HIGHLIGHTS OF WEEKLY INFORMATION REPORT WEEK ENDING AUGUST 15, 1986 Catawba Units 1 and 2 On August 9,1986, at about 7:15 a.m., an alert was declared at Catawba Unit 1 following reactor coolant pump 1B seal failure. Leakage was about 60 gpm, the pump was imediately secured, and the operators started cooling and depressurizing the reactor coolant system. At about 8:35 a.m. the alert was degraded to an unusual event as the leak rate decreased to about 35-40 gpm.
At 6:00 a.m., August 10, 1986, the leak rate was about 8 gpm and the Unit was in Cold Shutdown (Mode 5).
Shoreham Shutdown Shoreham's licensee declared an Unusual Event at 12:45 p.m. on August 11, 1986.
The reactor was operating at rated temperature and pressure, and the power level was at about 35 of full power. An indicator channel for reactor water level had become inoperable at 10:40 a.m., August 11, 1986. The Technical Specifications (T.S.) state that if the inoperable channel is not placed in the tripped condition, the channel should be restored to operable status within 2 hours2.314815e-5 days <br />5.555556e-4 hours <br />3.306878e-6 weeks <br />7.61e-7 months <br />, or the reactor enters an Action Item which requires a HOT Shutdown within 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br />. The plant manager decided to initiate the shutdown procedures and placed the reactor in Hot Shutdown at 12:45 p.m. The Shoreham Emergency Plan requires that an Unusual Event be declared whenever the reactor is shutdown to comply with a T.S. requirement. The Unusual Event was terminated at 4:55 p.m. after the reactor was placed in Cold Shutdown at 4:05 p.m.
Subsequent investigation by the licensee found that the level indicator channel was out of calibration, probably because of entrained air in the test-leg.
Both channels have since been recalibrated and the reactor was restarted August 13, 1986.
Fission Product Release Prom High Temperature Fuel s One of the significant contributors to severe accident source term uncertainty l for early containment failure accidents is in-vessel fission product release.
I The Oak Ridge High Temperature Fission Product Release Program is conducted to obtain realistic release data to reduce this uncertainty. Recently, a second fission product release test was performed in the vertically re-oriented furnace. In this test, a BR-3 fuel specimen (irradiated to approximately 40 in the BR-3 reactor in Belgium) was heated to 2300K at high steam GWD/Mt flow condi U0,tions. The test duration was held at I hour so that release data as a function of time could be obtained. Pre-test and post-test activity reading indicated approximately 60 to 70 percent of release for noble gases and volatile fission products. Detailed analysis is underway.
l Price-Anderson Legislation On August 12, 1986, the House Comittee on Energy and Comerce completed action and reported out H.R. 3653, the " Price-Anderson Amendments Act of 1986." Different versions of H.R. 3653 have also been reported out by the House Comittees of Interior and Insular Affairs and Science and Technology.
The Senate Comittees of Energy and Natural Resources and of Environment and Public Works have each reported out versions of S.1225, the " Price-Anderson Act Amen'dments Act of 1986." Thus, all of the Congressional Comittees with jurisdiction over this legislation have reported out proposed legislation.
l The Bills have been extensively amended in Subcomittee and Comittee markups l and it is problematical as to whether Price-Anderson legislation will reach l the floors of Congress, much less be passed and signed by the President, I this year.

- Week Ending August 15, 1986 Byron 1 On August 10,1986, Byron 1 began increasing power above the 75% self-imposed limit. The 75% restriction was taken as a preventative measure to decrease the potential for primary water stress corrosion cracking of the steam generator tubes.
The licensee is implementing under 50.59 a program that will allow Byron 1 to achieve 94% power and keep hot leg temperature to about 600*F. Prior to implementing this program, hot leg temperature was 618'F at 100% power, 600*F at 75% power.

