i Hope Creek is the last of the Mark I containment BWRs to be licensed in -

I the U.S. The facility was originally sited on Newbold Island (in the Delaware River), which is just above Philadelphia, PA, a relatively high-population site. In 1974. PSE&G relocated the facility to Lower

Alloways Creek Township. New Jersey, adjacent to its then under j ,

construction Salen Generating Station.

j PSE&G will commence fuel loading on Saturday. April 12, 1986. Initial criticality is scheduled for May 12, 1986. .

Hope Creek is the 100th nuclear power plant license at this time to

) generate electric power. This excludes plants which have been shut ,

1 NFS-Erwin On March 30, 1986, NFS and the OCAW Union reached an agreement on a new contract.

During the week of March 31, NFS conducted a two-day training session for all 1

supervisors and foremen. A special training program, consisting of classroom and on-the-job training, for the Union workers began during the week of April 7 l and is expected to be completed, for all 300 workers, by the end of the month.

NFS submitted information on April 7, 1986, concerning their plans for returning the facility to normal operations. A special inventory required by NRC letter (April 8, 1986) was begun, as s.cheduled, on April 15, with results expected by May 15, 1986.

Present NASA plans call for j these missions to be launched aboard the shuttle in December 1987 and i July 1988. Observers from NMSS will attend this meeting on April 22-24 j 1986, in Germantown, Maryland. .

ITEMS OF INTEREST Week Ending April 18, 1986 Arkansas Nuclear One, Unit 1 (ANO-1)

At 2:38 a.m. on April 8, 1986, ANO-1 tripped on high reactor coolant system pressure as a result of a failure in the turbine generator

control system. The unit was at 82% power at the time of the trip.

l The governor control valve of the turbine generator control system began to close which resulted in the RCS pressure to increase. The

unit was brought back to 13% power level for trouble shooting of the -

turbine generator control system in the afternoon of April 9,1986, and -

> returned to 82% power level in the afternoon of April 10, 1986, after .

making adjustments in the turbine generator control system.

Limerick On April 4, 1986, the Licensing Board denied and dismissed the I petitions of two intervenors, R. L. Anthony and F. R. Romano, for leave to intervene regarding the issuance of Amendment Nos. 1 and 2 to the 1 operating license. The Board's order, which also terminated the proceeding, denied the petitions on the basis of lateness and/or the finding that the proposed contentions are not within the scope of the issues addressed in the amendments.

1 Also, on April 4, 1986, the Appeal Board denied the licensee's Motion l to direct certification of a prior Board decision regarding Mr. Anthony. The Appeal Board found that the licensee's Motion was l premature, as a decision had not yet been reached to admit Mr. Anthony as a party to the proceeding since no contentions had as yet been admitted. The appeal Board noted that if the Board found none of Mr. Anthony's contentions acceptable, which is the case as noted above, l then there would be no need to deal with the licensee's Motion.

Shoreham Emergency Planning Mr. Frank Petrone, Director of FEMA Region II, which is the FEMA Region responsible for Shoreham, resigned on Monday, April 14, 1986.

Mr. Petrone cited " irreconcilable differences" with FEMA management over the content of the FEMA report on the Shoreham E-Plan exercise, as the reason for his resignation. )

Pilgrim Nuclear Power Station .

q At 3:00 p.m. on Friday, April 11, 1986, Pilgrim Station entered an LCO l when both injection valves in one LPCI loop were closed to control '

primary leakage into the low pressure piping. However, enough leakage still occurred to cause an alarm on high pressure. At 6:30 p.m. BEco declared an unusual event and began shutting down the plant. At

56 a.m. on April 12, the reactor scranned due to an MSIV isolation signal which occurred while the operator was changing the mode ' switch 4 from "run" to "startup". The signal is believed to have happened a because not all mode . switch contacts were properly made during the switchover. (This switchover problem had also occurred during a shutdown approximately one week earlier). The HPCI system was used to complete the shutdown. BECo was unable to reopen the MSIVs until 1 3:52 a.m. after equalizing the pressure on both sides of the MSIVs.

