NOTICE OF VIOLATION AND NOTICE OF DEVIATION
(NRC INSPECTION REPORT N05. 50-327/87-08 AND 50-328/87-08)
This refers to the Nuclear Regulatory Commission (NRC) routine resident
inspection conducted by Ken Jenison on February 6 - March 5, 1987.
The inspec-
tion included a review of activities authorized for your Sequoyah facility.
At
the conclusion of the inspection, the findings were discussed with those
members of your staff identified in the enclosed inspectinn report.
Areas examined during the inspection are identified in the report.
Within
these areas, the inspection consisted of selective examinations of procedures
and representative records, interviews with personnel, and observation of
activities in progress.
The inspection findings indicate that certain activities appeared to violate
NRC requirements and other activities appeared to deviate from a commitment to
the NRC.
The violation involves a failure to properly frisk prior to leaving
a radiologically controlled area and the deviation concerns a lack of preven-
tive maintenance on the condensate demineralizer waste evaporator pursuant to
your FSAR.
Accordingly, the violation and deviation, with pertinent references
and elements to be included in your response, are described in the enclosed
Notices.
Your attention is called to the unresolved items identified in the inspection
report which involve:
concern with the ability to effect adequate control
room evacuation and plar.t shutdown with minimum staffing levels; concern with
the adequacy of a specific diesel generator test; and a concern with the use
of a cable tray jumper without the use of a conduit.
These matters will be
pursued during future inspections.
In accordance with 10 CFR 2.790(a), a copy of this letter, its enclosure, and
any reply will be placed in the NRC Public Document Room.
If you wish to
withhold information contained therein, please notify this office by telephone
within ten days of the date of this letter and submit a written application to
withhold information contained therein promptly thereaf ter.
Such application
must be consistent with the requirement of 10 CFR 2.790(b)(1).
