We have received the enclosed Federal Emergency Management Agency (FEMA) letter dated March 4, 1987, containing the offsite emergency preparedness findings for the State of Illinois and DeWitt, Macon, McLean, Piatt and Logan Counties in Illinois for the Clinton Nuclear Power Station exercise conducted on January 13, 1987. Based on the performance of the offsite agencies during the exercise, FEMA identified no deficiencies requiring corrective action.
However, two Areas Requiring Corrective Action (ARCAs) were observed that required corrective actions by the State of Illinois. As noted in the enclosed report, the State of Illinois has responded to the ARCAs, and FEMA has evaluated and accepted the response.
We fully recognize that any such remedial actions implemented involve other parties and political institutions which are not under your direct control.
Nonetheless, we do expect the subject of offsite preparedness for the area around the Clinton Nuclear Power Station to be addressed by you as well as others.
In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of this letter and the enclosure will be placed in the NRC's Public Document Room.

H is is to transmit a copy of the Exercise Report of the January 13, 1987, joint exercise of the Illinois offsite radiological emergency preparedness plans for the Clinton Power Station, near the City of Clinton, Illinois.
Participants included the State of Illinois (full participation), DeWitt County (full), including the municipalities of Clinton, DeWitt Village, Weldon and Wapella; Macon County (partial); McLean County (partial);
Piatt County (partial) in support of DeWitt County; Logan County (relocation center); and the Illinois Power Cmpany (full).
Excluding togan County all of the above are impacted by the plume exposure 10-mile Emergency Planning Zone (EPZ). The date of the final report prepared by the Federal Emergency Management Agency (FEMA) Region V was February 23, 1987.

In the January 13, 1987 exercise, conducted by FEMA Region V, there were l
no deficiencies observed; however, there were two Areas observed as Re-quiring Corrective Action (ARCAs). he State of Illinois has received a copy of the exercise report and responded to the ARCAs cited by FEMA. The l

'!he Illinois hergency Services and Disaster Agency (IESDA) dispatcher for the State hergency Operations Center (EOC) and the Illinois Departnent of Nuclear Safety (IDNS) dispatcher for the Radiological hergency Assessnent Center (REAC) initiated activation procedures following receipt and verification of the " Alert" classification. Written call lists and pagers were used to notify staff for both facilities.

Connercial telephone, state radio and anateur radio are available to provide connunication capabilities in the State EOC, the REAC and the ECP. The Nuclear Accident Reporting Systen (NARS) served as the prinary neans for the dissenination of protective action infornation fron the State to DeWitt County. Sone difficulty in accurate conpletion of the NARS by the State lead to sone ninor delays in the dissenination of infornation.

It is reconnended that field teans be continuously notified of updated plant and neterological conditions. The field teans equipuent was adequate to nonitor i
anbient radiation levels and the plune. Air sanpling techniques were adequate to neasure radioiodine concentrations and noble gas activity.
Field teans were knowledgeable in the techniques for obtaining environnental sanples. Sanples were double bagged and double tagged. A procedure for sanpling snow should be developed to ensure a uniforn sanpling and analysis procedure. Analytical and laboratory equipnent in the nobile laboratory was capable of preparing environnental sanples for radionetric analysis.
The lab's equipnent was used with direction provided in written SOPS (except snow) to adequately denonstrate the analysis of all sanples collected.

Pltme exposure dose protective actions at the REAC.Were initially based on plant paraneters and updated based on neasured release data fron the plant.
'Ihyroid dose projections were initially based on estinated filter efficiency of the Standby Gas Treatnant Systen and later revised based on dose projections fron the air sanple concentration data received from the RAFT.
Protective action reconnendations were based on EPA guidelines and evacuation tine estinates. An early precautionary evauation was reconnended for the population in the zero to two aile radius based on plant conditions and geographic constraints on the evacuation routes. A subsequent reconnendation was nade for evacuation two to ten niles southeast of the plant. The first evacuation order was preceeded by and the second evacuation order included reconnendations to place livestock on stored feed and water in the affected areas.
De State EOC directed the activation of the Pronpt Alert and Notification Systen when these protective action reconnendations were issued to DeWitt County. The Illinois State Police (ISP) and Illinois Departnent of Transportation (IDOT) coordinated the establishnent of road blocks fron the ECP.