The plant achieved cold shutdown at 9:15 a.m., Saturday, April 12, 1986.

An augmented inspection team (AIT) was dispatched to Pilgrim upon recognizing the number of major problems that were involved in the event. A Confirmatory Action Letter was issued instructing BECo not to

, alter the plant configuration or perform tests until the AIT is present, to report on this event and the corrective actions.taken' prior to startup, and not to start up without prior approval.

Perry Perry 1 fuel loading'is continuing. Electrical failures were experienced in circuitry powering the refueling platform and fuel assembly hoist motor, which halted fueling operations from April 5 thru

! April 9th. Refueling operations were resumed on April 9th. As of

00 a.m. (EST) April lith, 332 fuel assemblies have been loaded out
of a total of 748 fuel assemblies. Despite the fuel loading interruption, the licensee's Plant Manager still expects to complete fuel loading operations by April 20, 1986, and to have initial criticality occur on April 28, 1986.

As a separate item,' a Perry plant full participation emergency

. exercise, involving the CEI, FEMA, State, County and local officials, ENCLOSURE B will be conducted on April 15, 1986. .

NRC TMI-2 CLEANUP PROJECT DIRECTORATE WEEKLY STATUS REPORT WEEK ENDING APRIL 18, 1986

1. DEFUELING

- As of April 20, 1986, a total of 44 defueling canisters have been loaded with fuel debris and transferred to the canister storage

- racks in the "A" Spent Fuel Pool in the Fuel Handling Building. The total weight of debris transferred out of the reactor vessel is -

50,170 lbs. representing 16% of the 308,000 lbs. estimated to be in the vessel.

- On April 20, 1986, operation of the newly modified Temporary Reactor Vessel Filtration System (TRVFS) began. The modifications to the system consist of a larger diatomaceous earth filter media capacity, higher flow rates, and the use of a fuel debris knockout canister as a discharge vessel. The modified TRVFS is one subsystem of the new Reactor Vessel Water Cleanup System (RVWCS) designed to cleanse the reactor vessel water of bio-organism growths. The other subsystem includes a positive displacement pump which will destroy the bio-organisms by high pressure.

- A thorough inspection of the Vacuum Defueling System (VDS) was conducted. During the inspection, the connect assembly between the knockout canister and the pumping station was found to be damaged.

Repair of the connect assembly consisted of replacement of the gripper subassembly which was found to be distorted. . Completion of other minor maintenance including load cell and back flush flow meter repair is scheduled this week. A vacuum defueling test will be conducted after completion of all repairs and improvement in t

t visibility in the reactor vessel.

2. RAIL SHIPPING CASKS APPROVAL On April 11, 1986, the NRC issued Certificate of Compliance Number 9200 to the Department of Energy (D0E). This Certificate approves the use of NUPAC-125B railcar mounted fuel debris shipping casks. DOE will use two of these casks to transport the TMI-2 core debris from the site to the Idaho National Engineering Laboratories for storage and research.
3. PLANT STATUS The reactor l remains in long term cold shutdown, vented to atmosphere. Core cooling is by natural heat loss to ambient building atmosphere. The average incore thermocouple reading is 79*F.

- The airborne radioactivity on the defueling platform is about 1.7 E-7 uCi/cc Tritium and 5.6 E-11 uCi/cc predominately Strontium-90.

The platform is mounted above the modified internals indexing fixture which is mounted on the reactor vessel flange. These provide water coverage of 151 feet over the core region. This water level is about 5 feet over the top of any debris canisters in the reactor vessel.

,. 4. WASTE MANAGEMENT ,

- The Submerged Demineralizer System (SDS) is shutdown. SDS total

. processed to date is 3,885,859 gallons.

- EPICOR II was in temporary shutdown this week. EPICOR II total.

processed to date is 2,854,371. .

5. ENVIRONMENTAL MONITORING

- US Environmental Protection Agency (EPA) sample analysis results show that TMI site liquid effluents are in accordance with regulatory limits, NRC requirements, and the City of Lancaster Agreement.