The following Vendor Program Branch inspections were conducted this week:
(1) General Electric Company, San Jose, CA - to review the procurement of ASCO solenoid valve refurbishment kits.
(2) Western Piping & Engineering, San Francisco, CA - to followup on an allegation concerning the qualification of personnel performing certifying pipe clamp calculations used in nuclear facilities.
(3) Salem Nuclear Power Plant, Hancocks Bridge, NJ - to review licensee's equipment qualification program implementation as
> required by 10 CFR 50.49.
(4) Ebasco Services, Inc., New York, NY - to review the implementation of the Ebasco QA program, specifically those activities relating to cable tray / conduit support evaluation, as part of the Comanche Peak Response Team Design Adequacy Program.
- 2. Performance Indicater Program The Director, Divisicn of Emergency Preparedness and Engineering Response and staff briefed INP0 and RII personnel on Performance Indicators in Atlanta, GA this week.
- f. Safety System Inspection Program Chief, Vendor Program Branch and the Acting Chief Reactor Construction Programs Branch, participated in a meeting with INP0 to discuss the INP0 and NRC outage inspection program.
- g. Congressional Briefings The Director, IE, Director, DEPER and Branch Chief, IRB presented testimony on the matter of the Emergency Response Data System Bill introduced by Rep. Huckaby (LA).
- h. ACRS Briefings The Director, IE and IE management provided the ACRS subcommittee on Inspection and Enforcement programs an overview of office activities.

- 1. Emergency Preparedness A FEMA emergency preparedness exercise report for Trojan was forwarded to the regions for appropriate action. The exercise tested the offsite emergency plans in the 50-mile ingestion pathway EPZ.
- j. Program Development (1) Temporary Instruction (TI) 0110/4 has been approved for issuance. The purpose of the TI is to provide instructic7s for determining whether modules for dry storage of spent fuel at H. B. Robinson are properly constructed.
(2) Inspection Procedures (IP) 81360, 81365 and 81370 have been approved for implementation. These IPs cover shipments of formula quantities of special nuclear material, and they replace the existing draft interim procedure. Manual Chapter 2681, Exhibit 6 has been revised to incorporate the new procedures.
(3) Drafts of revised inspection Storage Racks) and IP 37700 Design,(procedure IP and Design Changes 50095 (Spent Fuel Modifications) have been forwarded to the Regions for review and comment.
- k. Other Inspections A member of the Safeguards and Materials Programs Branch participated in the uranium mill inspections this week with Region IV inspectors.

IBM PC Software - Generation of Renodalized Plant Models for TRAC Analyses Energy Incorporated of Idaho Falls, Idaho has been awarded a two-year contract to develop IBM PC software to be used in the generation of renodalized plant models for TRAC analyses. The software to be known as " EIKON / TRAC" will be designed to operate on existing detailed three-dimensional nodalization models used for plant accident studies to produce reduced three-dimensional The models reduced that can be then used for scoping of parametric TRAC analyses.

2 Foreign Visitors On Monday, August 4, five members of the Hong Kong Legislative Council visited with representatives of the EDO, 0GC-Bethesda, NRR, RES, and IE.
Discussion topics included the legislative basis and evolution from AEC to NRC, the current organization, the license review process, current technical issues, the source term program, and IE responsibilities. The group also toured the Operations Center.

OFFICE OF STATE PROGRAMS ITEMS OF INTEREST WEEK ENDING AUGUST 15, 1986 Price-Anderson Legislation On August 12, 1986, the House Committee on Energy and Commerce completed action and reported out H.R. 3653, the " Price-Anderson knendments Act of 1986." Different versions of H.R. 3653 have also been reported out by the House Committees of Interior and Insular Affairs and Science and Technology.
The Senate Committees of Energy and Natural Resources and of Environment and Public Works have each reported out versions of S.1225, the " Price-Anderson Act Amendments Act of 1986." Thus, all of the Congressional Committees with jurisdiction over this legislation have reported out proposed legislation.
The Bilh have been extensively amended in Subcommittee and Committee markups and it is problematical as to whether Price-Anderson legislation will reach the floors of Congress, much'less be passed and signed by the President, this year.