'1he REAC nade the decision to reconnend the use of potassian iodide (KI) for mergency workers and innobilized populations. The REAC and the DeWitt County EOC coordinated the decision not to issue KI to the residents of the Clinton Nursing Hone.
KI distribution was sinulated for energency workers at the RAFT and the PCP.
A nedia briefing roon is available. at the State EOC. Slowever til nedia triefings were conducted at the Joint Public Infornation Center in Decatur.
IESDA and IDNS dispatched their JPIC staffs at the tine of the " Alert" notification. DeWitt County activated their staff at the " Site Area Ihergency" notification.
IE5DA and IDNS sraffed the JPIC for a single shif t with a roster available to denenstrate around the clock staffing capability. DeWitt County provided staffing for a two shift operation.
"he JPIC facility has a large briefing area as well as adequate support areas for the staff and nedia. A graphics section is located in the JPIC to support briefers with naps and displays. Connercial telephones are the prinary neans of connunications between the JPIC and the State EOC and the REAC. Telephones also serve as the prinary connunication link with the DeWitt County EOC.
Datafax was available for hard copy transnission.
Representatives fron IESDA, IDNS and Illinois Power Conpany briefed the nedia on eleven occasions. The briefings were clear, accurate and tinely.
Prior to each briefing the IESDA and IDNS coordinated the naterial to be released with their respective EOCs, each other and with the utility.
'1he REAC Connander and his staff reconnended unrestricted reentry based on the results of the analysis of environnental sanples. The radiological data indicated that potential doses would be well below the PAG 1evel. The State EDC in conjunction with the REAC issued reconnendations to DeWitt County to allow for the reentry into the affected evacuated areas. The ICP staff denonstrated their responsibilities in the area of traffic control and naintainence of an orderly reentry.

A reception, congregate care and decontanination center for evacuees was activiated at the Beason Elenentary School in Logan County. Se Red Cross was notified by ESDA to activate the center in conjunction with IDNS to receive evacuees fron DeWitt County. During this exercise, a bus load of twenty-one students fran Lincoln High School were pr&wa==9 through the reception center. The volunteers fron the Lincoln Ingan County Red Cross Osapter denonstrated a high degree of training in the registration and care of the evacuees.
IDNS denonstrated their equipnent and procedures for radiological nonitoring and decontanination of evacuees. Telephones and radios operated by the Radio hergency Associated Citizens hn were the prinary neans of connunication between this facility and other locations.
Provisions are available for this facility to function adequately as a congregrate care center.
DeWitt County We BOC staff was nobilized according to the procedures in the plan. Upon notification of the " Site Area Ehergency" the County ESDA staff innediately started to activate the EOC. The ability to naintain a full staff for an extended period of tine was denonstrated by double staffing and the presentation of a roster. Se EOC staff worked as a cohesive unit under the direction of the County Board Chairperson and County ESDA Coordinator.

9 Each County's ESDA Coordinator was the prinery individual charged with the coordination of his County's support to the DeWitt County energency response. These individuals denenstrated a high degree of training, knowledge of the Clinton plan and a thorough understanding of their duties and responsibilities.
Although not fully activated for this exercise, each County's BOC would be sufficient to support the County's energency response activity. The Clinton plan, NARS forns and the appropriate naps and displays were available in each soc. Telephone is the prinary neans of connunication for each County with radio backup. Macon County had a hard copy datafax capability.
Public alerting and instruction for the EPZ population in these three Counties is the responsibility of DeWitt County. McLean County was not affected by the pitme. However, the County did place its law enforcenent and relocation center staff and resources on standby should their activation have becone necessary.
Portions of Macon and Platt Counties were sinulated evacuated during the exercise. Upon receipt of the evacuation order, the Macon and Platt County Sheriff Departnents established traffic and access control points as called for in the Clinton plan. Dosinetry and KI were available for energency workers assigned to the -field. 'Ibe reconnendation for energency workers to take KI was acted upon pronptly by the Counties.

