This refers to the Nuclear Regulatory Commission (NRC) inspection conducted by
l
C. H. Bassett on July 19-22, 1988.
The inspection included a review of
activities authorized for your Catawba facility.
At the conclusion of the
,
inspection, the findings were discussed with those members of your staff
identified in the enclosed inspection report.
Areas examined during the inspection are identified in the report.
Within
these areas, the inspection consisted of selective examinations of procedures
'
and representative records, interviews with personnel, and observation of
,
activities in progress.
j
The inspection findings indicate that certain activities appeared to violate
NRC requirements.
The violation, references to pertinent requirements, and
elements to be included in your response are described in the enclosed Notice
of Violation. We believe that the failure of operations personnel to adhere to
the radiological control requirements during the events of June 4 and July 15,
i
1988, demonstrate their lack of awareness of or sensitivity to radiological
J
control requirements.
We also believe that if your internal system for
l
reporting and correcting radiological deficiencies had been effective, prompt
corrective action would have been taken following the event of June 4, 1988,
and the violation of NRC requirements would not have occurred on July 15,
,
1988.
Therefore, in your response to this letter, please provide a written

statement describing actions you have taken to increase
olant staff's
,,
awareness of or sensitivity to radiological control requirements and to
,
i
increase the effectiveness of the plant's radiological deficiency reporting and

corrective action system.
]

In addition, the inspection revealed that you failed to take full corrective
actions on a previous violation (50-413, 414/87-31-02) concerning failcre to
adhere to radiological control procedures for personnel contamination
j
monitorea and completion of daily dose cards.
On a previous inspection
conducted on February 22-26, 1988, it was also noted that you had failed to
,
take full corrective action for a prcvious violation (50-413, 414/87-40-03)

concerning failure to provide adequate written procedures for controlling

as a serious matter. Therefore, please include in your response to this letter
a written statement describing actions you have taken to ensure that connitted
actions are implemented,
in a:cordance with Section 2.790 of the NRC's "Rules of Practice," Part 2
Title 10, Code of Federal Regulations, a copy of this letter and its enclosures
i
will be placed in the NRC Public Document Room.
The responses directed by this letter and its enclosures are not subject to the
clearance procedures of the Office of Management and Budget as required by the
