Gentlemen:

This refers to the inspection conducted by Messrs. J. G. Luehman, H. F. Bundy,

and T. R. Staker of this office during the period June 1-30, 1986, of activities

authorized by NRC Operating License NPF-38 for Waterford Steam Electric Station,

Unit 3, and to the discussion of our findings with Mr. R. P. Barkhurst and other

members of your staff at the conclusion of the inspection.

Areas examined during the inspection included plant status, Licensee Event

Report (LER) followup, monthly surveillance, routine inspection, licensee

followup on previously identified items, monthly maintenance, potential generic

problems, license conditions, and IE Bulletins.

Within these areas, the

inspection consisted of selective examination of procedures and representative

records, interviews with personnel, and observations by the inspectors. These

findings are documented in the enclosed inspection report.

During this inspection, it was found that certain of your activities were in

violation of NRC requirements. Consequently, you are required to respond to

these violations, in writing, in accordance with the provisions of Section 2.201

of the NRC's Rules of Practice," Part 2, Title 10, Code of Federal Regulations.

Your response should be based on the specifics contained in the Notice of

Violation enclosed with this letter.

The timely and accurate processing of station modifications at your facility has

been an identified concern for some time. The failure to update drawings and

procedures has already led to one group of events in which unplanned radiological

releases having minor environmental and radiation exposure consequences occurred.

The new example of a failure to properly update plant drawings discussed in the

attached NRC inspection report points toward inadequacies in the actions your

staff previously committed. to implementing in order to address the original NRC

concerns.

Efforts to upgrade the facility's station modification program must,

on a priority basis, address procedures and drawings that support recently

completed and future modifications.

Also, during this inspection, it was found that certain of your activities

appeared to deviate from a commitment made to the NRC. This item and reference

to the commitment is identified in the enclosed Notice of Deviation. You are

requested to respond to this deviation in writing. Your response should be

based on the specifics contained in the Notice of Deviation enclosed with this

letter.

We have also examined actions you have taken with regard to previously

identified inspection findings. The status of these items is identified in

paragraph 7 of the enclosed report.

The response directed by this letter and the accompanying Notice is not subject

to the clearance procedures of the Office of Management and Budget as required

by the Paperwork Reduction Act of 1980, PL 96-511.