EXERCISE REPORT Introduction 1.
Exercise Background
'Ihis was the second joint exercise for the State of Illinois, DeWitt County, Macon County, McLean County, Platt County and Illinois Power Company resulting from a sinulated accident at the Clinton Power Station. The first joint exercise occurred on December 4, 1985 and involved full participation by the State of Illinois, DeWitt County and Illinois Power Ccapany.

'Ihe 10-nile plune exposure EPZ of the Clinton Power Station inpacts prinarily on DeWitt County.
It impacts to a lesser extent on Macon, McLean and Platt Counties. The State of Illinois, all four EPZ Counties, Logan County (relocation center) and the nunicipalities of Clinton, DeWitt Village, Weldon and Wapella participated in the exercise.

List of Evaluators For this exercise there was a total of fourteen Federal evaluators observing offsite exercise activities. Onsite activities were evaluated by a separate Itean fron the Nuclear Regulatory Connission (NRC). Of the offsite

'Ibe plans that were evaluated by this exercise were developed using the
" Criteria for Preparation and Evaluation of Radiological h ergency Response Plans and Preparedness in Support of Nuclear Power Plants" (NUREG-9654/FDR REP-1, Revision 1). Therefore, these criteria and the nodules based on these criteria entitled, " Modular Fornat for Unifornity of Radiological h ergency Preparedness Exercise Observations and Evaluation," dated June 1983, were used for exercise evaluation.

Sunnary of the Scenario
'Ihe Clinton Power Station radiological energency preparedness exercise objectives and scenario for the January 13, 1987, joint full participation exercise were developed by exercise planners of the Clinton Power Station, IESDA and IDNS. The exercise planners served as controllers during the exercise and protected the scenario fron the exercise participants so they would not be aware of the scenario events.
FEMA Region V participated in a conference call with State planners to select the objectives and deternine the scope of the exercise. Review of previous exercise weaknesses was included in these discussions and were reflected in the objectives selected for this exercise.
Sulmission of the scenario was according to guidelines outlined in NRC and FDiA guidance.
Idaho National Engineering Laboratory reviewed the onsite and offsite scenario for FDiA so evaluation of the objectives selected by the State of Illinois as well as DeWitt County could be accurately assessed. The exercise scenario was sufficient to initiate the energency response by the State of Illinois and DeWitt County. The scenario did allow for the denonstration and evaluation of the objectives selected for the January 13, 1987 exercise of the Clinton Power Station. A narrative sunnary of the exercise scenario follows.

INITIAL CONDITIONS Initially, the Clinton Power Station is operating at approxinately 100 percent power, near nid-cycle core live.
It is in its second day of a 14-day Liniting Condition for operation due to the inoperability of the High Pressure Core Spray (HPCS) systen. 'lhe Maintenance Departnant is replacing several sections of the HPCS pump inpeller to increase the slowly degrading efficiency of the punp.
Tine has nearly expired on the quarterly operability surveillance of Low Pressure Core Spray (LPCS) valves. The LPCS will be placed tenporarily out of service in the norning to cycle and record valves stroke times. The planned surveillance will place the Station in a Technical Specification action statenent to return LPCS to service or be in Hot Shutdown within 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br />. The tests are expected to take two hours, at which tine the LPCS systen will be returned to service.
UNUSUAL EVENT (0800-0900)
While perforning LPCS valve operability tests, the LPCS Suppression Pool Suction Valve lE21E001 will be tined shut. However, upon given an open signal, the valve fails to open. Problens with nechanical binding within the valve and notor operator render the valve inoperable. Realizing that the valve cannot be repaired quickly, the Shift Supervisor nust declare LPCS inoperable and order a controlled plant shutdown. An " Unusual Event" should be declared.
ALENT (.0900-0950)
While proceeding towards hot shutdown, a feedwater control systen nalfunction wil; occur causing feedwater punp rang up -and high reactor water level scran. The feedwater punps will not trip in tine to prevent flooding of the nain stean lines. Alnost sinultaneously, a failure in the Reactor Core Isolation Cooling (RCIC) initiation logic will cause the RCIC systen to autonatically start. Since the RCIC turbine utilizes stean fron the nain stean lines, a water slug will be forced through the ICIC stean supply piping. An unisolatable RCIC stean supply line break will then occur within the Drywell will innediately indicate a breach in the reactor coolant i
pressure boundary and an " Alert" should be declared.
SITE AREA EMERGENCY (0950-1130)
'Ihe RCIC line break will cause the reactor to depressurize allowing the operation of low pressure Ehergency Core Cooling Systens (ECCS). Low Pressure Coolant Injection punp A (LPIC A) fails to start. Low Pressure Coolant Injection punp C (LPIC C) is used to control reactor water level.
Residual Heat Renoval pung B (RHR B) will be placed in suppression pool cooling node.
Later, snoke is indicated on the 871' elevation of the Auxiliary Building followed by the loss of the Division II 125 VDC power. Control power is lost to Division II equipnent causing LPCI C and RHR B to trip. Nonajor I
source of water to the reactor is available. The snoke indicates a fire of sone nature occurred in the Auxiliary Building causing the lose of Division II 125 VDC, a safe shutdown systen. A " Site Area Ehergency" should be declared.

