A reactive inspection was conducted on December 15-19, 1986. This inspection
was to review an unplanned occupational radiation exposure received by a
worker. The worker's exposure exceeded his assigned administrative exposure '
limit. The exposure occurred on December 8, 1986, during preparation of a
resin liner / cask for shipment. A review of this job (dentified several con-
cerns in planning and control of this work. Also, two violations of 10 CFR
Parts 19 and 20 requirements were identified. Details are provided in the
enclosed report.