- TMI water samples taken by EPA at the plant discharge (incledes TMI-1 and TMI-2) to the river consisted of seven daily composite - '

samples taken from March 29 through April 5, 1986. A gamma scan detected no reactor related activity above the lower limit l detectability. Trace indications of Cesium-137 may be present but are not in sufficient quantities to be quantified or clearly identified.

The Lancaster water sample taken at the water works intake and analyzed by EPA consisted of a seven day composited sample taken from March 30 through April 5,1986. A gamma scan. detected no reactor related radioactivity.

NMSS representatives visited each Region in March 1986 to assess their s nuclear material licensing programs. NMSS will send draft reports to the .

Regions in May and will transmit final assessment reports to the EDO by l June 30, 1986. The' reports will document'the entire process and assess '

the technical adequacy, consistency, timeliness and efficiency of the licensing process.

Fuel Shipping Cask l

On April 11, 1986, a Certificate of Compliance was issued to the Department

of Energy (DOE) for a fuel shipping cask designed and scale model tested by J Nuclear Packaging, Inc. Two casks have been fabricated and will be used for the transport of the Three Mile Island core debris, packaged in canisters, to the Idaho National Engineering Laboratory, Idaho Falls, Idaho. The cask is designed to transport up to seven canisters by rail.

Galileo and Ulysses Space Missions The Interagency Nuclear Safety Review Panel (INSRP) and its subpanels will hold a meeting with DOE and NASA to discuss INSRP activities in connection with the Galileo and Ulysses space missions. Present NASA plans call for these missions to be launched aboard the shuttle in December 1987 and July 1988. Observers from NMSS will attend this meeting'on April 22-24, 1986, in Germantown, Maryland.

Possible Use of D0D/ DOE Technique for Recording Security Responses During a Regulatory Effectiveness Review (RER)

Under contract to the Defense Nuclear Agency, Essex Corporation developed

, a Security System Operational Recording and Analysis (SSORA) technique, used successfully at D0D and DOE sites. The NMSS safeguards staff is ex- .

ploring, with NRR and the licensee, the possibility of incorporating SSORA, on an experimental basis, into its next safeguards RER at Diablo Canyon in May 1986.

NFS-Erwin On March 30, 1986, NFS and the OCAW Union reached an agreement on a new contract.

During the week of March 31, NFS conducted a two-day training session for a.ll supervisors and foremen. A special training program, consisting of classroom and on-the-job training, for the Union workers began during the week of April 7, and is expected to be completed, for all 300 workers, by the end of the month.

NFS submitted information on April 7,1986, concerning their plans for returning the facility to normal operations. A special inventory required by NRC letter (April 8, 1986) was begun, as scheduled, on April 15, with results expected by May 15, 1986.

Certain enhanced physical protection and fire protection measures put in place during the strike are being maintained by NFS . Inspectors from Region II and staff from NMSS safe ~ guards licensing are at the site to observe the operations, the special inventory, the various training programs for the Union workers, and the check-out of certain equipment in the production area.

,' (1) An emergency preparedness exercise report from FEMA for Nine Mile Point was forwarded by IE to the region for appropriate action. While there were no deficiencies in offsite preparedness observed during the exercise, FEMA did identify areas requiring

(2) A staff member from the Emergency Preparedness Branch, Division of Emergency Preparedness and Engineering Response participated as an observer in the full participation exercise at Perry.

(3) FEMA has reported in accordance with their rule 44 CFR 350 that j the State of Iowa and associated local plans and preparedness for Quad Cities are adequate to protect the health and safety of the public. .

(4) FEMA has provided their interim findings on offsite radiological emergency plans and preparedness for Clinton. No deficiencies were identified and the results of the plan review and the

December 13, 1985 exercise were acceptable.