GDIERAL D1ERGENCY (1130-1439)
Reactor water level will fall below the top of the active fuel. Se reactor will be depressurized. A " General h ergency" should be declared due to the loss of two of three fission product barriers, and the potential loss of the third.

Efforts to restore significant quantities of water to the reactor will prove fruitless. Suppression pool tenperature will increase to above design lin its. Localized boiling will occur in the suppression pool causing an increase in containnent pressure.

Description of State and Local Resources Planned To Be Used in the Exercise The State of Illinois planned to staff and denonstrate response activity at the State EDC in Springfield; the IDNS REAC and RAFT (including field nonitoring teans); the State Forward Connand Post in Bloonington; the relocation center at the Beason Elenentary School in Logan County; and the JPIC in Decatur.

The respective agency representatives in the ECP at the Arnory in Bloonington were effectively in charge of their energency response. Overall coordination of the ECP operation was provided by IESDA. There were periodic briefings and nutual discussions relative to actions for access control and use of evacuation routes. All personnel were knowledgeable, responsive and well trained.
Facilities The facilities of the State EOC can support extended operations. There is sufficient furniture, space, lighting and telephones for the staff. Noise is controlled at a satisfactory level. Arrangenents can be nade to house parsonnel for an extended stay in the EOC. Backup power generation is available in the facility.

A decision to reconnend issuing potassiun iodide (KI) to energency workers and innobilized populations was nade by the REAC at 1190. Subsequently, a discussion was held between the Environnental Analyst and the County IDC l
staff with respect to the possible need to issue KI to the residents of the Clinton Nursing Hone. Based on neterological conditions and forecasts, it appeared that the Clinton Nursing Hone would not be in the pitme path so KI was not reconnended. The nursing hone location was kept under observation in the event that conditions warranted a need for further protective actions.

Recovery and Reentry he REAC Connander and his envirormental staff evaluated the radiological data from the environnental sanples that had been collected by the field nonitoring teans. The radiological data from these sanples indicated that the potential doses fran use or consunption of the various environnental nedia would be well below the PAG level for each nadia type. Once this deternination was nade, the REAC Connander nade a decision to reconnend unrestricted reentry to the affected areas.
The State EOC staff coordinated the relaxation of protective actions following the IDNS reconnendation for unrestricted reentry for the population. his reconnendation was connunicated to the DeWitt County EOC by telephone and NARS nessage. Each agency notified it's central office, advising then of the reentry. They, in turn, notified the respective County organizations to coordinate the reentry activities. Discussions were held in the State EOC relative to how these procedures would be inglenented. The recovery and reentry denonstration by the REAC and the State EOC corrected a 4
weakness identified in the previous exercise.
The ECP staff denonstrated their recovery and reentry responsibilities in the area of traffic control and naintenance of orderly reentry. The Illinois State Police are also available to assist local police departnents in conducting investigations.