- 1. AE0D Engineering Evaluation Report, AE0D/E610, " Loss of LPCI Loop Selection -
Logic at M111 stone-1," was issued on August 15, 1986. The report provides an evaluation of a recent event at Millstone-1 in which three Model 288 Barton differential pressure (D/P) switches in the LPCI loop selection logic were found in the tripped condition. The study found that under certain recirculation pump flow conditions, improper operation of these switches could result in the LPCI system injection into the broken recir-culation loop following a postulated design basis LOCA. The seven BWRs still equipped with LPCI loop selection logic were found to utilize this type of switch. The switch is also used in other safety-related applica-tions at various LWRs. The study concluded that the'Model 288 Barton D/P switch can be susceptible to setpoint drift and that this susceptibility appears to increase as the switches age. The study also concluded that the other BWRs with LPCI loop selection logic could be vulnerable to this type of LPCI system degradation. The study was transmitted to IE with the suggestion that IE issue an information notice which discusses the Millstone-1 event and the potential susceptibility of the Barton switch to setpoint drift. It was also suggested that the IE Vendor Inspection Branch evaluate the application of the Model 288 Barton D/P switch to the LPCI loop selection logic and the adequacy of the manufacturer's surveil-lance and maintenance recommendations.
- 2. AE00 Technical Review Report, AE0D/T606, " Inadvertent Recirculation Actua-tion Signals at Combustion Engineering Plants" was issued on August 7, 1986.
The study was prompted by the different system responses to an inadvertent recirculation actuation signal (RAS) at Arkansas Nuclear One-Unit 2 and Calvert Cliffs Unit 2. The study found that at plants without antidraindown check valves in the emergency core cooling system, pump suction lines could transfer a significant inventory of borated water from the reactor water tank (RWT) to the containment sump on an inadvertent RAS. Antidrain-down check valves prevented a similar loss of RWT inventory at the other plant design. Because plant responses were so different, the safety aspects of a postulated safety injection actuation signal subsequent to an inadvertent RAS were analyzed for each plant design. The analysis showed that, for either plant design, adequate suction flow would be provided to f' the ECC pumps in event of a coincident RAS and LOCA signal.

! The report documents the analysis of the rupture of one of eight high intensity (40 mil 11 curies each) fodine-125 seeds being used for the treatment of brain tumors.

The iodine-125 seeds were being prepared for treatment of a second patient when the seed was ruptured. - the seed was not detected prior to implanting the seeds in the patient; consequently, the patient and about 60 hospitel employees received uptakes of iodine-125 and the licensee's facility was contaminated. (The patient received a thyroid uptake of 557 microcuries; other personnel uptakes ranged from .00004 to .209 microcuries.)
The findings of the case study were that the consequences of the seed rupture could have been mitigated by improved radiation safety practices and precautions that would have led to prompt detection of the leaking sending an seed. The recommendations contained in the report include: '
information notice to affected licensees describing the event; requiring seed manufacturers to provide to users safety precautions for handling the seeds; and considering requiring the licensees who use the seeds to submit procedures for handling the seeds to NRC for review.
- 4. AE00 has published its " Trend and Patterns Analysis of Trips at U.S. Light Water Reactors for 1985," (AE0D/P602). Based on our evaluations and analysis, we have arrived at the following general observations: (a) in terms of industry-wide performance, we observed a slight change in the total trip rate from 1984 to 1985 [i.e., 5.9 to 6.0 trips per reactor year); (b) hardware failures associated with the Feedwater, Condensate, Main Steam, Main Turbine and Main Generator Systems (collectively referred to as Power Conversion Systems) dominated in 1985 as the cause of reactor trips; (c) at power levels above 15 percent, approximately 10 percent of all reactor trips were caused by unlicensed personnel; and (d) a number of post trip recoveries were complicated by equipment failures and personnel errors (unrelated to the original trip cause) that have the potential for having significant safety implications.
