposal facilities. OFFICE FOR ANALYSIS AND EVALUATION OF OPERATIONAL DATA ITEMS OF INTEREST Week Ending June 6, 1986 1.

A meeting between NRC and INP0 was held on May 29, 1986 at the INP0 offices in Atlanta to discuss recent significant plant-specific and generic reactor operating experiences. Representatives from AE0D, NRR, IE and Regions I and III were in attendance from the NRC. The topics discussed included motor-operated valve operating experiences, check valve problems, control rod mispositioning (Peach Bottom), INP0's 1985 root cause analysis study, AFW turbine overspeed trip experiences and the Pilgrim AIT investigation findings. Some preliminary discussions also addressed the recent accident at Chernobyl. The next technical informa-tion exchange meeting will be at AE0D's offices in Bethesda and will be scheduled for either late summer or early fall of this year.

On June 6,1986 AE00 informed IE by memorandum of a potential need to supplement IE Bulletin 86-01, " Minimum Flow Logic Problems that Could Disable RHR Pumps," which was issued on May 23, 1986. The bulletin was addressed to only BWR facilities.

The memo noted that AEOD had received and reviewed information which strongly indicated that a problem similar to the BWR RHR minimum flow control logic single failure vulnerability identified at the Pilgrim station (the topic of IE Bulletin 86-01) might also exist at a number of Westinghouse-designed PWRs. Documents from Wisconsin Electric and Carolina Power and Light described almost identical design deficiencies at Point Beach and H. B. Robinson.

The deficiency at both plants involved a single failure that could cause the loss of the common minimum flow recirculation path for all of the safety injection (SI) pumps. The deficiency at Point Beach had been the subject of a Part 21 report. Based on AE0D's preliminary review, it appears that a common recirculation (return to the refueling water storage tank) line with redundant isolation valve arrangement at Point Beach and H. B. Robinson is a standard design among a number of Westinghouse plants. Due to the potential safety significance which could be associated with a failure of the SI system at Westinghouse PWRs and the timeliness of IE Bulletin 86-01, the memorandum suggested that IE give immediate consideration to issuing either a supplemental or a separate IE bulletin to all Westinghouse PWR facilities concerning the SI recirculation path design deficiencies identified at Point Beach and H. B. Robinson.

OFFICE OF THE EXECUTIVE DIRECTOR FOR OPERATIONS WEEK ENDING JUNE 6, 1986 Proposed Rule Signed by EDO On June 9,1986, the Executive Director for Operations approved a proposed rule which will amend 10 CFR Parts 30, 40, 50, 61, 70, and 72. This rule will amend these parts to require a licensee to notify the NRC in the event that a bankruptcy petition is filed involving the licensee. Notification of NRC in these instances would enable NRC to take timely and appropriate measures to protect the public health and safety at a facility where the licensee's severe financial condition could affect its ability to handle licensed radioactive ma terial.

The proposed rule would require no action of a licensee unless and until a bankruptcy petition is filed. The effort required at that time to comply with this proposed action will be minimal. The Executive Director for Operations therefore certified under the Pegulatory Flexibility Act that the rule, if promulgated, will not have a significant economic impact on a substantial number of small entities.

This notice constitutes notice to the Commission that, in accordance with the rulemaking authority delegated to EDO, the EDO has signed this proposed rule and proposes to forward it to the Office of the Federal Register for publication.

The Commission requested OGC and ELD to coordinate and submit a paper to the Commission which addresses NRC's legal authority to:

(1) temporarily prohibit the storage of mixed waste at low level sites pending the formulation and promul'gation of standards by EPA and (2) limit the volume of mixed wastes by maximizing the treatment of mixed wastes under existing rules and without seeking additional legislation.

OGC and ELD, in coordination with the technical staff, should also identify any legal and technical questions the Commission needs to address prior to voting.

Commissioner Asselstine requested EDO to address whether funda-mental inconsistencies exigt between the RCRA and Part 61 that simply cannot be resolved technically.

This effort should identify the conditions under which RCRA permits some migration of hazardous wastes from land disposal and the options, such as pretreatment, liners, etc., that could bring LLW burial sites into compliance with RCRA.

The staff stated that it would quickly meet with EPA to obtain the information requested by individual Commissioners and, based upon this information, then revise the' staff's proposed options to ontain agreement on the course of action to resolve the technical and legal mixed waste issues.

The staff's options should include an assessment of EPA's ability to accelerate the development and promulgation of mixed waste standards.

Results of these meetings will be transmitted to the Commission.

These staff meetings could be followed by a Commission-level meeting with the EPA Administrator if necessary.

Chairman Palladino urged continued Commission attention on this matter for the next two weeks by identifying additional questions or by voting.

NMSS is to brief the Commissioner's Technical Assistants on the status and results of the meeting with EPA.