DeWitt County Activation and Staffing We direct around the clock connunication link between the State and DeWitt County is nonitored by the Sheriff's Dispatch office. At 0846 this office was advised that an " Unusual Event" had occurred at the Clinton Power Station and by 1921 the energency had escalated to a " Site Area Energency".

Overall the staff displayed a well coordinated ability to respond in an effective nanner to protect the population in the County during an energency.

Public Alerting and Instruction me County ESDA Coordinator ordered the Pronpt Alert and Notification Systen activated at 1955.
The systen was activated due to a protective action reconnendation fron the State at 1045 to evacuate the 0-2 nile radius around the Clinton Power Station. The DeWitt County ESDA Coordinator called the IBS station, WSOY, using the proper authentication code and by so doing corrected a weakness noted in the previous exercise. This denonstrated the ability to utilize appropriate prescripted nessages contained in the plan and ensure that the public receives tinely energency infornation.
Another weakness fron the previous exercise, which dealt with override of the cable TV for the City of Clinton, has been corrected by a plan revision. the protective action reconnendations were adjusted to compensate for the change in plant conditions and a shift in winds. The County displayed an ability to protect the affected population by supporting these reconnendations. Evacuation routes selected fran the plan were carefully evaluated and procedures established to ensure that any impedinants were removed so that an orderly and tinely evacuation could be inplemented.

Recovery and Reentry We IDC staff conducted a very infornative as well as a thought provoking discussion on recovery and reentry procedures. They considered uniqJe problens which could be anticipated with returning their connunity to a nornal condition should a release occur fron the Clinton Power Station. The County displayed an enthusian to further explore all aspects in depth and i
prepare thenselves for a nultitude of problens which could potentially arise during recovery and reentry. This denonstration corrected a weakness identified during the previous exercise.

Mutual Aid Counties Macon County

Se EOC was staffed by the County ESDA Coordinator, an IESDA representative and a secretary. t,pon receipt of the " Alert" notification, the ESDA Coordinator notified the County Board Chairnan and the County Sheriff. The EOC staff were knowledgable of their responsibilities and the County plan.

The County expressed concern over the periods of delay between the tine of the incidents at the utility and the tine the County was notified by DeWitt County. Bere was an elapsed time of approxinately one hour after the conditions were classified and Macon County was notiffed.

W e present EOC facility is tenporary and a new facility is planned. The present facility is snall but operational based on the snall nunber of personnel designated to staff the facility during an incident at the Clinton Power Station. There is sufficient furniture, lighting, telephones and adjacent work areas. A status board was clearly visible and was kept up to date on significant events by the EOC coordinator. The naps depicted the pitme EPZ, with sectors labeled, evacuation routes, relocation centers, access control points, radiological nonitoring points, and population by evacuation areas.
EOC staff denonstrated their ability to connunicate with the State and County EOCs by counercial telephone and the State ESDA net. Connunications with other organizations consists of back-up police radio and volunteer anatuer radio connunications. Connunications with the JPIC and EOCs include hard copy datafax capability. The datafax is reliable and reasonably fast.
Pdli: alerting and instruction for the portion of Macon County that is within the 10 nile EPZ (approxinately 130 persons) is initiated by DeWitt County in coordination with the State. The County is kept apprised of conditions and/or requests for support personnel as necessary.
Four traffic and access control points were nanned by Macon County personnel of the Sheriff's Departnent. There are appropriate and adequate resourses in terns of equipnent and personnel to keep the evacuation routes clear of inpedinents. There are presently no nobility inpaired individuals residing within that portion of Macon County that is located within the ten nile EPZ.

Platt County Activation and staffing of the Platt County EOC was acconplished according to the plan. Current call lists were used to notifiy the EOC staff of the sinulated energency. The County ESDA Coordinator has been in this position for approxinately six nonths. He dislayed knowledge of the plan as well as the ability to nanage the energency response.