OFFICE OF NUCLEAR REGULATORY RESEARCH Items of Interest Week Ending April 21, 1986 Cooperative Research Efforts with NASA - Hydrogen Combustion l Final documentation of flame acceleration experiments at Sandia are being ,

deferred so that the Sandia Staff can participate in a cooperative research effort with NASA and the U.S. Air Force on potential hydrogen safety problems during the space shuttle launch for which Sandia' Hydrogen Behavior Researchers are uniquely qualified. During shuttle launch, quantities of excess hydrogen  ;

up to 200 lbs are released at engine start-up or engine shutdown if the flight i is aborted. If enough hydrogen undergoes combustion to produce overpressures, the orbiter may be damaged. Sandia will assess the feasibility of disposing of excess hydrogen without developing overpressures due to accelerated flames or detonations. This research will be supported by the U. S. Air, Force.

Sandia will assess the planned design and test matrix, and will conduct limited experiments at the White Sands facility. In addition, experiments will also be performed in the NRC hydrogen test facilities _ at Sandia Laboratories in Albuquerque. These experiments should provide data to determine the flamability limits for hydrogen-air-steam-C0 mixtures 2 and will investigate 1 the potential for flame acceleration in hydrogen-air-CO, mixtures. This work -

will complement the existing NRC Hydrogen Behavior Program. NRC generated data from the recently completed test series did not include the effects of diluents ,

on transition from deflagration to detonation (DDT). Since CO2 is known to be a good simulant for steam, the hydrogen-air-C02 flame acceleration test planned for NASA can be directly applied to assessment of the potential for DDT for 4 accident environments in containments. The experimental documentation will be I delayed about 3 months, and will allow for a more comprehensive study of flame acceleration at no additional cost to the NRC. White Sands Operations has l consulted with Sandia staff on the design of a heated detonation tube to study '

detonation properties, and data has been collected for temperatures several hundred degrees hotte.r than hot tube data obtained in Albuquerque. This information is needed'to reduce the uncertainty in extrapolating for temperatures much higher than those experimentally measured. The NRC will get access to the results of this research and it will serve to strengthen the final product from the Hydrogen Behavior Program. 4 1 ,

will consist of a series of experiments employing a large shaker (that was  ;

specifically designed for this program) to excite the internal structure and '

piping at frequencies typical of seismic events. The NRC has contributed l

additional piping supports to replicate the stiffness of a typical U.S. plant l piping system. The gate valve was procured from the Shippingport facility,

, refurbished and qualified for use in the experiments. The shaker has been installed and successfully tested over the desired frequency range and the installation of the piping supports'and the valve is on schedule and will be I completed by April 18, 1986. The experiments are scheduled to begin on June 2, 1986.

General Requirements for Decomissioning Nuclear Facilities (Parts 30, 40, 50,  ;

The proposal would provide more specific guidance on decommissioning criteria for production and utilization facility licensees and byproduct, source, and special nuclear material licenses. This action is intended to protect public l health and safety and to provide the applicant or licensee with appropriate  ;

regulatory guidance for implementing and accomplishing nuclear facility decomissioning.

The completed draft of the analysis and responses to public comments was sent

i to CRESS on April 14, 1986 for final typing. This draft will be sent for ,

Primary Reactor Containment Leakage Testing for Water-Cooled Power Reactors (Part 50) (Appendix J) l

The proposed rule would update and revise the 1973 criteria for preoperational l 1 and periodic pressure testing for leakage of primary and secondary containment i boundaries of water-cooled power reactors. The revision is needed to reflect  ;

current regulatory practice and current technology which are no longer being i reflected accurately by the existing rule, and endorsement in the existing  !

regulation of an obsolete national standard that was replaced in 1981.  ;

The proposed rulemaking package is at the EDO for signature before being sent I to the Comission for vote. Currently RES staff is working with EDO staff to conform backfit analysis to current Comission guidelines.

Notification of NRC of Cases of Bankruptcy Filing (Parts 30,40,61,70,72) i The proposed rule would require licensees to notify the appropriate regional l office of the NRC within a certain time period in the event of a bankruptcy j proceeding involving the licensee. Notification of the NRC in cases of l bankruptcy would alert the Comission so that it may deal with potential

. hazards to the public health and safety posed by a licensee that does not have the resources to properly handle licensed radioactive material or clean up ,

, possible contamination. l As of April 11, 1986, all offices have now returned concurrences to send the proposed rulemaking package to the EDO. NRR concurrence was based on revising
