CORRECTIVE ACTION .

1. POLICY Measures shall be established to assure that conditions adverse to quality shall be identified promptly and corrected as soon as practi-cal and, in the case of significant conditions adverse to quality, the cause of the condition shall be determined and corrective action taken to preclude recurrence.
2. RESPONS1BILITIES 2.1 Personnel who discover a condition adverse to quality shall promptly notify their supervisor and/or the Nuclear QA Organization.

2.2 Supervisors who are aware of a condition adverse to quality shall take the actions prescribed by established procedures for report-ing and correcting of the condition.

2.3 When a significant condition adverse to quality is identified, NQA personnel shall review, approve, and participate to the extent necessary to determine the cause of the condition and the corrective action required to preclude recurrence.

3. DEFINITION Conditions adverse to quality inclu1, feitures, as1 functions, defici-encies, deviations, defective material and equipment, abnormal occur-rences and nonconformances. - -- -

5.2 Each reported condition adverse to quality shall be reviewed for its safety inp11 cations and if significant, sub:itted to NSRC f or r e vi ew.

5.3 Trend analysis shall be used as a method for early identification of a condition adverse to quality.

5.4 Uhen the condition is significant or it is a recurring problem, the cause of the condition shall be deterrained and the appropri-ate corrective action taken to preclude recurrence. The cause and corrective action shall be reviewed by NQA.

5.5 The identification of the significant condition adverse to quality, the cause of the condition, and the corrective action taken shall be documented and reported to responsible management.

5.6 Procedures shall be established for the review of reported conditions adverse to quality for reportability to the NRC.

5.7 To provide a means for the reporting of quality concerns by per-s onnel who choose to be anonymous, procedures shall be estab-21shed to permit reporting of the quality concerns and to assure timely response.

5.8 Procedures shall be established for the corrective action required f or Vendor Supplied Products which do not conf orm to requirements.
